Objective Sleep Characteristics and Cardiometabolic Health in Young Adolescents.

UNLABELLED: : media-1vid110.1542/5778442247001PEDS-VA_2017-4085Video Abstract BACKGROUND AND OBJECTIVES: Shorter sleep duration is associated with childhood obesity. Few studies measure sleep quantity and quality objectively or examine cardiometabolic biomarkers other than obesity. METHODS: This cross-sectional study of 829 adolescents derived sleep duration, efficiency and moderate-to-vigorous physical activity from >5 days of wrist actigraphy recording for >10 hours/day. The main outcome was a metabolic risk score (mean of 5 sex-specific z-scores for waist circumference, systolic blood pres

Committee Opinion No. 705: Mental Health Disorders in Adolescents.

Mental health disorders in adolescence are a significant problem, relatively common, and amenable to treatment or intervention. Obstetrician-gynecologists who see adolescent patients are highly likely to see adolescents and young women who have one or more mental health disorders. Some of these disorders may interfere with a patient's ability to understand or articulate her health concerns and appropriately adhere to recommended treatment. Some disorders or their treatments will affect the hypothalamic-pituitary-gonadal axis, causing anovulatory cycles and various menstrual disturbances. Adole

Sleep Patterns and Mental Health Correlates in US Adolescents.

OBJECTIVE: To investigate systematically the associations of sleep patterns with a range of mental disorders and other outcomes among a nationally representative sample of US adolescents. STUDY DESIGN: Using the National Comorbidity Survey Adolescent Supplement, a nationally representative cross-sectional survey of 10 123 US adolescents 13-18 years of age, we assessed associations between adolescent-reported sleep patterns (tertiles of weeknight bedtime, weeknight sleep duration, weekend bedtime delay, and weekend oversleep) and past-year mental disorders based on the Diagnostic and Statistic

The health diagnoses of homeless adolescents: a systematic review of the literature.

BACKGROUND: Homelessness during adolescence impacts negatively upon young people's physical and mental wellbeing. To be effective, programs aimed at addressing the health needs of this population must include knowledge of both the presenting and underlying acute and chronic conditions that characterise this high risk group of youth. METHODS: We undertook a systematic review of the international literature for studies that used validated instruments and techniques to diagnose prevalence rates of physical and mental health disorders in homeless adolescents. RESULTS: Twenty-one studies fulfille

Addressing the mental health needs of pregnant and parenting adolescents.

Adolescent parenthood is associated with a range of adverse outcomes for young mothers, including mental health problems such as depression, substance abuse, and posttraumatic stress disorder. Teen mothers are also more likely to be impoverished and reside in communities and families that are socially and economically disadvantaged. These circumstances can adversely affect maternal mental health, parenting, and behavior outcomes for their children. In this report, we provide an overview of the mental health challenges associated with teen parenthood, barriers that often prevent teen mothers fr

Polycystic ovary syndrome in adolescents: (women's health series).

Polycystic ovary syndrome is the single most common endocrine abnormality of women of reproductive age and is a leading cause of female infertility. Common clinical features include hirsutism, various ovarian abnormalities, obesity, and insulin resistance. Expert consensus recommendations on diagnostic criteria vary, but the most recent focus on the presence of clinical features of hyperandrogenism, hyperandrogenemia, polycystic ovaries, and ovulatory and menstrual dysfunction to the exclusion of alternative diagnoses. In adolescence, diagnosis is more difficult because of the frequent presenc

The mental health needs of children and adolescents with learning disabilities.

PURPOSE OF REVIEW: To provide an update on the mental health needs of children and adolescents with learning disabilities, by examining salient studies published predominantly in the last 12-18 months. RECENT FINDINGS: There have been further articles published supporting the findings of earlier landmark studies demonstrating an increased prevalence of mental health disorders in young people with learning disabilities. These articles suggest higher rates of comorbidity than were previously recognized. There are few published studies pertaining to the effectiveness of psychological and pharmac

Family management of a chronic health condition: perspectives of adolescents.

A growing number of adolescents and their families are dealing with a chronic health condition that impacts their daily life. Research using the Family Management Style Framework (FMSF) has added much to our understanding of how the family integrates chronic condition management into family life. Less clear, however, is the adolescent perspective of the FMSF components. The purpose of this secondary qualitative analysis was to explore 25 interviews of adolescents with spina bifida to uncover the adolescent's perspective of the three major FMSF components: Definition of the Situation, Managemen

[Unwanted sexual experiences and health complaints among adolescents].

