Preoperative evaluation for thoracic surgery.

The goal of the preoperative evaluation for thoracic surgery is to assess and implement measures to decrease perioperative complications and prepare high-risk patients for surgery. Major respiratory complications, such as atelectasis, pneumonia, and respiratory failure, occur in 15% to 20% of patients and account for most of the 3% to 4% mortality rate. Development of pulmonary complications has been associated with higher postoperative mortality rates. Strategies aimed at preventing postoperative difficulties have the potential to reduce morbidity and mortality, decrease hospital stay, and im…

Unsuccessful omentopexy in thoracic surgery.

Omentopexy has improved the treatment of chronic empyema and postpneumonectomy bronchopleural fistula, which otherwise are difficult to manage. However, omentopexy is not effective in some patients. Four of 17 patients who underwent omentopexy in our institution between January 1978 and March 1994 did not respond to the treatment and died. In two patients, a dead space remained after surgery and there was insufficient sterilization. In one patient, a dead space appeared after surgery and it was impossible to control infection. The fourth patient had dehiscence of the anastomosis triggered by p

Forgotten pioneers in thoracic surgery.

The names of Carrel, Sauerbruch and Blalock are milestones in the early history of thoracic surgery. The recognition of contributions by less renowned contemporaries does not diminish the decisive part played by these founders of prestigious schools of thoracic surgery. Krönlein who in 1883 successfully performed what was probably the first lobectomy is never mentioned. The esophagectomy without thoracotomy publicized by Orringer in 1978 had already been done by Gray-Turner 47 years before, in 1931. McLean who discovered heparin, and Forssmann who introduced cardiac catheterisation by passing …

Thoracic Surgery in Small Mammals.

Advances in imaging, anesthesia, and surgical techniques have made thoracic surgery in small mammals clinically feasible, yet it remains challenging due to their small size, unique physiology, and the anatomic complexity of the thorax. Improvements in computed tomography, endoscopy, and perioperative care now allow safer and more precise diagnosis and treatment. Consequently, procedures such as midline thoracotomies for thymomas, pulmonary lobectomies, thoracoscopy, and even more specialized interventions like pacemaker implantation are now performed with increasing reliability. Nevertheless, …

Nerve Injuries During Thoracic Surgery.

This article explores nerve injuries during thoracic surgery, focusing on prevention, diagnosis, and management of key nerves at risk, including recurrent laryngeal, phrenic, vagus sympathetic chain, and brachial plexus nerves. It emphasizes the importance of anatomic knowledge and careful surgical techniques to minimize injury. The article offers practical strategies for managing nerve damage, from restoring vocal function to treating diaphragm paralysis, and highlights the role of early detection and individualized care. With a focus on reducing long-term complications, it provides a compreh…

Video-Assisted Thoracic Surgery Lobectomy.

Video-assisted thoracoscopic surgery (VATS) has been established as the surgical approach of choice for lobectomy in patients with early-stage non-small cell lung cancer (NSCLC). Patients with clinical stage I NSCLC with no lymph node metastasis are considered candidates for VATS lobectomy. To rule out the presence of metastasis to lymph nodes or distant organs, patients should undergo meticulous clinical staging. Assessing patients' functional status is required to ensure that there are no medical contraindications, such as impaired pulmonary function or cardiac comorbidities. Although variou

Robotic Surgery for Thoracic Disease.

Robotic surgeries have developed in the general thoracic field over the past decade, and publications on robotic surgery outcomes have accum … y of robotic surgery based on acceptable morbidity and mortality compared to open or video-assisted thoracic surgery (VATS). Furthermore, there are accumulated data to indicate longer operation times and shorter hospital stay in robotic surgery. However, randomized controlled trials between robotic and open or VATS procedures are needed to clarify the advantage of …

New haemostat in thoracic surgery.

INTRODUCTION: The authors present the results of a promising method to reduce peroperative bleeding in thoracic surgery within large thoracic surgical interventions. Usage of a resorbable haemostat on the basis of oxidized cellulose in the form of cotton wool may minimize the cause of bleeding, mainly in patients with post-inflammatory thoracic complications. The strong point of this material is its easy application and malleability. Another advantage is the long-term antibacterial effect caused by lowering pH during its biodegradation. AIM: Retrospective evaluation of post-operative blood lo…

Nonintubated anesthesia for thoracic surgery.

