Liver function assessment in workers exposed to vinyl chloride.

OBJECTIVE: To investigate liver function in vinyl chloride workers and assess its relation with current/past occupational exposure to vinyl chloride monomer (VCM). METHODS: A medical examination including the execution of liver function tests (LFTs) and liver ultrasonography was executed in a group of 757 workers with a long-standing service in the production of VCM/polyvinylchloride (PVC). Cumulative and maximum VCM exposures were calculated. History of viral hepatitis and alcohol intake were carefully investigated. Regression analysis explored the association between abnormal LFTs and a gro…

Vasculitic purpura in vinyl chloride disease: a case report.

Vinyl chloride (VC), a volatile substance mostly used for polyvinyl chloride (PVC) synthesis, is a systemic toxicant particularly noxious to endothelium. Angiosarcoma of the liver, Raynaud's phenomenon, scleroderma-like lesions, acroosteolysis and neuritis are known to be typical vinyl chloride-associated manifestations (VC disease). A so far unknown feature of the disease is purpura. This was first observed by the authors in a worker of a PVC-producing plant. The skin eruption was characterized by small purpuric maculae with tiny, palpable spots and papulae, mostly concentrated on the lower p…

Carcinogenic, mutagenic and teratogenic risks associated with vinyl chloride.

The data presented demonstrate clearly that vinyl chloride (VC) is related to a significant excess of mortality from cancer of the liver, lung and brain among workers occupationally exposed to VC. The risk of dying from cancer of the lymphatic and hematopoietic system also appears to increase with an increase in latency. These cancer sites could have been predicted by the animal bioassay conducted by Maltoni. With regard to the liver, even the histophthologic type of cancer (angiosarcoma) was observed first in experimental animals. A study of cancer mortality among populations residing proxima…

Vinyl chloride: a case study of data suppression and misrepresentation.

When the U.S. Environmental Protection Agency (EPA) finalized its 2000 update of the toxicological effects of vinyl chloride (VC), it was concerned with two issues: the classification of VC as a carcinogen and the numerical estimate of its potency. In this commentary we describe how the U.S. EPA review of VC toxicology, which was drafted with substantial input from the chemical industry, weakened safeguards on both points. First, the assessment down-plays risks from all cancer sites other than the liver. Second, the estimate of cancer potency was reduced 10-fold from values previously used for…

[Effect of vinyl chloride and its polymers on the nervous system].

The study was undertaken for assessment of the effect of vinyl chloride and is polymers on the nervous system. A group of 200 patients working in the production of VC were studies. The group comprised 155 men and 45 women aged 19-46 years (mean 40). The duration of employment was over 5 years in most workers (85.5%), and 26% worked over 20 years (mean duration 14 years). On the basis of neurological examinations clinical groups were isolated: without neurological changes (58 workers), with symptoms (54), with organic signs (88). The studied groups were not different with respect to age. The sh…

Effects of vinyl chloride exposures to rats pretreated with phenobarbital.

Male rats were exposed to 10 consecutive days, 6 hr/day, to vinyl chloride vapors at an average concentration of 13,500 ppm. The exposed rats were divided into three groups of eight rats each: one group was pretreated with 3-methylcholanthrene, one group was pretreated with phenobarbital, and the third group received no treatment. Half the animals in each group were sacrificed 18 hr after the last exposure and half were sacrificed 4 days later. In a second experiment, four rats pretreated with phenobarbital were exposed to vinyl chloride vapors at a concentration of 17,300 ppm for 2 days and s…

Isopropanol and acetone induces vinyl chloride degradation in Rhodococcus rhodochrous.

In situ bioremediation of vinyl chloride (VC)-contaminated waste sites requires a microorganism capable of degrading VC. While propane will induce an oxygenase to accomplish this goal, its use as a primary substrate in bioremediation is complicated by its flammability and low water solubility. This study demonstrates that two degradation products of propane, isoproponal and acetone, can induce the enzymes in Rhodococcus rhodochrous that degrade VC. Additionally, a reasonable number of cells for bioremediation can be grown on conventional solid bacteriological media (nutrient agar, tryptic soy …

Review of epidemiologic study results of vinyl chloride-related compounds.

Epidemiologic study results addressing the carcinogenicity of six compounds related to vinyl chloride (vinylidene chloride, trichloroethylene, perchloroethylene, carbon tetrachloride, ethylene dibromide and epichlorohydrin) are reviewed. The study results suggest an increased carcinogenic risk among workers exposed to epichlorohydrin and to dry cleaning and degreasing solvents. Although several studies report no significant excess of cancer mortality, an evaluation of the design of these investigations demonstrates that these negative cohort studies consisted of populations of insufficient sam…

Vinyl chloride: its impact on occupational medicine practice in Iran.

Upon the establishment of a positive relationship between occupational exposure to VCM and angiosarcoma of the liver, strict precautionary measures for the protection of VCM exposed employees in the Abadan Petrochemical Company have been instituted. All such workers have been required to have pre-employment and annual medical examinations with emphasis on liver functions. Among 43 such employees who have undergone regular medical evaluation in the past three years, no instance of liver dysfunction of angiosarcoma has been discovered. The Iranian Standards Institute reviewing occupational healt

Upcycling Poly(vinyl chloride) and Polystyrene Plastics Using Photothermal Conversion.

