PURPOSE: Children who use manual wheelchairs encounter pain and injury risks to the upper body. Current literature does not describe how propulsion pattern and physiotherapeutic training methodologies impact response to treatment.
METHODS: This study assesses the effect of community-based intensive physical and occupational therapy on functional outcomes over a 7-week period in pediatric manual wheelchair users.
RESULTS: Key results include significant joint and musculotendon kinematic differences at the shoulder, improved speed and propulsion effectiveness, and changed propulsion pattern.

Those with lower limb disabilities are often dependent on manually propelled wheelchairs for their mobility, in Europe today some 3.3 million people. This implies a transfer from leg to arm work for ambulation and all other activities of daily living (ADL). Compared to the legs, arm work is less efficient and more straining, and leads to a lower physical capacity. Also, there is a major risk of mechanical overuse. Problems of long-term wheelchair use are not only pain or discomfort, but also a risk of a physically inactive lifestyle. Subsequently, serious secondary impairments (obesity, diabet…

Power wheelchair users often face challenges in safe navigation, especially when reversing or operating with limited visual scanning. Existing feedback systems, including joystick force feedback and wearable vibrotactile devices, suffer from low information fidelity and usability issues. This research presents a novel vehicle-embedded vibrotactile haptic feedback system integrated into the cushion, armrests, and backrest, employing ultrasonic and infrared sensors to detect obstacles and deliver spatially mapped vibration cues. Two research questions are addressed: (Q1) whether embedded haptic

BACKGROUND: Contributions of contaminated wheelchairs to nosocomial pathogen transmission are relatively unknown. Our aim was to develop a model predicting pathogen exposures for patients utilizing wheelchairs and estimate exposure reduction potential of wheelchair disinfection between rides.
METHODS: An agent-based model was informed by wheelchair location data from a connected 215-bed acute care and 250-bed long-term care facility. Simulated scenarios varied in frequencies of patient wheelchair contamination and wheelchair disinfection in between trips. Clostridioides difficile and methicil…

Falls are a serious health concern for persons with multiple sclerosis (PwMS) who use wheelchairs or scooters as their primary mode of mobility. Unfortunately, little is known about the fall prevalence and characteristics of this large segment of the multiple sclerosis (MS) community. The purpose of this study is to determine the prevalence and circumstances of falls in wheelchair and scooter users living with MS. Forty-four PwMS were recruited from research and medical centers in the United States and Asia. Participants completed a survey focusing on prevalence of falls, frequency of injuriou…

BACKGROUND: Rates of shoulder pain in individuals who use manual wheelchairs (MWCs) as their primary means of mobility have been reported to be as high as 70% during activities of daily living. Current prevailing thought is that mechanical impingement of the soft tissues that reside within the subacromial space between the humeral head and coracoacromial arch is a major contributor to the shoulder pain in users of MWCs. The subacromial space size is directly related to the kinematics at the shoulder joint. Yet to be answered are questions about which common daily tasks are characterized by the…

BACKGROUND: Involuntary extensor thrust experienced by wheelchair users with neurological disorders may cause injuries via impact with the wheelchair, lead to the occupant sliding out of the seat, and also damage the wheelchair. The concept of a dynamic seat, which allows movement of a seat with respect to the wheelchair frame, has been suggested as a potential solution to provide greater freedom and safety. Knowledge of the human-generated motion and forces during unconstrained extensor thrust events is of great importance in developing more comfortable and effective dynamic seats. The object

PURPOSE: Standard manual wheelchairs (MWCs) are inefficient and pushrim propulsion may cause progressive damage and pain to the user's arms. We describe a wheel for a MWC with a novel propulsion mechanism.
METHODS: The wheel has two modes of operation called "Standard" mode and "Run" mode. In Run mode, the wheelchair is propelled forward by pushing a compliant handle forward and then pulling it back, both strokes contributing to forward propulsion. We report the propulsive force and preliminary testing on a rough outdoor circuit by three able-bodied participants.
RESULTS: In Run mode, the pe…

OBJECTIVES: To test the hypothesis that challenges to community participation posed by winter weather are greater for individuals who use scooters, manual and power wheelchairs (wheeled mobility devices [WMDs]) than for the general ambulatory population, and to determine what WMD users identify as the most salient environmental barriers to community participation during the winter.
DESIGN: Cross-sectional survey organized around 5 environmental domains: technological, natural, physical, social/attitudinal, and policy.
SETTING: Urban community in Canada.

With the growth of wheelchair athletics at both grassroots and national levels, there has been an increased incidence of sports-related and overuse syndromes in the wheelchair-user population. This article provides an overview of the available literature on the incidence, prevalence, evaluation, prevention, and rehabilitative management of these types of clinical syndromes.

BACKGROUND: The purpose of this study is to examine the impact of trunk kinematic characteristics and trunk muscle electromyography (EMG) activity on propulsion speeds in wheelchair racing T54 athletes.
METHOD: The Vicon infrared high-speed 3D motion capture system was utilized to acquire kinematic data of the shoulders, elbows, wrists, and trunk from twelve T54 athletes at four different speeds (5.55 m/s, 6.94 m/s, 8.33 m/s, and personal maximum speed). Additionally, the Trigno Wireless EMG system was employed to collect synchronous surface electromyography (EMG) data from the rectus abdomin

The purpose of this study was to investigate whether athletic and nonathletic manual wheelchair users (MWU) display differences in kinetic and kinematic variables during daily wheelchair propulsion. Thirty-nine manual wheelchair users (athletic n = 25; nonathletic n = 14) propelled their own daily living wheelchair on a roller ergometer at two submaximal speeds for three minutes (1.11 m s-1 and 1.67 m s-1). A 10 camera Vicon motion capture system (Vicon, Motion Systems Ltd. Oxford, United Kingdom) collected three-dimensional kinematics of the upper limbs and thorax at 200 Hz during the final m

PURPOSE: Although approximately 45% of adults with Autosomal Recessive Spastic Ataxia of Charlevoix-Saguenay (ARSACS) are permanent wheelchair users, this sub population has been less studied. The purpose of this study was to document wheelchair mobility, motor performance, and participation in a cohort of adult wheelchair users with ARSACS.
METHODS: We recruited 36 manual and powered wheelchair users with ARSACS, aged between 34 and 64 years, for this cross-sectional study. Participants completed measures regarding wheelchair mobility (Wheelchair Skills Test Questionnaire [WST-Q-F], Wheelcha

Purpose: Standards of care for neuromuscular disorders recommend timely provision of wheelchair equipment to support independent and spontaneous movement, age-appropriate participation, and psychological, social, cognitive and communication skills. Parental engagement is crucial to initiating wheelchair prescription. No studies have investigated parents' perceptions of this process or their experiences of their child's transition to wheelchair equipment. Methods: Seventeen families of children with a neuromuscular disorder who were recommended wheelchair equipment participated in an interview (

PURPOSE: We present two case reports that shed light on the question of whether routine periodic wheelchair-skills assessment and training are relevant for long-standing wheelchair users. CASE 1: A 60-year-old man with a 15-year history of T12 complete paraplegia sustained an intertrochanteric fracture of his femur due to a tip-over accident that occurred 2 days after a follow-up clinic visit at which no limitations in wheelchair-skill performance were identified. If a procedure had been in place to identify and correct his wheelchair-skill deficiencies, this injury might have been prevented.
