Epileptic encephalopathy with continuous spikes and waves in the occipito-temporal region during slow-wave sleep in two patients with acquired Kanji dysgraphia.

We encountered two patients with acquired Kanji dysgraphia in whom continuous spikes and waves, dominant in the occipito-temporal region, were recorded during slow-wave sleep. Electrical status epileptics during sleep (ESES) was demonstrated on overnight electroencephalography, and dipoles clustered in and around the posterior inferior temporal cortex on magnetoencephalography. Functional neuroimaging suggested dysfunction in the left posterior temporal lobe, including the posterior inferior temporal cortex. The patients had normal intelligence with no problems in reading and writing Kana, as

Treatment efficacy of language and calculation disorders and speech apraxia: a review of the literature.

The aim of the study was to evaluate the efficacy of the treatment for language and calculation disorders and for speech apraxia in vascular subjects. Only therapeutic methods that could be qualified as neuropsychological were taken into account. For language disorders, we searched the pertinent literature published from 1950 to August 31, 2007 by means of electronic data banks and we took into consideration the Cochrane review, and papers in Cicerone et al. and Cappa et al. systematic reviews. For acalculia we examined the literature from 1980 by carrying out research on electronic data banks

The categorical distinction of vowel and consonant representations: evidence from dysgraphia.

Subject GSI presents with a graphemic buffer deficit following a massive left fronto-parietal lesion. His errors involved essentially only consonants (98.2%). However, he always spelled correctly the first consonant in a string, and produced steadily increasing numbers of errors on the following consonants, while always spelling all the vowels in the target correctly. The pattern of performance observed in this subject provides strong evidence for the separate (categorical) representation of consonants and vowels, and suggests that working memory interacts with independent consonant and vowel

Selective uppercase dysgraphia with loss of visual imagery of letter forms: a window on the organization of graphomotor patterns.

We report a patient who, after a left parieto-occipital lesion, showed alexia and selective dysgraphia for uppercase letters. He showed preserved oral spelling, associated with handwriting impairment in all written production; spontaneous writing, writing to dictation, real words, pseudowords, and single letters were affected. The great majority of errors were well-formed letter substitutions: most of them were located on the first position of each word, which the patient always wrote in uppercase (as he used to do before his illness). The patient also showed a complete inability to access the

[Cognitive and psychiatric evolution in autoimmune encephalitis in a school-aged child: Clinical case].

BACKGROUND: Autoimmune encephalitis (AE) is a rare entity in children and can manifest psychiatric symptoms without evident neurological signs. CLINIC CASE: A 10-year-old female with a subacute case of 3 weeks of evolution, denotating disinhibition, behavioral errors, mood swings with irritability, auditory stimuli intolerance, restlessness in social situations; sleep cycle disturbances, right arm weakness upon physical exertion that progressed into right hemiparesis, adynamia, verbal apraxia, and agraphia. The patient was hospitalized, and infectious processes were excluded. Brain imaging wi…

Gray Matter Correlates of Finger Gnosis in Children: A VBM Study.

Accumulating evidence relates finger gnosis (also called finger sense or finger gnosia), the ability to identify and individuate fingers, to cognitive processing, particularly numerical cognition. Multiple studies have shown that finger gnosis scores correlate with or predict numerical skills in children. Neuropsychological cases as well as magnetic stimulation studies have also shown that finger agnosia (defects in finger gnosis) often co-occurs with cognitive impairments, including agraphia and acalculia. However, our knowledge of the structural and functional correlates, and the development…

Dissociative disturbance in Hangul-Hanja reading after a left posterior occipital lesion.

Since the Korean language has two distinct writing systems, phonogram (Hangul) and ideogram (Hanja: Chinese characters), alexia can present with dissociative disturbances in reading between the two systems. A 74-year-old right-handed man presented with a prominent reading impairment in Hangul with agraphia of both Hangul and Hanja after a left posterior occipital- parietal lesion. He could not recognize single syllable words and nonwords in Hangul, and visual errors were predominant in both Hanja reading and the Korean Boston Naming Test. In addition, he had difficulties in visuoperceptual tes…

Number processing in posterior cortical atrophy--a neuropsycholgical case study.