Nonintubated thoracic surgery has been used in procedures including pleura, lungs and mediastinum. Appropriate anesthesia techniques with or without sedation allow thoracic surgery patients to avoid the potential risks of intubated general anesthesia, particularly for the high-risk patients. However, nonintubated anesthesia for thoracic surgery has some benefits as well as problems. In this review, the background, indication, perioperative anesthetic consideration and management, and advantages and disadvantages are discussed and summarized.

[Major surgery in thoracic injuries].

Chest injuries have a high and steadily increasing incidence in western countries, but only some of the most common problems they create require an emergency thoracotomy or surgical video thoracoscopy. Flail chest, persistent pneumothorax, massive haemothorax, mediastinal emphysema, cardiac tamponade and intrathoracic foreign bodies can be identified as major surgical problems. Some of such patients (i.e. those with flail chest or foreign bodies) would be immediately candidates for major intervention. Other require fast but diagnostic procedures, because the choice of a therapy is dependent up

[Blood transfusion during thoracic surgery].

In 90 patients, to whom blood transfusion was done while the thoracic operation conduction, the blood hemoglobin (Hb) content was analysed. In the patients, to whom intraoperatively the blood preparations transfusion was done, Hb concentration in the blood was lower than in patients with similar blood loss, but who was not blood transfused. While the own blood transfusion conduction, which was reinfused from the pleural cavity, the Hb level lowering was not observed. In 1998, comparing with 1984, the blood transfusion rate had lowered from 14.7 to 4.1% while the intraoperative complications ra

Thoracic surgery: the irreversible evolution toward endoscopic surgery.

Endoscopic techniques have established themselves as an important means for reducing the traumatic impact of thoracic surgery on the patient. They considerably lighten postoperative pain and are cosmetically much more pleasing. Procedures such as lung biopsy, pericardial window, treatment of spontaneous pneumothorax, mediastinal benign tumour excision and sympathectomy have become routine, other such as oesophagectomy require further evaluation. Advanced procedures such as lobectomy and pneumonectomy require further clinical and technological development prior to a conclusive assessment.

Asian perspective in surgery: thoracic surgery in Turkey.

Turkey with a population of 78 million is located between Asia and Europe geographically and culturally. There are 577 active pure thoracic surgeon and 37 thoracic surgery teaching units. Thoracic surgeons usually deal with lung cancer patients due to relatively higher rate of tobacco usage as well as inflammatory diseases such as pulmonary hydatid disease, bronchiectasis and empyema. Minimally invasive thoracic surgery has been a new approach which is being adapted by increasingly more surgeons. There are a number of reasons to predict that the number of thoracic surgical cases will be increa…

Needlescopic video-assisted thoracic surgery for reversal of thoracic sympathectomy.

Thoracic sympathectomy is a commonly performed surgical procedure for the treatment of palmar hyperhidrosis. However, one major complication of such a procedure is compensatory truncal hyperhidrosis. We describe an extreme case of compensatory truncal hyperhidrosis and anhidrosis over the head and neck region which led to a heatstroke. Bilateral reoperative needlescopic video-assisted thoracic surgery was performed for the reversal of thoracic sympathectomy with an interposition intercostal nerve graft. The patient's truncal hyperhidrosis resolved gradually over 1 month following the reversal …

Case report: Post-thoracic surgery acquired venous thoracic outlet syndrome.

Thoracic outlet syndrome (TOS) is a rare entity responsible for the vascular and/or nervous symptoms of the upper limbs. Unlike the congenit … hrombosis. To the best of our knowledge, this is the first description of acquired venous TOS after thoracic surgery. Conservative treatment was found to sufficiently avoid the need for more invasive methods.

Failure to Rescue in Thoracic Surgery.

Historically, quality improvement efforts have focused primarily on preventing complications from occurring. In contrast, failure to rescue is an emerging quality metric that follows patients after a complication occurs and describes a system's ability to prevent subsequent mortality and rescue them from death. Future efforts to implement the rescue framework in thoracic surgery should consider selective care consolidation, standardized pathways for detection of clinical deterioration and subsequent escalation of care, distribution of best practices, and evaluation of patient factors that cont…
