The benefits of touched vertebra concept for the selection of the lowest instrumented vertebra in thoracic curves of adolescent idiopathic scoliosis.

PURPOSE: To assess the benefits of using the touched vertebra (TV) rule in Lenke classification for thoracic curves of adolescent idiopathic scoliosis (AIS). METHODS: A total of 141 AIS patients with thoracic curves determined by Suk classification were divided based on whether the lowest instrumented vertebra (LIV) matched the TV into groups of mTV (n = 84, LIV = TV), TV- (n = 21, LIV above TV), and TV+ (n = 36, LIV below TV) for 5-year follow-up. The radiological parameters were the central sacral vertical line (CSVL)-LIV distance, distal end vertebra rotation, coronal, and sagittal paramet

Pulmonary Calcium Phosphate Cement Embolism After Percutaneous Vertebroplasty for Thoracic Vertebrae Fractures.

Pulmonary cement embolism (PCE) is a rare but severe complication following percutaneous vertebroplasty (PVP). Calcium phosphate cement (CPC) has emerged as an alternative to traditional materials for vertebral augmentation. There appear to be no established guidelines for managing symptomatic PCE, and there is scarce literature on CPC embolisms. This is a first report of a case of pulmonary CPC embolism following PVP. The patient, a 63-year-old Chinese female, was administered anticoagulant treatment and achieved a satisfactory outcome. Her case highlights the severe potential morbidity assoc

Osteoporotic Fractures of the Thoracic and Lumbar Vertebrae: Diagnosis and Conservative Treatment.

BACKGROUND: The prevalence of osteoporotic vertebral body fractures in Europe is 18-26%. Although most of these injuries can be treated conservatively, the underlying concepts have not been defined clearly or uniformly. In this article, we present the current state of the evidence on the diagnosis and conservative treatment of osteoporotic fractures of the thoracic and lumbar vertebrae. METHODS: A systematic review of the literature up to May 2020 was carried out in the PubMed and Web of Science Core Collection databases. 549 articles were identified, of which 36 were suitable for inclusion i

Mini Transsternal Approach to the Anterior High Thoracic Spine (T1-T4 Vertebrae).

Purpose. The anterior high thoracic spine is one of the most complex segments to be accessed surgically due to anatomical constraints and transitional characteristics. We describe in detail the mini transsternal approach to metastatic, infective, traumatic, and degenerative pathologies of T1 to T4 vertebral bodies. We analyse our surgical series, indications, and outcomes. Methods. Over a 5-year period 18 consecutive patients with thoracic myelopathy due to metastatic, infective, traumatic, and degenerative pathologies with T1 to T4 vertebral bodies involvement received a mini transsternal app

Imaging diagnosis of monostotic fibrous dysplasia in thoracic and lumbar spine vertebrae.

The X-ray radiograph, CT scan and MRI appearance of 5 patients with pathologically proven fibrous dysplasia in thoracic and lumbar spine vertebrae were retrospectively analyzed. Plain radiographs, CT scans and MR images showed the presentation of eccentric lesion with intact cortex bone and marginal sclerosis in vertebral bodies without involvement of vertebral appendix and extraosseous soft tissue. The lesion masses were round (one being oval-shaped) and radiolucent in plain radiographs and CT scans. Homogeneous long signal was observed on T1 weighted image and strongly enhanced when gadolini

Multifocal extensive spinal tuberculosis (Pott's disease) involving cervical, thoracic and lumbar vertebrae.

An unusual case is reported of a 53-year-old woman presenting with spinal tuberculosis involving cervical, thoracic and lumbar vertebrae. The patient originally presented with progressive quadriparesis, fever, night sweats and weight loss. Imaging studies demonstrated vertebral body destruction with and without paraspinal and/or intraspinal abscess in cervical, thoracic and lumbar regions. The laboratory studies confirmed the diagnosis of Pott's disease and the patient was placed on anti-tuberculosis chemotherapy. The anterior approach was used for removal of the infected necrotic material, bo

Comparative finite element analysis of the first thoracic vertebra in artiodactyls.

Artiodactyls exhibit a striking diversity of the cervical vertebral column in terms of length and overall mobility. Using finite element analysis, this study explores the morphology at the cervico-thoracic boundary and its performance under loads in artiodactyls with different habitual neck postures and body sizes. The first thoracic vertebra of 36 species was loaded with (i) a compressive load on the vertebral body to model the weight of the head and neck exerted onto the trunk; and (ii) a tensile load at the spinous process to model the pull via the nuchal ligament. Additional focus was laid

Optimal Lowest Instrumented Vertebra for Thoracic Adolescent Idiopathic Scoliosis.

STUDY DESIGN: Retrospective cohort chart review. OBJECTIVE: To determine the optimal lowest instrumented vertebra (LIV) following posterior segmental spinal instrumented fusion (PSSIF) of thoracic adolescent idiopathic scoliosis (AIS) with LIV at L2 or above. SUMMARY OF BACKGROUND DATA: Few studies evaluate the optimal LIV based on rotation or center sacral vertical line (CSVL). METHODS: A radiographic assessment of 544 thoracic major AIS patients (average age 14.7 years) with minimum 2 years' follow-up (average 4.1 years) after PSSIF was performed. The LIV was divided by CSVL: stable verte

Intraosseous Schwannoma Arising within the Thoracic Vertebra: A Case Report.

INTRODUCTION: Schwannomas are common benign tumors that develop in the peripheral nerves and are common in intradural extramedullary lesions of the spine. However, intraosseous schwannoma is extremely rare. CASE PRESENTATION: A 24-year-old man experienced back pain for 5 months. Imaging revealed a lytic lesion with marginal sclerosis in the body of the 12th thoracic vertebra, with vertebral collapse, thinning, and bulging of the posterior margins of the vertebral body. Needle biopsy suggested a benign neurogenic tumor, and a total en bloc spondylectomy was performed. Postoperative pathologica

Telangiectatic osteosarcoma of the thoracic vertebra: a case report and literature review.

Telangiectatic osteosarcoma (TOS) is a rare high-grade malignant tumour found primarily in the metaphysis of long bones, but can also occur in the pelvis, spine and skull. Characterized by extensive vascular dilatation and aggressive behavior, TOS often has a poor prognosis and may be misdiagnosed due to similarities with other lesions. This report presents a case of TOS of the thoracic vertebrae and reviews the relevant literature to enhance clinical awareness. A 44-year-old woman presented with chest and back pain of worsening over two months, accompanied by lower limb numbness and incomplet

Thoracic Vertebra Chance Fracture Resulting from Mechanical CPR.

Chest compressions, used in cardiopulmonary resuscitation (CPR), cause rib and sternum fractures in around 79% and 54% of patients, respectively. Spinal fractures resulting from CPR are far rarer. We present the case of a 70-year-old man who underwent mechanical CPR after choking whilst eating. The patient received a cerebral and thoracic CT scan upon arrival to the hospital. The cerebral scan was normal, but the chest CT scan revealed signs of ankylosing spondylitis and an unstable Chance fracture of the 12th thoracic vertebra. The patient was hospitalised but passed away. This case highlight
