Trigeminal neuralgia.

We completed a retrospective study of 270 patients with essential trigeminal neuralgia treated by gamma knife surgery. The target was localized on the retro-Gasserian portion of the nerve with 90 Gy at maximum. Among them, 150 patients were followed up more than 2 years. They were divided into 2 groups: pre-APS (41 patients) and post-APS (109 patients) treated by model C-APS since 2003. In the post-APS group, initial electric discharge was observed in 98.2% and at the last follow-up of at least 2 years in 79.8%. Complete recurrence was observed in 4.7% and postoperative complications were obse…

Trigeminal neuralgia.

Trigeminal neuralgia (TN) is the most common facial neuralgia, and is considered to be one of the most painful conditions to affect patients. The rate of occurrence of TN in men and women is 2.5 and 5.7 per 100,000 per year respectively. TN is generally characterized by lancinating, unilateral, paroxysmal pain occurring in the distribution of the fifth cranial nerve. The diagnosis of TN is made clinically by excluding other possible causes of facial pain and is based on signs and symptoms from the patient history such as a trigger zone, typical unilateral lancinating paroxysms following neural…

Trigeminal Neuralgia.

The initial treatment for trigeminal neuralgia is medical. Carbamazepine is the drug of choice. If the patient proves to be intolerant of carbamazepine, a number of second-line drugs are available, though data on their relative efficacy are nonexistent. Phenytoin, baclofen, clonazepam, and sodium valproate are all worthy of consideration. Oxcarbazepine may be as effective as carbamazepine, but its availability is limited. Newer agents being tried in this condition include lamotrigine and gabapentin. Their comparative value has not been established. For patients resistant to or intolerant of dr…

Trigeminal neuralgia.

Trigeminal neuralgia is a sudden, severe, brief, recurrent, stabbing pain in the distribution of the trigeminal nerve. It is a rare condition whose etiology remains unknown. Diagnosis is by careful history and there are few investigations that are of value in the management of this condition. Management is initially medical with anticonvulsant drugs being the primary drugs. If the drugs are no longer effective or tolerated then surgical management needs to be considered. Surgery can be at a variety of different levels along the pathway of the trigeminal nerve. Each procedure has its advantages…

Trigeminal neuralgia.

Severe trigeminal neuralgia (TN) is probably the most excruciating pain experienced in the orofacial region. Apart from the primary idiopathic form (ITN), this lancinating pain may be secondary to a range of underlying organic diseases (STN). Diagnosis requires differentiation of these and elimination of other causes of orofacial pain.

Trigeminal neuralgia.

Although the full and precise etiology of TN still awaits further elucidation, this extremely painful disorder can be treated successfully by a variety of techniques, pharmacologic and surgical, with minimal side effects. There is no excuse in the 1990's for patients with TN not to be diagnosed promptly and treated expeditiously and successfully.