Posterior cortical atrophy (PCA) is an uncommon syndrome of dementia with early onset, characterised by disorders of higher visual function, variable symptoms of Balint's syndrome, visual agnosia, alexia, agraphia, finger agnosia, right-left disorientation and dyscalculia [Benson D. F., Davis R. J., & Snyder B. D. (1988). Posterior cortical atrophy. Archives of Neurology, 45, 789-793]. In a single case study specific numerical deficits were observed which may be predicted by parietal neurodegeneration (more pronounced on the right side; verified by SPECT). Besides impairments in all tasks invo…

Visuographemic alexia: a new form of a peripheral acquired dyslexia.

We report a single-case study of peripherally acquired dyslexia that meets the clinical criteria of "alexia without agraphia." The patient, AA, has a large infarct involving the left posterior cerebral artery. The most striking feature is a severe impairment in recognizing single visually presented letters that precludes explicit or implicit access to reading, even in a letter-by-letter fashion. AA can, however, differentiate letters from similar nonsense characters and digits, and he is also able to identify alphanumeric signs when the visual channel is bypassed (through somesthesic or kinest…

Reversible T2 hyperintense signal abnormality in the corpus callosum, although frequently seen after diffuse brain injury, has not been well … chological examination of the patient did not show callosal syndrome, namely hemialexia, unilateral agraphia and unilateral apraxia. Repeat MRI on day 20 demonstrated a signal decrease of both T2-weighted images and diffusion-weighted images (DWI) in the lesion. Follow-up MRI at 6 months showed complete resolution of the T2 signal abnormalities and of the …

Subcortical contributions to drawing.

Constructional impairment is often considered a sign of cortical damage. However, aphasia, agraphia, and apraxia, disorders traditionally deemed cortical, have been well described following subcortical lesions, suggesting an important role for subcortical structures in cognition. Drawing impairment following subcortical lesions has not been systematically explored or compared with that following cortical damage. We examined relative incidence and severity of drawing impairment after cortical and subcortical strokes and whether there were qualitative differences between these two groups' drawin…

A case of associative visual agnosia: neuropsychological findings and theoretical considerations.

A 61-year-old right-handed man fell downstairs and was found unconscious. There was no abnormality in the general physical examinations. Neurological examinations revealed a quadrantanopia. Neuropsychological examination revealed an impairment in recent verbal memory, alexia, agraphia, object agnosia, color naming difficulty, prosopagnosia, and visuospatial constructional disability. CT scan demonstrated subcortical hematomas in the temporo-occipital regions of both hemispheres. MRI demonstrated extensive low-intensity lesions in the lingual, fusiform and posterior inferior temporal gyri on bo…

Visuospatial deficits due to impaired visual attention: investigation of two cases of slowly progressive visuospatial impairment.

Two patients with slowly progressive visuospatial impairment demonstrated a peculiar type of visuoconstructive deficit. The most prominent manifestation appeared when handling kanji (logogram) characters and other figurative patterns. The patients showed pure agraphia for complex kanji but not for kana (syllabogram) or Arabic numerals. Their abilities to read and understand kanji characters and to orally describe the structure of a kanji character were preserved. They could not draw or copy figures or symbols except for single lines or simple symbols, although they could identify and name the …

INTRODUCTION: Pure alexia is a syndrome characterized by the inability to read aloud in the absence of agraphia or apnasia. CLINICAL CASES: Three clinical cases showing this syndrome are presented. Case I had a left occipital lesion compatible with a subacute haematoma. He had problems with reading, visuo-spatial recognition, digital gnosia and memorizing texts. One year later there was considerable improvement in most of the sub-tests evaluated. There was still deficient colour naming right/left orientation and understanding of letters and words. Case 2 presented with a right homonymous hemi…

Reading and alexia.

Alexia refers to a reading disorder caused by some form of acquired brain pathology, most commonly a stroke or tumor, in a previously literate subject. In neuropsychology, a distinction is made between central alexia (commonly seen in aphasia) and peripheral alexia (a perceptual or attentional deficit). The prototypical peripheral alexia is alexia without agraphia (pure alexia), where patients can write but are impaired in reading words and letters. Pure alexia is associated with damage to the left ventral occipitotemporal cortex (vOT) or its connections. Hemianopic alexia is associated with l…

Gerstmann Syndrome.

PURPOSE OF REVIEW: Gerstmann (left angular gyrus) syndrome includes the tetrad of finger agnosia (inability to distinguish, name, and recognize the fingers), agraphia (acquired disturbance in the ability to write), acalculia (loss of the ability to perform arithmetical operations and use numerical concepts), and right-left disorientation (right-left discrimination defect when using language). There is some disagreement regarding its exact localization, but it most likely involves the left angular gyrus with a probable subcortical extension. This article reviews recent research on the clinical …
