More than 700,000 cases of Zika virus (ZIKAV) disease have been officially reported to the Pan American Health Organization (PAHO) from 48 countries and territories of the Americas. The response led by the PAHO and partners suggests major lessons of this outbreak. A seemingly innocuous pathogen became the new villain, causing fear, economic losses and, most importantly, debilitating birth defects and neurological problems, reaffirming the well-known war principle of never to underestimate one's opponent. The ZIKAV tested public health capacities under the International Health Regulations, high…

We have reported the epidemic patterns of dengue disease in the Region of the Americas from 1980 through 2007. Dengue cases reported to the Pan American Health Organization were analyzed from three periods: 1980-1989 (80s), 1990-1999 (90s), and 2000-2007 (2000-7). Age distribution data were examined from Brazil, Venezuela, Honduras, and Mexico. Cases increased over time: 1,033,417 (80s) to 2,725,405 (90s) to 4,759,007 (2000-7). The highest concentrations were reported in the Hispanic Caribbean (39.1%) in the 80s shifting to the Southern Cone in the 90s (55%) and 2000-7 (62.9%). From 1980 throu…

The incidence of dengue and dengue hemorrhagic fever has increased dramatically in the past 15 years in most urban centers of the tropics. Coincident with this increase has been the emergence of epidemic dengue hemorrhagic fever in the American region. The current changing disease pattern in the Americas is very similar to that which occurred in southeast Asia 30 years ago. The similarities in the evolution of severe disease in the two regions and the possible reasons for the changing disease pattern are discussed.

This paper describes the significant changes in the relationship between Aedes aegypti mosquitoes and the human population in the Americas from the 1800s to present. First, the history of the arrival of the mosquito, the consequent epidemics of yellow fever and dengue, and the eventual implication of the mosquito as vector is described. Second, the Pan American Health Organization's Aedes aegypti Eradication Program is discussed and the potential threat to the Americas of development of the dengue hemorrhagic fever/dengue shock syndrome (DHF/DSS) is emphasized. Third, the limited efforts of th…

OBJECTIVE: Adrenocortical carcinoma (ACC) is a rare and aggressive malignancy of the adrenal cortex. Given the heterogeneity of healthcare systems across the Americas, this review examines regional differences in access to diagnosis, treatment availability, and clinical outcomes, as well as their association with structural and socioeconomic factors.
DESIGN: Narrative literature review of observational studies and relevant regional evidence.
METHODS: PubMed, Scopus, and Web of Science were investigated for studies published between 2010 and 2025 reporting on diagnostic resources, treatment a…

BACKGROUND: Leprosy remains a persistent public health challenge, where human-to-human transmission of Mycobacterium leprae via respiratory droplets is well established. In the tropical Americas, growing evidence implicates armadillos as important zoonotic reservoirs, particularly through direct contact during hunting and handling. However, such transmission has so far been considered rare and highly localised. This study provides a comprehensive spatial analysis of the role of armadillo hunting in human leprosy transmission, quantifying its contribution to disease prevalence and identifying g…

Angiostrongylus vasorum is a metastrongylid parasite infecting wild canids and domestic dogs. Its patchy distribution, high pathogenicity an … ngylus is larger than expected, since additional species may be circulating in wild canids from the Americas.

The Pan American Health Organization (PAHO) and its Member States have been leading the efforts to eradicate wild poliovirus in the Region of Americas since smallpox's successful elimination in 1971. The region became the first to be certified free of wild poliovirus in 1994. However, in July 2022, an unvaccinated patient with no recent travel history was diagnosed with poliomyelitis in the United States of America. In response to the emergence of a circulating vaccine-derived poliovirus in the United States, PAHO established the Polio Incident Management Support Team. This team has been coord…

BACKGROUND: Robotic intraperitoneal mesh placement (rIPOM) has emerged recently as an alternative to laparoscopic intraperitoneal mesh placement (LapIPOM) for minimally invasive incisional hernia repair. We aimed to compare LapIPOM with rIPOM in terms of hospital length of stay (LOS) and 30-day postoperative complications in patients undergoing incisional hernia repair within the Americas Hernia Society Quality Collaborative.
STUDY DESIGN: Propensity score analysis was used to compare matched groups of patients within the Americas Hernia Society Quality Collaborative undergoing LapIPOM vs rIP…

OBJECTIVE: Reporting a novel mutation in the HTRA1 gene in a CARASIL patient from Americas.
METHODS: Clinical presentation and neuroimaging were consistent with CARASIL. HTRA1 DNA sequencing was performed using advanced ("next generation") sequencing technology. The results revealed a homozygous missense mutation as c.616G>A (p.Gly206Arg) in the HTRA1 gene.
RESULTS: A 24-year-old man with a history of chronic back pain presented with recurrent ischemic strokes. A diagnosis of CARASIL was made with the finding of a novel homozygous missense mutation c.616G>A in HTRA1 gene, resulting in change…

Pulmonary metastrongylosis, a zoonotic disease found primarily in pigs, is caused by eight different species of the cosmopolitan nematode Metastrongylus genus. To date, only four human cases have been reported, all from Europe. Herein, a severe case of pulmonary infection caused by Metastrongylus salmi in an Ecuadorian man, with successful treatment with ivermectin, is described.

OBJECTIVE: To characterize current leptospirosis reporting practices in the Americas.
METHODS: Information was collected from the official websites of national ministries of health from the Americas region and two international organizations; personal communications; and three international morbidity databases. For all sources other than the morbidity databases, the review was limited to official reports citing clinically suspected and laboratory confirmed leptospirosis cases or deaths during the period 1996-2005.
RESULTS: A total of 73 out of 1 644 reports met the selection criteria and wer…

Yellow fever virus (YFV) remains the cause of severe morbidity and mortality in South America and Africa. To determine the evolutionary history of this important reemerging pathogen, we performed a phylogenetic analysis of the largest YFV data set compiled to date, representing the prM/E gene region from 133 viral isolates sampled from 22 countries over a period of 76 years. We estimate that the currently circulating strains of YFV arose in Africa within the last 1,500 years and emerged in the Americas following the slave trade approximately 300-400 years ago. These viruses then spread westwar…
