INTRODUCTION: Silent uterine perforation following aspiration for molar pregnancy is a rare but critical complication that can significantly impact maternal prognosis. Early detection and prompt intervention are crucial to prevent severe outcomes. This case highlights the importance of vigilant follow-up and clinical suspicion in managing such cases.
CASE PRESENTATION: A 53-years-old patient, presented 1 month after an aspiration procedure on a molar pregnancy with abdominal pain, physical examination found tachycardia, hypotension, minimal vaginal bleeding and significant pain on uterine mob…

Uterine perforation is an uncommon but potential risk during all intrauterine procedures. We display a set of images from ultrasound, hysteroscopy, and laparoscopy, as well as a video from laparoscopy, pertaining to a case of uterine perforation with omental adhesions. The complication was diagnosed several months after dilatation of the cervix and curettage of the uterus following a missed miscarriage. This is a rare but serious complication following a commonly performed procedure and the case highlights the importance of investigating new symptoms even after a seemingly uncomplicated proced…

BACKGROUND: Specimens of endometrial biopsies can sometimes present with an artifact within blood, composed of optically clear vacuoles mimicking adipose tissue, pseudolipomatosis. This artifact can be mistaken for adipose tissue and lead to an overdiagnosis of uterine perforation.
CASE: We describe the case of pseudolipomatosis seen within the evacuated products of conception from a missed abortion.
RESULTS: Areas of vacuolization in the blood clot mimicked adipose tissue. However, the vacuoles varied in size and did not contain adipocytes.
CONCLUSION: Familiarity with this artifact will l…

Pyometra is the accumulation of pus in the uterine cavity, and spontaneous perforation of pyometra resulting in generalized diffuse peritonitis is extremely uncommon. We report a rare case of diffuse peritonitis caused by spontaneous perforation of pyometra. A 66-year-old postmenopausal woman with diffuse abdominal pain and vomiting was admitted to our institution. She had a history of mixed connective-tissue disease and had been taking steroids for 20 years. Under a diagnosis of generalized peritonitis secondary to perforation of the gastrointestinal tract or uterus, supravaginal hysterectomy

INTRODUCTION: Pyometra is an accumulation of purulent material or pus in the uterine cavity. Spontaneous perforation of uterus by pyometra is rare. This is a clinical presentation and management of a spontaneous perforation of uterine caused by pyometra.
CASE PRESENTATION: This is a case report on spontaneously perforated associated with pyometra secondary to cervical malignancy. The patient underwent exploratory laparotomy with total hysterectomy and bilateral salpingo-oophorectomy.
CONCLUSIONS: Spontaneous rupture of pyometra duo to cervical cancer in cases of acute abdomen in elderly pati

BACKGROUND: Uterine perforation is a rare but recognized complication of abortion. Perforations may not be recognized at the time of the procedure, and patients may present days or weeks later with sequelae of the complication.
OBJECTIVES: To raise awareness of this rare complication that can present days to weeks after the precipitating event.
CASE REPORT: A 21-year-old woman presented 3 weeks after an elective abortion with symptoms of bowel obstruction. Exploratory laparotomy revealed small bowel herniation into a perforated uterus, causing the obstruction. In retrospect, a pre-operative …

Two cases of uterine perforation are described, occurring 11 days and 4 months, respectively, after the insertion of GyneFix, a frameless intra-uterine contraceptive device (IUD). In both the cases initial ultrasound scan showed the intra-uterine position of the device. Removal of the IUD, either by laparoscopy or laparotomy, had to be carried out. Awareness of this complication, insertion of GyneFix by a trained operator, appropriateness of ultrasound scan monitoring and possible underreporting of this complication are discussed.

Since 1965 there has been a substantial increase in the number of women in the United States who use the intrauterine contraceptive device (IUD). A collaborative multicenter case--control study was conducted to examine uterine perforation and uterine incarceration as well as several other suspected complications related to use of an IUD. To determine which attributes of the IUD and which user characteristics contribute to the risk of uterine perforation and incarceration, the authors analyzed 32 women with uterine perforation requiring transperitoneal removal, 106 women with uterine incarcerat…

A case of ectopic endometrium in the omentum following perforation of the uterus by a Lippes Looptm is reported. Evidence is presented substantiating Sampson's theory of endometrial implantation.

Generalized peritonitis with sepsis is a common general surgery emergency. The most likely implicated structure for generalized peritonitis with pneumoperitoneum is the gastrointestinal tract with urgent explorative laparotomy being the most definitive treatment. In this particular case, perforated diverticulitis was suspected and upon an exploration of the abdomen, frank pus in the setting of normal colon was noted. Some common differential diagnoses for frank pus in the abdomen include viscus perforation, pancreatic necrosis, gangrenous cholecystitis, or penetrating abdominal trauma. Here, w

UNLABELLED: The evisceration of the bowel through the vaginal vault is an extremely rare condition and a surgical emergency with a high-reported mortality rate. Vaginal evisceration most commonly affects menopausal women with a hysterectomy or those with previous vaginal surgery. The most common risk factors include the triad of post-menopausal atrophy, previous vaginal surgery and enterocele. Estrogen deficiency in post-menopausal women leads to weaker pelvic support structures and a thin, atrophic vagina, making it more prone to rupture. Previous vaginal surgery leaves scar tissue with dimin

From 1981 to 1988, 32 uterine perforations per 44,052 artificial abortions (0.07%) were recorded. Also three more perforations having occurred outside the Department were treated. In two patients the perforation was done by a probe and the evacuation of the uterus was completed vaginally. Six perforations were in the parametrium, producing no injury of the serosa; three of them required laparotomy because of abundant bleeding (laparotomy had to be applied previously in the two) and in the remaining 27 patients the uterine corpus was perforated either by a dilatator, forceps, curette or aspirat…

UNLABELLED: Uterine perforation is a rare but potentially life-threatening complication of gynecologic procedures. Serious complications include hemorrhage, infection, and injury to surrounding organ systems (eg, gastrointestinal, urological, vascular, etc.). Risk factors include advanced maternal age, prior gynecologic surgeries, and other anatomical features that impact the difficulty of accessing the uterine cavity. In this case report, we discuss a patient who presented to the emergency department (ED) with diffuse abdominal pain and vaginal bleeding that occurred after an elective dilatio…

A 29-year-old parous woman with a history of a T-shaped copper intrauterine device (IUD) insertion presented 8 months later with a complaint of the contraceptive device being missing. Computed tomography with contrast turned out to be superior to the combined abdominal and pelvic X-ray and transvaginal ultrasound in providing the detailed extrauterine location of the device between the urinary bladder and uterus. A laparoscopy was successful in the atraumatic freeing of the IUD from omental and bladder adhesions, and in its final removal.
