Recent advances in improved tuberculosis vaccines.

Tuberculosis continues to be a major infectious cause of global morbidity and mortality, both in children and adults, in spite of widespread … sis control strategies. This unfortunate state of affairs has driven an intensive search for better tuberculosis vaccines. This article reviews the various vaccine development strategies which are being used e.g., purification or synthesis of protein peptide and non-peptide antigens from M. tuberculosis, creation of rationally-attenuated mutants of BCG and M. tuberculo …

Challenges in developing new tuberculosis vaccines.

Tuberculosis (TB) is a preventable and curable disease caused by the bacillus Mycobacterium tuberculosis. In 2022, according to the World Health Organisation (WHO), TB was the second leading cause of death worldwide caused by a single infectious agent, after coronavirus disease (COVID-19). Brazil is ranked among the 30 countries with the highest TB burden. Currently, the neonatal Bacillus Calmette-Guérin (BCG) is the only vaccine against TB and offers significant efficacy against disseminated and meningeal disease in children. However, BCG has a limited efficacy in preventing adult-type cavita

New Drugs and Vaccines for Tuberculosis.

BACKGROUND: Tuberculosis (TB) is an infectious disease that resulted in estimated 9.6 million new cases in 2014 and 1.5 million deaths. The available drug regimen for TB is time consuming which more often leads to the patient non compliance which then results in occurrence of drug resistant TB (Multi-drug and extremely drug resistant TB) in several portions of the world. METHODS: The dangerous combinations of TB and HIV is taking its toll on human health. The foremost factor is non- profit associated with the development of anti TB drugs. There is almost 10 different drugs in various levels o

Tuberculosis vaccines: beyond bacille Calmette-Guerin.

Tuberculosis (TB) disease caused by Mycobacterium tuberculosis (M. tb) remains one of the leading infectious causes of death and disease throughout the world. The only licensed vaccine, Mycobacterium bovis bacille Calmette-Guérin (BCG) confers highly variable protection against pulmonary disease. An effective vaccination regimen would be the most efficient way to control the epidemic. However, BCG does confer consistent and reliable protection against disseminated disease in childhood, and most TB vaccine strategies being developed incorporate BCG to retain this protection. Cellular immunity i

Novel tuberculosis vaccines on the horizon.

Eleven new tuberculosis (TB) vaccines are in various phases of clinical trials. These include subunit vaccines to improve the current vaccine BCG, and recombinant BCG to substitute for BCG, both given pre-exposure to prevent active disease. A plethora of potential candidates have reached various stages of the pre-clinical development pipeline, some ready to enter Phase I clinical trial soon. A boost vaccine to top up the immunity of existing BCG is on the horizon and will have to suffice until a better candidate to replace BCG is ready. The live mutants of Mycobacterium tuberculosis show great

Development of vaccines against bovine tuberculosis.

Bovine tuberculosis caused by Mycobacterium bovis remains an economically important problem in Great Britain with potential zoonotic consequences, and the incidence is rising exponentially. In 1997 an independent scientific review recommended that the best option for disease control in Great Britain was the development of a cattle vaccine. Bovine tuberculosis remains a significant problem in countries of the developing world. Indeed, more than 94% of the world's population live in countries in which the control of bovine tuberculosis in buffalos or cattle is limited or absent. Effective vaccin

Advances in protein subunit vaccines against tuberculosis.

Tuberculosis (TB), also known as the "White Plague", is caused by Mycobacterium tuberculosis (Mtb). Before the COVID-19 epidemic, TB had the highest mortality rate of any single infectious disease. Vaccination is considered one of the most effective strategies for controlling TB. Despite the limitations of the Bacille Calmette-Guérin (BCG) vaccine in terms of protection against TB among adults, it is currently the only licensed TB vaccine. Recently, with the evolution of bioinformatics and structural biology techniques to screen and optimize protective antigens of Mtb, the tremendous potential

Tuberculosis vaccines: current status and future prospects.

