Urinary tract tomography.

The application of the priniciples of linear tomography to enhance the visualization of the kidneys during excretory urography represents a major advancement in the quality of excretory urography. The ability to visualize the renal parenchyma and renal outlines on tomograms has resulted in a significant increase in the detection of renal masses, parenchymal scars, and adrenal tumors. The use of tomography may also contribute significantly to the evaluation of patients with various other urinary tract diseases. It has been our practice during the past 5 years to obtain linear tomograms as a rou…

Urinary Incontinence and Urinary Tract Infections.

Urinary incontinence results from disorders of the lower urinary tract or neurologic diseases either of the nerve supply to the bladder/urethra or within the central nervous system. Congenital causes include patent urachus and ectopic ureter. Coordination of lower urinary tract function involves the interaction of both the sympathetic and parasympathetic system as well as somatic branches of the central nervous system. Well-recognized causes of incontinence include equine herpes virus 1 myeloencephalopathy, polyneuritis equi (neuritis of the cauda equina), and sacral/coccygeal trauma. Idiopath…

[Urinary cytology in tumors of the urinary tract].

Views on the usefulness of urinary cytology and the best methodological technic differ widely. We perform the examination with spontaneously voided urine; after centrifugation, a smear of the sediment is Papanicolaou-stained. With this method 15 patients were found over a two-year period to have a tumor of the urinary tract or a positive urinary cytology. In 13 patients a tumor was diagnosed; of these 10 had a positive, 1 a questionable and 2 a false negative cytology. In 2 patients the cytology was false positive. Retrospective reexamination of the two false negative results revealed unequivo…

Maternal Urinary Tract Infection: Is It Associated With Neonatal Urinary Tract Infection?

Objective: Maternal urinary tract infection is associated with intrauterine growth restriction, preterm delivery and low birth weight. The purpose of this study was to evaluate whether maternal urinary tract infection is related to neonatal urinary tract infection. Materials and methods: The present prospective study included 230 singleton neonates. The participants were divided into two groups based on in utero exposure to maternal urinary tract infections. The study group (exposure to maternal urinary tract infection) included 115 neonates and the control group (without exposure to maternal …

Renal and Urinary Conditions: Urinary Tract Infections.

Urinary tract infections (UTIs), including cystitis and pyelonephritis, are common. Each year, they account for more than 10 million outpatient visits and more than 3 million emergency department visits. Recurrent UTIs (defined as three in 1 year or two in 6 months) also are common, occurring in 20% to 30% of women. The annual incidence of UTIs is 12.1% among women and 3% among men. Cystitis symptoms include lower abdominal pain, dysuria, and urinary urgency or frequency. Escherichia coli is the most common pathogen. Cystitis often is diagnosed inappropriately when patients have asymptomatic b…

[Complicated urinary tract infections].

Urinary tract infections (UTIs) are among the most common bacterial infections in Germany, both in outpatient and inpatient settings. The clinical spectrum is heterogeneous and ranges from benign, often self-limiting, uncomplicated infections to complicated urinary tract infections (cUTIs) with life-threatening courses. Complicated urinary tract infections have a higher risk of chronicity, recurrence and/or progression than uncomplicated infections and may be associated with severe disease progression. Stratification of patients with urinary tract infections is, therefore, of utmost importance…

Prenatal Urinary Tract Dilatation.

Urinary tract dilatation (UTD) is the most common congenital anomaly detected on prenatal ultrasonography (US), affecting 1% to 3% of all pregnancies. This article focuses on the prenatal detection of UTD and the postnatal evaluation and management based on the UTD grading system risk assessment. Prophylactic antibiotics and postnatal imaging are discussed. The recent management trend is for a more conservative approach to minimize unnecessary testing and exposures to the fetus and neonate while detecting those who may have clinically significant disorder. The renal bladder US remains a critic…

[Pediatric urinary tract catheters.].

Urinary tract catheters are hollow and flexible tubes used to facilitate urine output or to achieve urine transit through them. We can find different types of catheters depending on the purpose they have been designed for: urethral, cystostomy and ureteral catheters among others. We review the indications for insertion of these catheters, their designs, composition, covering materials, insertion techniques and associated complications. Pediatric catheters are behind what is produced for adults, so we perform a bibliographic review about urinary catheters in general with emphasis on the pediatr…

Nosocomial urinary tract infection.

Nosocomial urinary tract infection, a common complication in surgical patients, is primarily related to the use of indwelling urinary catheters. Discontinuation of catheter usage within 2 days, whenever possible, is the cornerstone to avoiding these infections. Patients with asymptomatic bacteriuria may be treated with catheter removal only, and do not necessarily require antibiotic therapy. Patients with symptomatic infections should receive effective antimicrobial therapy, but removal of the catheter is also fundamental to clearing the urinary tract of infection. Antibiotic therapy of urinar…

Recurrent urinary tract infections.

Urinary tract infection (UTI) is one of the most common bacterial infections in women, and one in four of these women will develop a recurrence. Various risk factors predispose women of different age groups to recurrence. These factors include sexual intercourse, use of contraception, antimicrobials, oestrogen, genetics, and the distance of the urethra from the anus. Of the different pathogens, Escherichia coli is the organism most commonly isolated. A variety of treatment options has been proposed, including long-term or post-intercourse prophylaxis and patient-initiated therapy. Oestrogen an…

[Recurrent urinary tract infections].

Recurrent urinary tract infections (RUTI) are a frequent clinical problem in sexually active young women, pregnant or postmenopausal women and in patients with underlying urological abnormalities. The present chapter reviews RUTI based on their classification: relapses, which usually occur early (< 1 month), are caused by the same microorganism and are associated with underlying urological abnormalities, and reinfections, which usually occur later and are caused by a new distinct microorganism (or by the same microorganism usually located in the rectum or uroepithelial cells). The pathogenesis…

Complicated urinary tract infections.

Complicated urinary tract infections (UTIs) are defined as those infections causing severe complications, such as urosepsis, renal scarring, and end-stage renal disease. Major complicating factors are infection stones, infected atrophic kidney, renal papillary necrosis, indwelling catheters, and other causes of bacterial persistence, as well as vesicoureteral reflux and urinary tract obstruction in recurrent UTI. Operative therapy is mandatory in all types of persistent UTI besides chronic bacterial prostatitis, as well as in UTI associated with urinary tract obstruction. Recurrent UTI complic…
