Implementation of a Medication for Addiction Treatment (MAT) and Linkage Program by Leveraging Community Partnerships and Medical Toxicology Expertise.

INTRODUCTION: Implementing a hospital medication for addiction treatment (MAT) and a linkage program can improve care for patients with subs … MAT clinic: 72.68% on buprenorphine/naloxone, 24.59% on naltrexone, 1.09% buprenorphine, and 0.55% acamprosate. CONCLUSION: Implementing a MAT and linkage program in the ED and hospital setting was feasible. Leveraging medical toxicology expertise as well as community and funding partnerships was crucial to successful implementation.

Barriers to and facilitators of pharmacotherapy for alcohol use disorder in VA residential treatment programs.

Among US military veterans, alcohol use disorder (AUD) is prevalent and in severe cases patients need intensive AUD treatment beyond outpatient care. The Department of Veterans Affairs (VA) delivers intensive, highly structured addiction and psychosocial treatment through residential programs. Despite the evidence supporting pharmacotherapy among the effective treatments for AUD, receipt of these medications (e.g., naltrexone, acamprosate) among patients in residential treatment programs varies widely. In order to better understand this variation, the current study examined barriers and facili…

Patients with alcohol dependence and HCV infection, can be treated with acamprosate, nalmefene, topiramate, and disulfiram, although baclofen is the only drug specifically tested for this purpose in patients with ALD and/or HCV infection.

Endpoints for Pharmacotherapy Trials for Alcohol Use Disorder.

Alcohol use disorder (AUD) is a debilitating disorder, yet currently approved pharmacotherapies to treat AUD are under-utilized. The three medications approved by the US Food and Drug Administration (FDA) for the indication of AUD are disulfiram, acamprosate, and naltrexone. The current landscape of pharmacotherapies for AUD suggests opportunities for improvement. Clinical trials investigating novel pharmacotherapies for AUD traditionally use abstinence-based drinking outcomes or no heavy drinking days as trial endpoints to determine the efficacy of pharmacotherapies. These outcomes are typica…

Improving adherence in patients with alcohol dependence: a new role for pharmacists.

PURPOSE: To review medication adherence rates with various pharmacotherapies and the role of pharmacists in improving medication adherence in patients with alcohol dependence. SUMMARY: Nonadherence diminishes the effectiveness of treatment for all chronic diseases, including alcohol dependence. Rates of adherence to pharmacotherapies for alcohol dependence range widely, both within and among agents. Factors undermining adherence include high pill burden and adverse effects, both of which occur with common treatments for alcohol dependence. There is a clear need for interventions that improve

Treatment of Alcohol Use Disorder: Behavioral and Pharmacologic Therapies.

The prevalence of alcohol use disorder (AUD) has significantly increased over the last decade, leading to an increase in alcohol-associated liver disease (ALD) rates worldwide. Despite this prominence, AUD in ALD remains undertreated and carries significant implications in the progression to end-stage ALD and increased mortality. In efforts to bridge this gap, interprofessional and integrated AUD treatment is necessary for patients with ALD to ensure early detection and an appropriately targeted level of care. Although pharmacotherapy, psychotherapy, and psychosocial interventions independentl

From the neurobiologic basis of alcohol dependency to pharmacologic treatment strategies: bridging the knowledge gap.

Alcohol dependence (AD) is a complex disease involving biologic, psychosocial, and environmental factors. Increasing evidence shows that the development of AD involves changes in neurotransmitter function in the areas of the brain associated with alcohol craving and reward. Although AD is gaining acceptance as a medical disease, management of AD rarely includes medical treatment. Pharmacotherapy is a useful adjunct to psychosocial therapy and can target the biologic changes associated with AD. Similar to the role of medications in depression, pharmacotherapy can improve the response to psychos

Psychosocial and medication interventions to stop or reduce alcohol consumption during pregnancy.

BACKGROUND: Despite the known harms, alcohol consumption is common in pregnancy. Rates vary between countries, and are estimated to be 10% globally, with up to 25% in Europe. The BRENDA model: integrating psychosocial treatment and pharmacotherapy for the treatment of alcohol use disorders.

While the U.S. Food and Drug Administration has approved several medications for the treatment of alcohol-related problems, their use has not gained wide acceptance in the United States. Typically, patients with alcohol use disorders are only referred to psychosocial support (e.g., Alcoholics Anonymous). However, the use of pharmacotherapy may complement psychosocial treatments, as evidence shows that pharmacotherapy can improve treatment outcomes. The effectiveness of pharmacotherapy depends on patient compliance with taking the medication and the context in which the medication is administer

Treatment of alcohol dependence: recent progress and reduction of consumption.

Alcohol dependence (AD) is a major public health problem. Currently, three drugs for the treatment of AD have been approved by both the European Medicines Agency (EMA) and the Food and Drug Administration (FDA): acamprosate, disulfiram, and oral naltrexone. The FDA also approved the use of long-acting injectable naltrexone. In Austria and in Italy sodium oxybate is also approved. The EMA's Committee for Medicinal Products for Human Use has recently granted marketing authorization for nalmefene for the reduction of alcohol consumption. Many patients, while accepting the problem, are unable or u…

Modeling longitudinal drinking data in clinical trials: an application to the COMBINE study.

BACKGROUND: There is a lack of consensus in the literature as to how to define drinking outcomes in clinical trials. Typically, separate statistical models are fit to assess treatment effects on several summary drinking measures. These summary measures do not capture the complexity of drinking behavior. We used the COMBINE study to illustrate a statistical approach for examining treatment effects on high-resolution drinking data. METHODS: This is a secondary data analysis of COMBINE participants randomly assigned to naltrexone, acamprosate, with medical management and/or combined behavioral i…