There is an urgent need to develop more effective tuberculosis vaccines as chemotherapy and Bacille Calmette-Guérin (BCG) have failed to control the current epidemic. BCG does have some protective effect in childhood, so using a second vaccine to boost BCG would be the most ethical and logistically feasible strategy. The cost of tuberculosis efficacy trials will be high and return on investment into the development of a tuberculosis vaccine will be low. Incentives such as orphan drug status could encourage industrial interest. As more vaccines enter into early clinical trials, there is an urge…

New vaccines for the prevention of tuberculosis.

Mycobacterium bovis, bacille Calmette-Guérin (BCG) is administered widely to newborns throughout the world and has been shown to be effective in preventing childhood tuberculosis but not reactivation pulmonary disease or human immunodeficiency virus-associated tuberculosis. Development of a more effective, better standardized, affordable vaccine with durable activity and fewer side effects is a major priority. Contemporary molecular techniques have identified promising immunodominant antigens and novel immunization strategies. Vaccine development has also been informed by an improved understan

Tuberculosis vaccines: the past, present and future.

Tuberculosis still remains a leading infectious cause of death worldwide, although the BCG vaccine has been used for 80 years. There is an urgent need to develop improved BCG or new tuberculosis vaccines. This apparently represents a daunting task, since it will take a long time before a vaccine can be declared to be better than the current BCG vaccine, both in experimental and human studies. The current review takes a brief historic look at the use of current BCG vaccine and provides an overview on what are considered to be the key immunologic criteria that have to be met by a new generation …

Tuberculosis vaccines: developmental work and the future.

The last year in tuberculosis vaccine research has witnessed the initial flowering of the benefits promised by the tuberculosis genome sequencing product. Although the real benefits in terms of clinical treatments are yet to be realized, genomics is making its presence felt in the rapid identification and expression of proteins with vaccine potential from Mycobacterium tuberculosis, the definition of species-specific antigens for diagnostic use, and the construction of a variety of novel living vectors for vaccination. At the same time, the recent increase in work on animal models with more di

Toward tuberculosis elimination: An update on tuberculosis vaccines in clinical trials.

Despite major advances in diagnosis and treatment, tuberculosis (TB) control - and ultimately elimination - will remain unachieved without a vaccine capable of preventing pulmonary disease and transmission. Bacille Calmette-Guérin (BCG), a live attenuated vaccine derived from Mycobacterium bovis and the only licensed TB vaccine, has been widely implemented because of its proven efficacy against severe childhood TB. However, despite global coverage approaching 90%, BCG provides limited and inconsistent protection against pulmonary TB in adolescents and adults, the populations that sustain trans

Tuberculosis vaccines update: Is an RNA-based vaccine feasible for tuberculosis?

OBJECTIVES: Despite concerted efforts, Mycobacterium tuberculosis (M.tb), the pathogen that causes tuberculosis (TB), continues to be a burden on global health, regaining its dubious distinction in 2022 as the world's biggest infectious killer with global COVID-19 deaths steadily declining. The complex nature of M.tb, coupled with different pathogenic stages, has highlighted the need for the development of novel immunization approaches to combat this ancient infectious agent. Intensive efforts over the last couple of decades have identified alternative approaches to improve upon traditional va

New tuberculosis vaccines for people with HIV.

BACKGROUND: In 2024, tuberculosis (TB) caused 160 000 deaths in people with HIV (PWH). Several new TB vaccines are in development. The best way to use new vaccines to reduce the TB burden in PWH, and how this may vary by setting, is unknown. METHODS: We used a mathematical model to simulate TB vaccination in 15 high HIV/TB burden countries with varying HIV prevalence and ART coverage. We estimated the impact of vaccination with 50% efficacy against TB disease targeted by HIV status and explored how the impact depends on assumptions about vaccine efficacy by HIV and ART status. RESULTS: Vacci
