Successful termination of combined rapid atrial flutter/fibrillation and ventricular tachycardia by intravenous sotalol.

Combined rapid atrial flutter/fibrillation and recurrent ventricular tachycardia occurred in an 82 year old man with acute myocardial infarction. Both arrhythmias were promptly terminated by intravenous sotalol, suggesting another use for this unique drug in the absence of hypotension, heart block or cardiac failure.

[Atrial flutter with 1/1 nodo-ventricular conduction with amiodarone. From physiopathology to diagnosis].

Atrial flutter with 1/1 nodo-ventricular conduction is a classical complication of Vaughan-Williams's Class I antiarrhythmic drugs. The increase of the flutter cycle and weak action of the antiarrhythmic on the atrioventricular node leads to 1/1 conduction of atrial depolarisation to the ventricles. In view of their marked action on the atrioventricular node, this type of pro-arrhythmic effect is very unexpected with Class III antiarrhythmics. The authors report 7 cases of 1/1 atrial flutter with oral amiodarone observed between 1994 and 2001. The patients were 6 men and 1 woman with an averag

Atrial fibrillation and flutter with left bundle branch block aberration referred as ventricular tachycardia.

Five patients were referred for electrophysiologic evaluation of nonsustained or sustained ventricular tachycardia. In each patient, the clinical rhythm disturbance was reproduced and identified as atrial fibrillation or flutter with left bundle branch block aberrancy. All five patients demonstrated enhanced or accelerated atrioventricular conduction through the normal atrioventricular nodal-His Purkinje pathway. This rapid conduction created an electrophysiologic substrate suitable to the preferential development of this less common form of aberration. Four of five patients responded well (ve

Hemodynamics of cough cardiopulmonary resuscitation in a patient with sustained torsades de pointes/ventricular flutter.

A 43-year-old female with old myocardial infarction and stenosed bypass grafts developed sustained Torsades de Pointes/ventricular flutter (rate = 300-400 beats per minute) during coronary arteriography after contrast injection to the diagonal graft. Cough-CPR (rate = 37/min) was started within 5 s of dysrhythmia initiation and continued through two defibrillation attempts (200 and 360 joules), and IV lidocaine was administered until return of spontaneous circulation 62 s later. The patient never lost consciousness during this very rapid dysrhythmia. Certain cardiac arrest resuscitation measur…

Acceleration of the ventricular response to atrial flutter by amiodarone in an infant with Wolff-Parkinson-White syndrome.

Amiodarone resulted in a rapid ventricular response to atrial flutter in an infant with Wolff-Parkinson-White syndrome.

Dilated phase of hypertrophic cardiomyopathy caused by Fabry disease with atrial flutter and ventricular tachycardia.

We describe a case of a 60-year-old male with dilated phase of hypertrophic cardiomyopathy caused by Fabry disease. He was diagnosed to have a cardiac variant of Fabry disease by an enzyme assay and a right ventricular endomyocardial biopsy which revealed specific features of this disease and cardiac involvement was the sole manifestation. He has developed dilated cardiomyopathy with sustained atrial flutter and frequent non-sustained ventricular tachycardia requiring isthmus ablation and cardiac resynchronization therapy with defibrillator.

A patient with sick sinus syndrome, atrial flutter and bidirectional ventricular tachycardia: Coincident or concomitant presentations?

Channelopathies are among the major causes of syncope or sudden cardiac death in patients with structurally normal hearts. In these patients, the atrium, ventricle or both could be affected and reveal different presentations. In this case, we present a patient with an apparently structurally normal heart and recurrent syncope, presented as sick sinus syndrome with atrial flutter and bidirectional ventricular tachycardia. (Cardiol J 2007; 14: 585-588).

The value of rate regularity and multiplicity measures to detect ventricular tachycardia in the presence of atrial fibrillation or flutter.

The predominant cause of inappropriate therapy by implantable antitachycardia devices with pacing and nonpacing cardioverter defibrillators, is mistaking a fast ventricular response during atrial fibrillation or flutter with true ventricular tachycardia (VT). The distinction between these arrhythmias is an important consideration in addressing the problem of reducing false-positives in detection mechanisms for implantable devices. Dual chamber analysis that examines atrial and ventricular event ratios has been proposed as a solution to this problem, but would still fail in distinguishing parox

Pulmonary edema after cardioversion for paroxysmal atrial flutter: left ventricular diastolic dysfunction induced by direct current shock.

This report describes a patient with the pulmonary edema after cardioversion for paroxysmal atrial flutter without organic heart disease. A 68-year-old man was admitted to hospital for paroxysmal atrial flutter. Antiarrhythmic agents were not effective, and direct current cardioversion was performed on the 4th hospital day. Three hours after cardioversion, the patient complained of dyspnea, and a chest X-ray showed pulmonary edema. He responded to oxygen, intravenous furosemide and drip infusion of nitroglycerine. During tapering of the medication, his condition remained stable. The patient wa

Recurrence of paroxysmal atrial fibrillation or flutter after successful cardioversion in patients with normal left ventricular function.

One hundred twenty-four consecutive patients (85%) with paroxysmal atrial fibrillation (AF) and 21 (15%) with atrial flutter (AFI) were studied immediately after pharmacologic or electrical cardioversion to sinus rhythm. Mean age was 59 +/- 13 years (range 23 to 79). Patients with reduced left ventricular function were excluded from the study. After restoration to sinus rhythm, the clinical course of all patients was followed for the first recurrence of paroxysmal AF or AFI irrespective of the therapeutic approach. Mean follow-up was 23 +/- 16 months. After 12 months of follow-up, 50% of all p

Catheter ablation of typical atrial flutter in a patient with left ventricular assist device support: a case report.

BACKGROUND: Atrial arrhythmias are a frequent and clinically significant complication in left ventricular assist device (LVAD)-supported patients, with the potential to impair right ventricular (RV) function, compromise device hemodynamics, and reduce quality of life. While atrial fibrillation (AF) has received considerable attention in this population, the distinct hemodynamic impact of typical cavotricuspid isthmus (CTI)-dependent atrial flutter and the role of catheter-based rhythm control remain poorly characterized. CASE PRESENTATION: A 60-year-old male with non-ischemic cardiomyopathy w