BACKGROUND: Strong indicators support a causal relationship between sexual assaults and impaired emotional health. The aim of this study was to investigate the prevalence of unwanted sexual experiences among adolescents and potential associations between such experiences and other health problems. MATERIAL AND METHODS: The study is based on a survey among senior high school students in Førde in 2001. Linear regression analyses were used to examine the association between negative sexual experiences and emotional and somatic impairment. RESULTS: 837 students participated, representing 93 % of

Family acceptance in adolescence and the health of LGBT young adults.

ISSUE: the role of family acceptance as a protective factor for lesbian, gay, bisexual, and transgender (LGBT) adolescents and young adults has not been established. METHODS: a quantitative measure with items derived from prior qualitative work retrospectively assessed family accepting behaviors in response to LGBT adolescents' sexual orientation and gender expression and their relationship to mental health, substance abuse, and sexual risk in young adults (N= 245). FINDINGS: family acceptance predicts greater self-esteem, social support, and general health status; it also protects against d

Vascular health in children and adolescents: effects of obesity and diabetes.

The foundations for cardiovascular disease in adults are laid in childhood and accelerated by the presence of comorbid conditions, such as obesity, diabetes, hypertension, and dyslipidemia. Early detection of vascular dysfunction is an important clinical objective to identify those at risk for subsequent cardiovascular morbidity and events, and to initiate behavioral and medical interventions to reduce risk. Typically, cardiovascular screening is recommended for young adults, especially in people with a family history of cardiovascular conditions. Children and adolescents were once considered

Use of hormonal contraception in adolescents: skeletal health issues.

PURPOSE OF REVIEW: To summarize the current literature regarding the effects of hormonal contraceptives on adolescents' bone health. RECENT FINDINGS: Use of progestin-only pills, implant and progestin-releasing intrauterine device is associated with low systemic progestin levels that do not appear to impact ovarian estradiol production or bone mineral density (BMD). In contrast, higher systemic progestin levels associated with the use of depot medroxyprogesterone acetate and combination oral contraceptives suppress ovarian estradiol production and reduce acquisition of BMD in teenagers and yo

Oral health of adolescents--it's more than dental caries.

Of all the health issues common to adolescents, oral health has not seemed to receive much attention. However, just as "eyes are the windows to the soul," the oral health of an adolescent can be a reliable and expedient indicator of general health. Lesions discovered during an oral exam can indicate systemic problems that may not otherwise be disclosed during the health history. Tobacco use, sexually transmitted infections, and type II diabetes can manifest in the mouth, and many of these lesions mimic other illnesses. It is essential that nurses who work with adolescents be aware of possible

Play therapy as a mental health intervention for children and adolescents.

Play therapy is a child-centred therapy for children between the ages of 3 and 16 years who have mental health and/or other difficulties. The play therapist works one to one with the child, allowing children to explore their inner emotional world in almost any way they choose in the play room. The therapist sets a few necessary behavioural limits in order to safeguard physical and emotional safety during the weekly sessions. Children from all kinds of families come for play therapy. Some have witnessed domestic violence, or experienced abuse or other trauma. Some have conflicted relationships

The menstrual cycle: a biological marker of general health in adolescents.

Menstruation is the cyclic, orderly sloughing of the uterine lining on account of the interactions of hormones produced by the hypothalamus, pituitary, and ovaries. There is a tendency among parents and clinicians to view oligo-amenorrhea as a normal variant in the teen years. In fact, the 95th percentile for the time interval between cycles is 90 days. Thus, it is abnormal for an adolescent to be amenorrheic for greater than 3 months, even in the early gynecologic years. Identification of abnormal menstrual patterns throughout adolescence may permit early identification of potential health co

Health plans' role in preventing overweight in children and adolescents.

The increasing prevalence of overweight children and adolescents in the United States threatens the well-being of a vast segment of this population. This paper examines how U.S. health plans can promote evidence-based behavioral-change strategies by directly intervening in medical settings and by supporting efforts to modify the environments in which young people live, study, and play. We describe a variety of innovative initiatives launched in recent years by health plans to address overweight among children and adolescents. Despite gaps in the evidence base, enough is now known to support ag

Health-promoting behavior of adolescents with congenital heart disease.

PURPOSE: To evaluate and compare the health-promoting behavior of adolescents with congenital heart disease (CHD) to that of adolescents without CHD. METHODS: Cross-sectional data were collected from pediatric cardiology outpatient departments at two medical centers in Taiwan. A total of 1209 adolescents, including 316 with various forms of CHD and 893 without CHD, completed the Adolescent Health Promotion (AHP) scale. Of those with CHD, 162 were female, and 12-18 years old. The scores of adolescents with CHD were compared with published normative adolescent data. RESULTS: No significant dif…
