The true triplication of the thumb: a case of unclassified thumb polydactyly.

A child with true triplication of the thumb is reported. Each thumb has a well-developed metacarpal, and one of them is biphalangeal; the remaining 2 are triphalangeal. Satisfactory result was obtained from a functional and esthetic point of view. Our surgical approach to this interesting anomaly and the necessity of a new group under the Wassel classification for the categorization of different forms of thumb triplications is discussed.

Primary toe-to-thumb transfer in the acutely avulsed thumb.

The case is presented of a primary toe-to-thumb transfer by microvascular anastomosis for reconstruction of an acute degloving amputation of the thumb. This approach is not recommended as the standard practice for this type injury, however, there were certain advantages in this patient: (1) one stage procedure, (2) restoration of length, sensibility, and cover, (3) cosmetic improvement, and (4) minimal length of hospitalization.

Selected outcomes of thumb replantation after isolated thumb amputation injury.

PURPOSE: The aim of this study was to assess thumb survival, pinch strength, grip strength, and need for secondary surgery in patients undergoing thumb replantation after isolated thumb amputation injury. METHODS: We conducted a retrospective review of 52 consecutive isolated thumb replantations performed over a 4.5-year period. Charts were reviewed for mechanism of injury, level of amputation, and surgical technique. Primary outcomes of interest included survival and secondary surgery (eg, tenolysis, neurolysis) rates. Functional outcome was assessed by pinch and grip strengths after a mean …

[Thumb reconstruction with cryopreserved autogenous thumb skeleton and microneurovascular flap].

This paper describes a procedure for thumb reconstruction using autogenous cryo-conserved bone from the thumb itself. In a case of severe explosion injury, the thumb and index finger were amputated at the carpometacarpal joint level and the remaining fingers at the distal interphalangeal joint level. Severe soft tissue damage and lesion of both the superficial and deep palmar arterial arches rendered customary surgical procedures for thumb reconstruction impossible. The cryo-conserved bone was replanted following soft tissue healing and then covered with a free radial forearm flap. This proced…

Oligodactyly with Thumb.

BACKGROUND: Oligodactyly of the hand is one of the rarest congenital anomalies of upper extremities and defined as the presence of fewer than five fingers on a hand. Although it usually occurs in association with hypoplasia or absence of ulna, it can occur without abnormality of the forearm bones. The purpose of this study is to present clinical features and radiographic characteristics of hand oligodactyly with thumb. METHODS: Five patients of oligodactyly with thumb who showed normal forearm bones, were evaluated. Two patients had threefingered hand with thumb, and three had two-fingered ha…

Thumb replantation or not?

Over a three-year period 14 consecutive cases of amputated thumbs, 10 complete and 4 incomplete, were replanted/revascularized. Eight thumbs (57%) survived. These cases were compared with a control group, in which replantation had failed or had not been attempted. The control group was chosen to be as similar as possible with respect to thumb amputation level, type of trauma, age, occupation and sex. Costs for hospital care, duration of sick leave and disability rate were calculated. Hand function was estimated as the ROM of the thumb, power of pinch and grip, two-point discrimination and abil…

[The triphalangeal thumb].

A theory about formal aetiology of triphalangeal thumb is emphasized. All teratological grades of triphalangism of the thumb associated with preaxial polydactyly or reduction abnormality of the thumb are to be submitted to our diagram of thumb malformations.

The thumb in syndactyly.

Syndactyly of the thumb is a rare but very serious malformation. It produces a loss of the grasping function of the thumb, which is the main function of the human hand. The author presents different methods for the reconstruction of the first web space and a functional thumb. The surgical methods of reconstruction depend upon the extend and type of the malformation. The opening of the deep intermuscular fascia of the first web is a most important step of the procedure.

Radiographic Characteristics of the Triphalangeal Thumb in Patients With Congenital Thumb Duplication: An Analysis of 383 Thumbs in 368 Children.

PURPOSE: The purpose of this study was to evaluate the morphologic and radiographic characteristics of the triphalangeal thumb (TPT) in patients with congenital thumb duplication. METHODS: We retrospectively reviewed 368 children with TPT duplication (n = 383). Data on age, sex, and laterality were obtained from medical records. The morphologic and radiographic characteristics of the TPT duplication were evaluated using plain radiographs. RESULTS: Of the 383 fingers, 322 (84%) had a TPT duplication on the radial side, 36 (10%), on the ulnar side, and 25 (7%) were bilateral. The mean age at t…

The hypoplastic thumb.

Thumb hypoplasia is part of a spectrum of radial longitudinal deficiencies involving the upper limb. Systemic involvement of other organ systems is not uncommon, thus requiring a team approach to management. Because of the variety of anatomic abnormalities associated with thumb hypoplasia, clinical and intraoperative evaluation of the thumb must be precise. Effective management of thumb hypoplasia requires an understanding of the embryology, epidemiology, classification, presentation, and management options. Management, which is primarily determined by the grade of thumb hypoplasia, may includ…

Triplication of the thumb.

A child with triplication of the thumb is presented. Each thumb was fully developed with neurovascular bundles, flexor and extensor tendons. Although all three thumbs were triphalangeal, they shared a common metacarpal and two shared a common proximal phalanx. Since this anomaly does not fit in the existing accepted Wassel (1969) classification, a new category is suggested.

Other Thumb Deformities.

The thumb accounts for approximately 40% of overall hand function, and deformities carry significant implications for pediatric hand development and quality of life. This article reviews three distinct congenital and acquired thumb deformities: pediatric trigger thumb, triphalangeal thumb, and congenital clasped thumb. Pediatric trigger thumb, an acquired condition resulting from a size mismatch between the flexor pollicis longus tendon and the A1 pulley, is staged using the Sugimoto classification and can resolve spontaneously, though surgical release of the A1 pulley reliably restores full r…

Thumb Carpometacarpal Instability.

The basal joint complex of the thumb provides the framework necessary for function of the human hand. Although its unique saddle articulation allows for a wide range of motion necessary for routine function of the hand, it is rendered inherently unstable because of poor bony congruency and reliance on its capsuloligamentous support. Painful instability of this joint can stem from several causes including traumatic dislocation, various hypermobility conditions, and chronic overuse and microtrauma. A thorough history and examination as well adequate imaging is necessary for proper evaluation of …

Thumb duplication/polydactyly.

A significant lesson for those assessing the work of others stems from the necessity to review what has been written before. It remains a common but human error to misinterpret or misrepresent the words of others and indeed, for inconsistencies in these to become apparent. This is certainly so in the field of congenital hand surgery and especially so with discussion of thumb duplication/polydactyly. Many of the presentations in this journal issue deal with the subject of thumb duplication. All involve a consideration of classification despite an emphasis on investigative measures and technique…

Developmental Trigger Thumb.

Developmental trigger thumb, although uncommon, can be easily identifiable in the pediatric outpatient visit. Patients often present with their thumb locked in flexion and a firm nodule at the base of the thumb. The thumb is usually passively correctable and nonpainful. It is important to examine the opposite thumb as bilateral trigger thumbs occur at a rate of 25% to 30%. Nonsurgical options have been proposed in the past including watchful waiting, extension exercises, splinting, and steroid injections with mixed results. Surgical intervention is indicated when there is painful triggering or…