Poly(vinyl chloride) (PVC) and polystyrene (PS) are among the least recycled plastics. In this work, we developed a simple and novel strategy to valorize PVC and PS plastics via photothermal conversion to (1-chloroethyl)benzene, a commodity chemical with excellent versatility. As PVC is known to release HCl gas and decompose into conjugated polyenes, we envisioned a dual role for PVC plastics. While the in situ-generated HCl serves as a chlorine source, the resulting dehydrochlorinated-PVC (DHPVC) functions as a photothermal agent to accelerate the hydrochlorination of styrene. We converted PV…

Pyrolysis of poly(vinyl chloride) and-electric arc furnacedust mixtures.

An investigation into the pyrolysis kinetics of PVC mixed with electric arc furnace dust (EAFD) was performed. Mixtures of both materials with varying PVC ratios (1:1, 1:2, 1:3) were prepared and pyrolyzed in a nitrogen atmosphere under dynamic heating conditions at different heating rates (5, 10, 30 and 50 °C/min). The pyrolysis process proceeded through two main decomposition steps; the first step involved the release of HCl which reacted with the metal oxides present in the dust, subsequently forming metal chlorides and water vapor. Benzene was also found to release as detected by TGA-MS. T

Vinyl chloride monomer and other contaminants in PVC welding fumes.

An investigation into the nature of fumes produced during thermal welding of plasticized PVC sheeting has been carried out with the objective of determining if the known carcinogen vinyl chloride monomer (VCM) is formed and to assess the level of exposure to the operator. The results show that the atmospheric concentrations of VCM are well below accepted occupational exposure limits. This finding is consistent with reports in the technical literature which suggest that VCM is produced during thermal degradation of PVC only at temperatures considerably higher than those encountered during plast…

Cytogenetic investigations on lymphocytes from workers exposed to vinyl chloride.

Eleven male workers from the polymerization department of a vinyl chloride factory, seven people employed in the laboratory of another vinyl chloride plant, and ten controls from outside the factory environment were examined for the presence of chromosome aberrations in blood lymphocytes. Chromosome anomalies such as fragments, rings, translocations, and dicentrics were observed in most of the workers from the polymerization department. However, the medical history of these workers shows that they received frequent radiographs of the hands, feet, vertebral column, and digestive tract. It is th…

Synthesis of Vinyl Chlorides via Triphosgene-Pyridine Activation of Ketones.

Herein, we describe a mild method to prepare aliphatic and aromatic vinyl chlorides from their corresponding ketones via triphosgene-pyridine activation in dichloromethane at reflux. The mechanism of this reaction is proposed to involve formation of a putative α-chloro pyridinium carbamate intermediate, which appeared to readily undergo E2 elimination in the presence of pyridine.

Effect of the CYP2E1 genotype on vinyl chloride monomer-induced liver fibrosis among polyvinyl chloride workers.

Although a relationship between vinyl chloride monomer (VCM) and liver cirrhosis has been reported, the underlying mechanisms are not clear. Cytochrome P450 2E1 (CYP2E1), aldehyde dehydrogenase 2 (ALDH2) and glutathione S-transferase theta 1 (GSTT1) enzymes are involved in activation and detoxification of VCM, and thus may be important determinants of interindividual susceptibility to VCM-induced liver damage, including liver cirrhosis. The objective of this study was to evaluate if metabolizing genetic polymorphisms could modify individual susceptibility to liver fibrosis of the VCM exposure.…

Quantitative estimated exposure to vinyl chloride and risk of angiosarcoma of the liver and hepatocellular cancer in the US industry-wide vinyl chloride cohort: mortality update through 2013.

OBJECTIVE: To evaluate mortality risks of angiosarcoma of the liver (ASL), primary hepatocellular carcinoma (HCC) and other cancers among 9951 men employed between 1942 and 1972 at 35 US vinyl chloride (VC) or polyvinyl chloride plants followed for mortality through 31 December 2013. METHODS: SMR and time-dependent Cox proportional hazards analyses were used to evaluate mortality risks by cumulative VC exposure. RESULTS: Liver cancer mortality was elevated (SMR=2.87, 95% CI 2.40 to 3.40), and ASL and HCC were strongly associated with cumulative VC exposure ≥865 parts per million-years (ppm-y…

Mortality experience of workers exposed to vinyl chloride monomer in the manufacture of polyvinyl chloride in Great Britain.

Identification particulars were obtained for over 7000 men who were at some time between 1940 and 1974 exposed to vinyl chloride monomer in the manufacture of polyvinyl chloride. Approximately 99% of these men have been traced and their mortality experience studied. The overall standardised mortality ratio, 75-4, shows a significant reduction compared with the national rates. Four cases of liver cancer were found. Two of these have been confirmed by a panel of liver pathologists as angiosarcoma and two as not angiosarcoma. There is no evidence to support the hypothesis that cancers other than …
