We report the case of a 6-year-old girl who presented in December 2002 with fever and a vesicular rash. Initially she was diagnosed with Stevens-Johnson syndrome. The differential diagnosis was expanded, however, when an exposure to a person who was believed to be vaccinated recently against smallpox was revealed. We describe the sequence of events that ensued and the workup for a possible case of disseminated virus.

A 21 year old woman received a smallpox revaccination during the fifth to sixth month of pregnancy. She developed accidental vaccinia by autoinoculation and, five weeks later, gave birth to a healthy child.

A thirty year old woman in whom an uncommon complication of smallpox vaccination developed is presented herein. In this case, virus was transmitted from the recently vaccinated child to the vulva and the vagina of the incompletely immune mother. The diagnosis was confirmed by a viral culture and a direct smear. The patient was treated intravenously with cytosine arabinoside (Cytosar) 3 mg/kg and was completely cured after seven days.

The problems of vaccination in pregnancy are discussed with regard to the specific immunological circumstances of pregnancy and the immunological relationship between the mother and the fetus. In the whole medical literature only 18 cases of fetal vaccinia are described. The author emphasizes that fetal vaccinia is a complication of primovaccination of a pregnant woman. Experiments on rabbits disclosed that the suppression of cellular immunity in the mother may cause prolanged virusemia and thus favour intrauterine infection. Pregnancy is an absolute contraindication for primovaccination again…

A recently reported case of progressive vaccinia (PV) in an immunocompromised patient has refocused attention on this condition. Uniformly fatal prior to the licensure of vaccinia immune globulin (VIG) in 1978, PV was still fatal in about half of VIG-treated patients overall, with a greater mortality rate in infants and children. Additional therapies would be needed in the setting of a smallpox bioterror event, since mass vaccination following any variola virus release would inevitably result in exposure of immunocompromised people through vaccination or contact with vaccinees. Well-characteri…

BACKGROUND The emergence of the monkeypox virus (mpox) is causing a large-scale re-administration of vaccinia-based vaccines. Many physicians have not been exposed to the rare, but implicit, complications, revealing a glaring need for updated evidence and re-examination. We present a case of a Human Immunodeficiency Virus (HIV)-positive male patient who presented to the Emergency Department (ED) with vaccinia symptoms several days after receiving the JYNNEOS vaccine. CASE REPORT A 45-year-old man with a past medical history of well-controlled HIV presented to the ED for 5 days of nocturnal dia…

Using vaccinia immune globulin (VIG), a high-titer antibody preparation from immunized subjects, we demonstrate that the humoral immune response in humans is directed against numerous antigens in the Dryvax vaccine strain. Western blot and immunoprecipitation analyses revealed highly antigenic proteins associated with both the extracellular enveloped virus and intracellular mature virus forms. The modified vaccinia virus Ankara (MVA), a new generation smallpox vaccine that is attenuated for replication in humans, expresses most, but not all, of the major vaccinia antigens recognized by antibod…
