Warfarin in vitreoretinal surgery: a case controlled series.

INTRODUCTION: Warfarin is a commonly used anticoagulant whose effect in vitreoretinal surgery has not been well studied. METHODS: A series of 60 patients on warfarin therapy undergoing pars plana vitrectomy were retrospectively case controlled to 60 patients with similar presenting complaints. In addition, an online survey was performed of current practice in the UK. RESULTS: 2% of the patients receiving vitrectomy were on warfarin. There were 33 males and 27 females with a median age of 72.5 years; follow-up was for a mean of 0.88 years. The international normalised ratio (INR) ranged betwe

An applicator-shaped scleral depressor for vitreoretinal surgery.

This article describes an instrument that combines a standard T-shaped scleral depressor at one end with a cotton applicator-shaped depressor at the other end. The applicator-shaped depressor has a matted surface to prevent slippage. This simple instrument has several advantages over depressors now available for intraoperative use.

Reliability of preoperative ultrasonography evaluation for vitreoretinal surgery.

To evaluate the present level of reliability of common ultrasonography prior to vitreoretinal surgery, we carried out a prospective study to compare preoperative ultrasonography data with direct intraoperative findings. A total of 228 eyes with different vitreoretinal disorders and opaque or clear media were studied. In the group of 187 eyes with tractional retinal detachment (group I), the percentage of agreement between the ultrasonography data and the surgical findings ranged from 77.1% to 92.2%, depending on the pathology. In the group of 41 eyes without retinal detachment (group II), agre

Pediatric Vitreoretinal Surgery and Integrated Intraoperative Optical Coherence Tomography.

Intraoperative portable handheld and microscope-integrated OCT enhance the pediatric vitreoretinal surgeon's diagnostic abilities during examination under anesthesia and surgery, particularly in children who are challenging to examine preoperatively due to young age or ocular trauma. Improved OCT-guided visualization of vitreoretinal anatomic relationships has the potential to improve surgical safety and efficiency. In retinopathy of prematurity and other pediatric retinal vascular conditions, intraoperative OCT can be critical for distinguishing between retinoschisis and retinal detachment an

Vitreoretinal surgery and endoresection in high posterior choroidal melanomas.

PURPOSE: Eyes with posterior choroidal melanomas more than 9 mm in thickness frequently are enucleated because of the potential complications of radiotherapy. The aim of this study was to evaluate the safety and efficacy of internal resection of these tumors. METHODS: Twenty-five consecutive patients with high posterior choroidal melanomas with a diameter less than 15 mm and a thickness greater than 9 mm were treated. If the retina was not invaded by the tumor, a vitrectomy was performed, followed by posterior hyaloid dissection, 120 degrees anterior retinotomy, endophotocoagulation 2 mm past

The artificial iris diaphragm for vitreoretinal silicone oil surgery.

BACKGROUND: Silicone keratopathy frequently develops as a complication of silicone oil tamponade in the management of severe trauma in eyes with partial or complete aniridia. We therefore designed an "open" artificial iris diaphragm to prevent silicone oil-endothelial contact. In hypotony, where insufficient circulation of aqueous allows silicone in the anterior chamber despite an open diaphragm, a new solution became necessary. The "closed" artificial diaphragm was developed. METHODS: In this retrospective study, two consecutive series of artificial iris diaphragms were compared. Forty-four

Sudden Visual Loss After Vitreoretinal Surgery: A Case Report.

A 51-year-old male underwent vitrectomy with retrobulbar anesthesia for retinal detachment. Post surgery, he experienced systemic hypotension which normalized after 3 h. The day after, he complained of a central scotoma in the operated eye. Intraocular pressure was normal, but fundus examination revealed hemorrhages and whitening along the papillomacular bundle and macula, with additional whitening in the upper midperipheral region. Multimodal imaging confirmed branch retinal vein, artery, and cilioretinal artery occlusion. Further examination revealed mild-to-moderate obstructive sleep apnea

Patent blue: a novel vital dye in vitreoretinal surgery.

OBJECTIVE: Melanoma is one of the most dreaded types of cancer in the world today, and therefore, early detection becomes crucial. Deep learning models need annotated training data, and such training data are difficult and expensive to acquire. To solve this challenge, we introduce a new framework called the Contrastive Self-Supervised Ensemble Transfer Learning (CSSL-ETL) that combines the techniques of CSSL and ETL to improve the feature learning and classification accuracy of the model. METHODS: The CSSL-ETL framework integrates Contrastive Self-Supervised Learning (CSSL) and Ensemble Tran
