Instrumented stabilization in spinal tuberculosis.

Spinal tuberculosis (TB) produces neurological complications and grotesque spinal deformity, which in children increases even with treatment and after achieving healing. Long-standing, severe deformity leads to painful costo-pelvic impingement, respiratory distress, risk of developing late-onset paraplegia and consequent reduction in quality and longevity of life. The treatment objective is to avoid the sequelae of neural complications and achieve the healed status with a near-normal spine. In TB, the spine may become unstable if all three columns are diseased. Pathological fracture/dislocatio

[Spinal cord involvement by tuberculosis].

INTRODUCTION: We are observing an increase of patients with tuberculosis in Spain. Within the forms of extrapulmonary presentation, the involvement of the central nervous system by the bacillus that cause this disease is a relatively frequent location. Although sometimes is affected the spinal cord of secondary form, rarely is observed isolated involvement. DEVELOPMENT AND CONCLUSIONS: We describe the diverse forms of spinal cord tuberculosis presentation, attending of its location within the spinal cord or adjacent annexes, in order that the clinician knows and remembers this infrequent but

Chemotherapeutic treatment for spinal tuberculosis.

AIM: To evaluate whether 6 months of chemotherapy for patients with spinal tuberculosis prevents relapse as effectively as more than 6 months of chemotherapy. METHOD: Literature review. Medline search including references, from January 1978 to November 2000. Inclusion criteria for publications: diagnosis of spinal tuberculosis confirmed bacteriologically and/or histologically, or probable on the basis of clinical and radiological parameters; treatment regimen (whether or not in combination with surgery) included isoniazid (H), rifampicin (R) and pyrazinamide (Z); follow-up period after comple

Another look at spinal tuberculosis.

The steady decline in tuberculosis rates in Canada has led to a certain lack of awareness of the disease. We describe 7 patients with culture proven spinal tuberculosis, seen in a teaching hospital over a 10-year period. Persistent spinal pain and local tenderness were the most frequent presenting findings. There was a mean delay in diagnosis of 5.2 (range 1-18) months from the time of presentation. Only 2 patients had active extraspinal tuberculous infection. Plain spinal radiographs were the initial diagnostic procedure of choice showing a destructive vertebral lesion in 5 of 7 patients. Com

Computed tomography of spinal tuberculosis.

The computed tomographic (CT) features of nine documented cases of spinal tuberculosis are analyzed. The mechanisms of vertebral involvement are reviewed. The ability of CT to facilitate the diagnosis as well as to delineate the extent of soft tissue involvement is emphasized. The need for increased alertness to the diagnosis of tuberculosis is stressed.

Spinal cord compression from spinal tuberculosis in a child.

Tuberculosis in children most frequently involves the pulmonary system. Spinal tuberculosis is very rare and insidious in children. We present a 1-year-old girl who was brought to our clinic with complaints of a bulge on her back and weakness in the feet. She was investigated with magnetic resonance imaging and at the thoracic level which revealed vertebral destruction and para- and perispinal abscess. Computed tomography of the lungs showed tuberculous lesions. The child's mother gave a post history of recovered lung tuberculosis. Due to appearance paraparesis secondary to vertebral destructi

Spinal tuberculosis in an infant associated with maternal urinary tuberculosis.

A ten-month-old infant who presented with regression of milestones and seizures was noted to have a gibbus deformity in the upper thoracic region. She was diagnosed to have spine and central nervous system tuberculosis by culture of pus from the paravertebral abcess which showed a growth of Mycobacterium tuberculosis. The mother, who had been having recurrent episodes of Urinary tract infection, was diagnosed to have Urinary TB proven by culture. Spinal tuberculosis, though rare, can be encountered in infancy and should be kept in mind while treating infants presenting with related symptoms.

Tuberculosis of the spine and spinal cord.

Tuberculosis remains an important problem in underdeveloped countries, especially in individuals under the age of 20 years. In developed countries, there was an increase of the disease due to several factors, especially the appearance of A.I.D.S. The arterial haematogenous seeding is the most important way to the propagation of the disease to the rachis. The bacillus mycobacterium tuberculosis is characterized by a slow growth, propention for an oxygen rich environment and the absence of proteolytic enzymes. These characteristics are responsible for the imaging particularities of tuberculous o

The problem of deformity in spinal tuberculosis.

Spinal tuberculosis is the most common cause for a kyphotic deformity in patients in many parts of the world. There is an average increase of 15 degrees deformity in all patients who are treated conservatively and 3% to 5% of patients may end up with a deformity that is greater than 60 degrees. The progress of deformity occurs in two distinct phases: Phase I, which includes the changes in the active phase, and Phase II, which includes changes after the disease is cured. The progress of deformity is influenced by the severity of the angle before treatment, the level of the lesion, and age of th

Imaging of musculoskeletal and spinal tuberculosis.

The diagnosis of tuberculosis of the musculoskeletal system is difficult for many reasons. As Walker states, to diagnose tuberculosis one must consider the possibility. The uncommonness of osteoarticular MTb results in clinician inexperience, which leads to overlooking the diagnosis. Subtle early manifestations may elude detection. Negative skin tests and normal chest films do not exclude the consideration of tuberculosis. The most conclusive means of reaching the diagnosis (biopsy and culture) necessitate invasive procedures that are not always definitive, and may require repeated attempts. M

Torticollis as a sign of spinal tuberculosis.

Bone localization of tuberculosis mainly affects the thoracolumbar spine. The cervical spine is rare. Its diagnosis is often late which exposes to great instability and potentially serious complications. We report the case of a 12-year old girl with no medical history, showing torticollis and high temperature without neurological complication. In the physical examination, he had torticollis and pain in the third, fourth and fifth cervical vertebra. When the biopsy was performed, we find an inter apophysis (between C7 and D1) collection. The histological examination confirmed the diagnosis of a

Spinal Tuberculosis with Paraplegia in Pregnancy.

Spinal tuberculosis leading to paraplegia is uncommon in pregnancy and is a diagnostic and therapeutic challenge. We report a case of tubercular paraplegia presenting at 35 weeks of gestation. She was managed with Anti-tubercular drugs and did not require surgical intervention. Her neurological status improved and she was allowed to go in labour. She delivered a healthy term infant by cesarean. At three months follow-up, both mother and child are doing well.

Prediction of deformity in spinal tuberculosis.

Tuberculosis of the spine may cause kyphosis, which may in turn cause late paraplegia, respiratory compromise, and unsightly deformity. Surgical correction therefore may be considered for large or progressive deformities. We retrospectively analyzed clinical and radiographic parameters to predict the final kyphotic angle in spinal tuberculosis and to identify patients at risk of unfavorable outcomes at an early stage of the disease when surgery may be indicated. Unfavorable outcome was defined as progression of 10 degrees or greater and/or a final angle of 40 degrees or greater. We analyzed 53
