INTRODUCTION: Necrotizing fasciitis is a rare and severe infectious disease with infectious necrosis often extending in depth to the subcutaneous tissue in the absence of rapid medical-surgical treatment. We report two cases of necrotizing fasciitis of the thigh, which underline the principle clinical and therapeutic characteristics of this often underrated disease.
OBSERVATION: The first case concerns a young 21 year-old woman without remarkable medical history who, following injury from the thorn of a palm tree, developed an aerobic germ necrotizing fasciitis which regressed following medic…

Compartment syndrome in the thigh is an uncommon condition. Most of the patients have multiple injuries. The main etiological factors are ipsilateral fractures of the femur and isolated severe soft-tissue injuries following blunt trauma. If the diagnosis is made clinically, compartment syndrome is treated by lateral incision of the thigh with incision of the fascia lata and the lateral intermuscular septum. Dermatofasciotomy does not lead to severe complications. In the follow-up there is no functional loss or neurological deficit. The indications for decompressive fasciotomy should be broad.

A 61-year-old patient had complained of sciatica in the left lower limb for three years. The pain was located in the posterior area of the thigh and leg and radiated to the first toe. Examination showed a tumor of the middle part of the thigh. A sciatic nerve tumor was confirmed at operation. Pathological examination revealed a neurofibrosarcoma. The patient died from pulmonary metastasis 17 months later.

Proximal, posterior thigh wounds from oncologic or traumatic defects can be difficult wounds to reconstruct if local flap options have been sacrificed during the trauma or oncologic resection. Free flap options to cover these defects are also difficult because of the lack of convenient recipient vessels in the region. The authors present 2 cases (oncologic and traumatic) wherein a myocutaneous anterolateral thigh (ALT) flap was harvested and tunneled from the anterior muscle compartment to the posterior muscle compartment of the thigh through a medially based transmuscular tunnel, decreasing t…

Anastomotic hemangioma (AH) is a rare benign vascular tumor primarily occurring in the genitourinary tract; however, only two cases have been reported in the thigh. In this report, a 42-year-old female patient presented to the hospital for examination due to "a subcutaneous mass on the lateral aspect of the left thigh discovered 9 months ago, which has been gradually enlarging." Subsequently, the lump was removed via local surgery. Histological examination reveals: At low magnification, the tumor was situated within the superficial subcutaneous fascia layer, presenting a loose lobular structur…

Overuse injuries affecting the hip, groin, and thigh represent a significant burden in both endurance and field sport athletes. This narrative review examines the anatomy, biomechanics, and imaging manifestations of the common overuse injuries in these regions, with particular emphasis on magnetic resonance imaging (MRI) findings and their clinical correlation. We systematize injuries within the Doha classification framework, discussing adductor-related pain, pubic-related pain, iliopsoas-related pain, and inguinal-related pain, along with thigh-based pathology including proximal hamstring ten…

BACKGROUND AND OBJECTIVES: Standard treatment for extremity sarcoma is limb-sparing surgery often with radiation, but complications occur frequently. We sought to determine factors predictive of wound complications after thigh sarcoma resection and reconstruction while analyzing trends over time.
METHODS: We reviewed all thigh defects requiring plastic surgeon reconstruction following sarcoma resection at our institution from 1997 to 2014. Patient demographics, comorbidities, operative characteristics, multi-modality therapies, and complications were analyzed. Wound complications were: infect…

Penile cancer is a neoplasm that predominantly affects males in the sixth decade of life, with an incidence of .3-1 per 100,000. Traditionally, the treatment of the primary lesion has consisted of total or partial amputation of the penis. However, the psychological and functional impact has influenced the development of preservation techniques We present 2males with lesions on the glans diagnosed by biopsy of squamous cell carcinoma. The patients underwent glandectomy and reconstruction with free thigh skin graft. The pathological anatomy was superficial squamous cell carcinoma. 6 months later…

The anterolateral thigh (ALT) flap is a viable and reliable option for phalloplasty. The primary advantages of the ALT flap remain an inconspicuous donor site and flexibility in phallus length. The disadvantages of the ALT flap are a higher incidence of both flap and urethral complications compared with a radial forearm phalloplasty. Although the ALT phalloplasty can achieve the primary goals of standing micturition, penetrative intercourse, and an aesthetic phallus, multiple stages and revisions are often necessary. Careful patient selection is paramount in attaining acceptable results with t…

Quadriceps and hamstring injuries occur frequently in football and are generally treated conservatively. While return to competition following hamstring strains is relatively quick, a high rate of injury recurrence highlights the importance of targeted rehabilitation and conditioning. This review describes the clinical manifestations of thigh-related soft-tissue injuries seen in football players. Two of these-muscle strains and contusions-are relatively common, while a third condition-the Morel-Lavallée lesion-is a rare, yet relevant injury.

In brief A sailboarder suffered an acute brief thigh compartment syndrome following a severe quadriceps contusion. Physical signs and interstitial pressure readings helped in making a prompt diagnosis of this uncommon injury, and the patient recovered fully after fasciotomy.

Correction of sequelae of primary hip-buttock-thigh plasty has become a common challenge in aesthetic plastic surgery. Suggested techniques for dealing with this problem include denuding the skin at the depressed area, pulling thigh flaps upward and outward, using dermal buried flaps, and utilizing liposuction. Liposuction can be used successfully in combination with classic hip-buttock-thigh plasty to enhance the aesthetic result as well as to facilitate the surgical technique.
