OBJECTIVE: To study and evaluate the therapeutic effects of external valvuloplasty for deep venous valve insufficiency of the legs.
METHODS: External valvuloplasty of the femoral vein valve was performed in 26 patients (37 legs) with deep venous valve insufficiency of the legs. Among them, 12 were male and 14 were female. These patients ranged in age from 16 to 69 years (x +/- s, 50.8 +/- 10.0). External valvuloplasty of the first pair of the superficial femoral vein valve was carried out in all the legs. External valvuloplasty of the second pair of the superficial femoral vein valve was adde

Doppler and photoplethysmography studies were compared with invasive venous pressure measurements in ten healthy volunteers and 20 patients with a postthrombotic syndrome to assess the accuracy of noninvasive studies in detecting venous valve incompetence in the lower extremity. Using a refilling time of 20 seconds or less for valve incompetence, photoplethysmography was as accurate as invasive pressure measurements. In the Doppler studies there were two false-positive and three false-negative studies as compared with the invasive method. Photoplethysmography is an accurate and simple method f

The aim of this work is to demonstrate the possibility to identify persistent right sinus venous valve and to differentiate it from other right sided heart pathology by two dimensional echocardiography. We report the echocardiographic findings observed in three out of 215 paediatric patients we examined for clinically suspected congenital heart disease in 12 months period. The first patient was a 24 hours old newborn with transient pulmonary hypertension; the second one was a 6 months old child with pulmonary atresia, severe right ventricular hypertrophy, atrial septal defect and patent ductus

We studied the function of right internal jugular vein valves during cardiac catheterization in 32 patients and external jugular vein valves … ained competent during pulsatile transvalvular pressure of 64.8 +/- 1.9 mm Hg. Thus, thoracic inlet venous valves are usually competent during sudden increases in intrathoracic pressure. These valves may play an important role in establishing the extrathoracic arteriovenous pressure gradient necessary for forward blood flow during cardiopulmonary resuscitation …

OBJECTIVE: To determine the role of microvenous valves in the superficial venous system in the prevention of reflux and skin changes in the progression of venous insufficency.
METHODS: The venous anatomy of 15 amputated lower limbs, eight free from clinical venous disease and seven with varicose veins and ulcers, was examined using retrograde venography corrosion casting. Prior to amputation, all limbs were scanned by duplex ultrasound to confirm the presence or absence of reflux in the great (GSV) and small saphenous veins or their tributaries. The resulting resin casts were photographed and…

Ten cadaveric great saphenous vein systems have been dissected to assess the relationship between the valves and the termination of venous tributaries. Such tributaries were classified on the basis of size, competence and course, and the percentage of each of four classes terminating within 1 cm of each valve site has been assessed. Above the knee, valves were more numerous, closer together, and two-thirds of competent tributaries terminated within 1 cm of them, this relationship being weaker for other classes of tributary. Below the knee, less than 50% of any class of tributary was related to

PURPOSE: Options for pediatric valve replacement are limited by inability to adapt to child growth. An expandable valve design is presented in which human cryopreserved venous valved conduits are constructed within a bare-metal stent scaffold. We demonstrate functionality in vitro and acute competence in vivo.
DESCRIPTION: A cryopreserved femoral venous valved conduit was compressed within a bare-metal stent scaffold and sutured at the proximal and distal ends. In vitro testing was performed at low and high pressure after sequential balloon expansion. Valve morphology was measured in the nond

Chronic deep venous insufficiency is a debilitating disease with limited therapeutic interventions. A bioprosthetic venous valve could not only replace a diseased valve, but has the potential to fully integrate into the patient with a minimally invasive procedure. Previous work with valves constructed from small intestinal submucosa (SIS) showed improvements in patients' symptoms in clinical studies; however, substantial thickening of the implanted valve leaflets also occurred. As endothelial cells are key regulators of vascular homeostasis, their presence on the SIS valves may reduce the obse

PURPOSE: To develop a system for potential use in the treatment of chronic venous insufficiency by using percutaneous techniques.
MATERIALS AND METHODS: A segment of a glutaraldehyde-fixed bovine external jugular vein with valves was trimmed and sutured to a nitinol stent. Animals were premedicated and anesthetized (n = 11). Venography of the right external jugular vein, inferior vena cava (IVC), and common iliac vein was performed. Deployment was accomplished via a sheath (12-24 F) with use of fluoroscopic guidance. Eleven bioprostheses were deployed in 11 animals. Bioprostheses were deploye

We describe the rare case of a 76-year-old man with left partial anomalous pulmonary venous connection with intact atrial septum. He experienced dyspnea due to significant aortic stenosis and mitral regurgitation, and required valve repair. Preoperative computed tomography revealed connection of the left upper pulmonary vein to the innominate vein. Although right heart overload was not recognized, we repaired the left partial anomalous pulmonary venous connection with concomitant aortic valve replacement and mitral annuloplasty, because of the potential for unpredictable harmful effects of a r

Incomplete involution of valve of systemic venous sinus can present across a spectrum of anatomical lesions ranging from eustachian valve to division of right atrium (cor triatriatum dexter) with overlapping features. We present the case of a neonate presenting with cyanosis, having persistent valve of systemic venous sinus with anatomical details of the redundant tissue in right atrium suggesting an intermediate form between Chiari network and division of right atrium.

We report a case of infective endocarditis at the tricuspid valve attributed to central venous catheterization. The patient was a 35-year-old woman who had multiple septic emboli in her lung due to tricuspid valve endocarditis after successful treatment of bronchiolitis obliterans organizing pneumonia. She also had right ileosacral arthritis. The case was closely related to catheter-associated Staphylococcus aureus bacteremia. She was treated with intravenous administration of vancomycin and surgical removal of vegetation and tricuspid valvuloplasty. Since infective endocarditis can be a compl

PURPOSE: Chronic deep venous insufficiency, usually secondary to the postthrombotic syndrome, is due to primary valve failure in approximately 15% of cases. In these cases surgical repair of the valvular mechanism may be indicated.
METHODS: Fifty-two limbs in 42 patients were treated with superficial femoral vein valvuloplasty after appropriate investigation. Adjunctive operations on the superficial or perforating veins were performed on 49 limbs during the same hospital admission.
RESULTS: Of the 27 patients who have completed one year of follow-up, 85% are free of reflux on duplex scanning

PURPOSE: A phase I feasibility study was conducted to determine whether cryopreserved venous valved segments would remain patent/competent in a short-term period (6 months).
METHODS: The target group consisted of 10 patients (C(4-6), E, A(D), P(R)). The exclusion criteria included untreated superficial/perforator venous disease, significant venous or arterial obstruction, hypercoagulability or coagulopathy, and significant preexisting medical conditions. Required preoperative tests were venous duplex, ascending/descending venography, and a physiologic study (eg, APG, blood typing, an ankle/br

PURPOSE: The durability of the variety of valve reconstruction techniques in "primary" reflux and postthrombotic reflux was studied.
METHODS: A total of 423 valve repairs in 235 patients with a follow-up period ranging from 1 to 12 years were analyzed. End points for assessment consisted of ulcer recurrence and Doppler competence in serial duplex examination. Multivariate analysis with Cox proportional hazards model was used.
RESULTS: Ulcer-free survival curves were similar for "primary" and postthrombotic reflux. No significant difference in ulcer recurrence was seen regardless of the techn
