Celiac disease (CD) is a gluten related disorder which affects all age-groups and occurs in genetically susceptible population after introduction of gluten in diet. The worldwide prevalence of CD is ~1% and it is higher in certain "at-risk groups". The clinical features are variable, ranging from classical diarrhea to an asymptomatic state. Diagnosis requires serology and duodenal histology although a non-biopsy diagnosis is recommended by European Society of Pediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) for a select group of children. Treatment of CD is with a life-long stric

Patients with glomerular disease (glomerulonephritis, GN) experience complex care transitions that increase the risk of fragmented care, delayed decision-making, and inadequate preparedness. Within the Mid-Atlantic Glome rulonephritis Network (MAGNET), an interdisciplinary nephrology care team identified the absence of a standardized approach to support patients across key transition points, including transplant referral, dialysis planning, and pediatric-to-adult care. In response, the PATHWAY GN protocol was developed as an interdisciplinary transitional care model embedded within routine nep…

BACKGROUND: Due to advances in the treatment of pediatric brain tumors (PBT), an increasing number of patients are experiencing the transition from the pediatric to the adult health care system. This requires efficient transitional models.
METHODS: We systematically reviewed the literature regarding PBT concerning different transitional models and aspects of the transitional period. For this purpose, PubMed, Medline, and Embase databases were searched systematically through January 2022.
RESULTS: We reviewed a total of 304 studies, of which 15 were ultimately included. We identified five tra

Significant progress has been made in the understanding of transitional care in rheumatology over the last few decades, yet universal implementation has not been realised and unmet needs continue to be reported. Possible explanations for this include lack of evidence as to which model is most effective; lack of attention to the multiple dimensions, stakeholders and systems involved in health transitions; and lack of consideration of the developmental appropriateness of transition interventions and the services/organisations/systems where such interventions are delivered.Successful transition h…

BACKGROUND: Hirschsprung disease (HD) and anorectal malformations (ARM) are congenital disorders with potentially lifelong consequences. Although follow-up is performed in most pediatric patients, transfer to adult health care is often problematic. This study assesses transitional care with the help of questionnaires in consultation with adult patients.
METHODS: This study was conducted in an outpatient clinic of a pediatric surgical center in the Netherlands. All patients born and treated for ARM or HD before 1992 were invited to visit our clinic. Patients completed questionnaires concerning…

Readmissions to the hospital are common and costly, often resulting from poor care coordination. Despite increased attention given to improving the quality and safety of care transitions, little is known about patient and provider perspectives of the transitional care needs of rehospitalized Veterans. As part of a larger quality improvement initiative to reduce hospital readmissions, the authors conducted semi-structured interviews with 25 patients and 14 of their interdisciplinary health care providers to better understand their perspectives of the transitional care needs and challenges faced…

PURPOSE: Rural patients face an increased risk of 30-day mortality after hospitalization, compared to their urban counterparts. The Department of Veterans Affairs (VA) Geriatric Learning Health System's operational partners sought to identify care-related and contextual factors associated with mortality among rural patients being discharged from hospital to home.
METHODS: We examined clinical notes from medical records of 200 (N = 100 rural; N = 100 urban) Veterans ≥65 years-old who were discharged alive to home after a medical or neurological index admission but who died within 30 days of di

This study, conducted in parallel with the Multisite Replication of the Transitional Care Model, evaluated implementation of the Transitional Care Model protocol delivery (primary aim) and the randomized clinical trial protocol (secondary aim) across nine hospitals and community partners within four healthcare systems. Guided by the Practical, Robust, Implementation, and Sustainability Model and applying a concurrent mixed-methods design, outcomes were assessed using an embedded Reach, Adoption, Implementation, and Maintenance framework, excluding effectiveness. Fidelity to implementation was …

BACKGROUND: Transcatheter aortic valve replacement (TAVR) volumes continue to expand, increasing the need for optimized periprocedural and transitional care. Readmissions have substantial clinical and financial impact.
PROJECT RATIONALE: In 2018-2019, Lake Charles Memorial Hospital's 30-day post-TAVR readmission rate was 12.7%, with 58% unrelated to valve complications, prompting a quality-improvement initiative.
PROJECT SUMMARY: A phased, multidisciplinary pathway incorporated standardized discharge education, early follow-up, conduction system disease monitoring, social determinants of hea…

Interhospital transfers (IHT) are indispensable for providing timely access to specialized services, particularly for rural populations and high-acuity conditions such as trauma, stroke, and myocardial infarction. However, IHTs introduce substantial risks, including transport complications, care discontinuity, increased costs, prolonged hospitalization, and elevated mortality. Although Emergency Medical Treatment and Labor Act and Centers for Medicare & Medicaid Services regulations establish baseline safeguards, coordination deficiencies persist. Centralized transfer centers, standardized doc

BACKGROUND: Transitions of care are defined as critical moments along the care continuum for patients as they navigate changes in their health status or level of care. Researchers suggest that the existing research on transitions of care is largely descriptive and lacking in theoretical depth, which limits its application to different clinical contexts.
AIM: We therefore aim to review the existing literature and theoretical frameworks on transitions of care, with a focus on primary care settings.
METHOD: We undertook a systematic hermeneutic review - an approach that emphasises analytical re

OBJECTIVES: Bronchopulmonary dysplasia (BPD) remains one of the most important complications of extreme prematurity and a leading cause of long-term respiratory, cardiovascular, and neurodevelopmental morbidity. Increasing evidence suggests that BPD should be viewed not only as a parenchymal lung disorder but as a complex cardiopulmonary syndrome involving disrupted vascular development, abnormal transitional circulation, and ventricular dysfunction. This review aimed to summarize current evidence on the hemodynamic mechanisms underlying BPD, emphasizing pulmonary vascular disease (PVD), bronc

OBJECTIVES: Patients with congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD) account for most 30-day hospital readmissions nationwide. The Coordinated-Transitional Care (C-TraC) program is a telephone-based, nurse-driven intervention shown to decrease readmissions in Veterans Affairs (VA) and non-VA hospitals. The goal of this project was to assess the feasibility and efficacy of adapting C-TraC to meet the needs of complex patients with CHF and COPD in a large urban tertiary care VA medical center.
DESIGN: We used the Replicating Effective Programs model to guide…

Most juvenile idiopathic arthritis (JIA) patients need to attend adult rheumatology centres to continue the clinical management of their disease and to receive adequate long-term treatment. Transition from the paediatric to the adult health care team is a critical moment in the clinical history of these patients, but unfortunately, about 50% of the transfer processes to adult rheumatology are not successful, putting these patients at high risk of unfavourable outcomes. There are several obstacles to the success of transitional care for JIA patients, such as the absence of specific criteria for…
