Cogan's Syndrome. High resolution MRI indicators of activity.

In the chronic-relapsing form of Cogan's syndrome, it can be difficult to evaluate the activity of the disease. In contrast to the initial stage, routine diagnostic techniques sometimes fail to indicate progression in the chronic stage. To determine whether high-resolution magnetic resonance imaging (HR-MRI) can be used to differentiate between active and inactive stages, we examined three patients with Cogan's syndrome (one during an acute relapse, two with chronic audiovestibular deficits), all of whom had antibodies to inner ear tissue (cochlea, vestibular labyrinth). Unenhanced T1-, T2, ga

The vestibular system in the elderly.

Aging in the peripheral vestibular apparatus is shown to consist of processes similar to those occurring in other parts of the nervous system. Sensory epithelium and primary efferents degenerate and are not replaced. Type I hair cells and elements of the pars inferior are most susceptible. Remaining cells show a build-up of lipofuscin and microstructural changes suggesting compromised metabolism. Rupture of the saccular membrane and microfractures of the otic capsule are frequently seen. Otoconia undergo degeneration, and the tissue between the endolymphatic duct and bony vestibular aqueduct b

[Development of the osseous structures of internal ear in human foetuses].

PURPOSE: Assessment of development of selected osseous structures of internal ear (cochlea, vestibule, modiolus) in human foetuses aged between 9-32 weeks was the aim of this study. Human osseous labyrinth is a very interesting albeit difficult research subject. Current study represents a trial to summarize the knowledge about its development in prenatal period available so far. At the same time it underlines the importance of research on the development of ear's structures for prevention of hearing disorders as well as for their early development and treatment. METHODS: Descriptive methods:

Anatomical studies of the posterior petrous apex with regard to hearing preservation in acoustic neuroma removal.

Some surgeons have shown that tumors of the internal auditory canal and cerebellopontine angle may be removed with preservation of hearing through the suboccipital approach. If hearing is to be conserved, the cochlear division of the VIIIth cranial nerve and blood supply of the labyrinth must be preserved. In addition, surgical entry into the labyrinth, upon removal of the posterior wall of the internal auditory canal, must be avoided since it is likely to result in permanent sensorineural hearing loss. Careful anatomic dissection of 20 human temporal bones has shown that exposure of the later

Direction-Changing Positional Nystagmus in Acute Otitis Media Complicated by Serous Labyrinthitis: New Insights into Positional Nystagmus.

OBJECTIVE: To demonstrate characteristic nystagmus findings in acute otitis media (AOM) complicated by serous labyrinthitis and discuss the mechanism of direction-changing positional nystagmus (DCPN) in this condition. PATIENTS: A patient with AOM complicated by serous labyrinthitis on the left side. INTERVENTION: Video nystagmography and 3D fluid attenuated inversion recovery (FLAIR) magnetic resonance imaging (MRI). MAIN OUTCOME MEASURES: Characterize positional nystagmus in a head-roll test observing the change of nystagmus direction in process of time and compare findings of temporal bo

Peripheral vestibular disorder of cervical origin.

The provoking mechanism of peripheral vestibular disorder in the cervicovestibular syndrome is discussed on the basis of the results of perivascular sympathectomy of the vertebral artery. The peripheral type of this syndrome can be differentiated from the central type in that it shows fatigue due to positioning nystagmus and the reversion phenomenon in the self-recording cupulogram. 42 cases of the peripheral type could be completely cured of episodes of vertigo by the unilateral operation, but the other 6 cases required bilateral operations to prevent such episodes.

Hypertrophic Pachymeningitis of the Internal Auditory Canal Induced by a Labyrinthine Fistula Complicated With Cholesteatoma.

Hypertrophic pachymeningitis (HP) is defined by inflammation and thickening of the dura mater, and the etiologic factors are idiopathic or secondary to various conditions. To date, HP in the internal auditory canal (IAC) has rarely been reported. There have only been 3 reports of HP in the IAC. Magnetic resonance imaging showed enhancement of along the IAC and vestibule. After antibiotic treatment, enhancement was reduced with visible seventh and eighth nerves. The patient underwent tympanomastoidectomy. To our knowledge, this is the first case of HP associated with a labyrinth fistula complic

The knotted wire in stapes surgery: one possible factor in postoperative sensori-neural hearing loss.

In four instances of revision stapedectomy in patients with sensori-neural hearing loss, with or without dizziness, a wire prosthesis was found with an extention beyond the knot which could impinge on the contents of the vestibule. Four commercially manufactured knotted wire prostheses were examined, and the end of three was found projecting beyond the knot. The wire piercing the membranous labyrinth appears to be one of the factors in causing further sensori-neural hearing loss after stapedectomy. A wire loop or piston prosthesis should be considered to eliminate the hazard of a trailing end

Vestibular dysfunction presenting as syncope.

A case of chronic vestibulitis presented as episodes of recurrent drop attacks that had been interpreted as recurrent syncopal episodes. The importance of distinguishing between syncope and drop attacks-a difficult task-is emphasized. This case stresses the need to include benign vestibulitis in the differential diagnosis of syncope and drop attacks.

3D MRI of the membranous labyrinth. An age related comparison of MR findings in patients with labyrinthine fibrosis and in persons without inner ear symptoms.

PURPOSE: We compared MRI of the membranous labyrinth in patients with chronic non-neoplastic inner ear disease and MR signs of labyrinthine fibrosis and controls depending on their age, in order to establish whether there were any MR differences regarding patient age groups, control age groups and between the patients and controls themselves. MATERIALS AND METHODS: Clinical ENT examinations as well as a T2* weighted 3D CISS (Constructive Interference in Steady State) sequence with a slice thickness of 0.7 mm were performed. Our collective was subdivided as follows: 0-19 years (10 controls, 3

[Acute vertigo].

Three questions are raised when a patient experiences sudden vertigo. Is it due to a disease of the vestibule? This would be suggested in case of a revolving nature of the vertigo, the presence of nystagmus and of vestibular ataxia. Are there associated otological (infection of the labyrinth, zoster neuritis) or neurological (ischemic vascular accidents of the cerebral trunk and of the cerebellum, cerebellar haemorrhage) signs? What is the evolution of the sudden vertigo? A single, isolated episode of vertigo will generally be related to vestibular neuritis. However, it may rather be a first e

[The radiological pattern of cochlear otosclerosis. A CT study of 14 patients].

In this review of cochlear otosclerosis 14 cases were studied by CT scan aiming to establish a densitometric pattern of the capsular foci and relating it to the hearing and vestibule dysfunctions. Severe demineralization with characteristics of probable activity (increased lucency of 30-40%) was demonstrated in the capsular foci. These were mainly cochlear with endosteal involvement (93%): large (64%) or discrete (29%). Cochlear otosclerosis was widespread in 64% of the patients, with coexisting foci in the semicircular canals (38%), vestibule aqueduct (43%) and internal auditory canal (43%).

What can posturography tell us about vestibular function?

Patients with balance disorders want answers to the following basic questions: (1) What is causing my problem? and (2) What can be done about my problem? Information to fully answer these questions must include status of both sensory and motor components of the balance control systems. Computerized dynamic posturography (CDP) provides quantitative assessment of both sensory and motor components of postural control along with how the sensory inputs to the brain interact. This paper reviews the scientific basis and clinical applications of CDP. Specifically, studies describing the integration of
