The long-term prognosis of Transient Global Amnesia: a systematic review.

Transient Global Amnesia (TGA) constitutes an enigmatic amnestic condition. In view of the admittedly limited knowledge regarding the nature of TGA, we decided to systematically review existing evidence for the generally regarded benign course of the disease. MEDLINE, EMBASE, CENTRAL and PsycINFO were searched for relevant articles. Observational (case-control, cross-sectional and cohort) controlled studies were retrieved. TGA diagnosis was made according to the diagnostic criteria of Caplan, validated by Hodges and Warlow. The TGA group was compared with either healthy controls (HC) or/and in

Brain networks in patients with isolated or recurrent transient global amnesia.

OBJECTIVES: The aim of this study was to investigate differences in cerebral blood flow (CBF) and functional networks between transient global amnesia (TGA) patients with a single event and those with recurrent events using arterial spin labeling (ASL) MRI. METHODS: We enrolled patients with TGA and classified them into two groups according to the number of TGA events: TGA patients with a single event and those with recurrent events. MRI scans were performed within 24 h after TGA ictal onset in all patients. We quantified CBF and analyzed the functional network based on CBF using graph theory

Shedding light on the clinical recognition process of transient global amnesia.

BACKGROUND AND PURPOSE: Diagnostic uncertainty is common in the emergency evaluation of neurological conditions such as acute confusional states, particularly for non-neurologists. We aimed to investigate the clinical recognition process of transient global amnesia (TGA) before arrival at the hospital and in the emergency department (ED). METHODS: In this retrospective observational study, medical records of 365 patients with TGA were analysed concerning mode of arrival, symptoms and suspected diagnosis made by pre-hospital medical care providers and the ED neurologist. RESULTS: More than ha

Comprehensive Ultrasound Assessment of the Craniocervical Circulation in Transient Global Amnesia.

OBJECTIVES: Structural changes and metabolic stress have been reported on diffusion-weighted magnetic resonance imaging in the cornu ammonis 1 area of the hippocampus in patients with transient global amnesia (TGA), but a consensus on pathogenesis is still lacking. The aim of our study was to perform a comprehensive ultrasound analysis of the cerebrovascular circulation in our population of patients with TGA. METHODS: One hundred patients with TGA and 50 age- and sex-matched control participants underwent ultrasound examinations of the cervicocranial circulation. RESULTS: The most significan

Vascular Ischemia as a Cause of Transient Global Amnesia: A Patient Series.

INTRODUCTION: Epileptic, migrainous, and vascular pathologies may cause transient global amnesia (TGA); however, the mechanism of causation remains unclear. We investigated possible vascular causes of TGA. METHODS: We retrospectively evaluated the clinical and radiologic studies of 13 patients with TGA. On admission, patients underwent diffusion-weighted imaging (DWI) and intra- and extracranial magnetic resonance angiography (MRA); vascular risk factor profiles for diabetes, hypertension, and hyperlipidemia; electroencephalography; and neuropsychological tests. Seven patients underwent contr

Left dominance of EEG abnormalities in patients with transient global amnesia.

PURPOSE: Transient global amnesia (TGA) is a syndrome of unknown etiology. Electroencephalographic (EEG) abnormalities in TGA have been reported previously. We analyzed the frequency and characteristics of EEG abnormalities in patients with TGA. METHODS: We collected EEGs of patients with a clinical diagnosis of TGA who had visited the emergency room or the outpatient clinic over a period of 8 years and compared clinical and demographic characteristics of the patients with normal EEGs with those with abnormal EEGs. RESULTS: EEG abnormalities were found in 35 (22.9%) out of 153 patients and e

Transient global amnesia as the presenting feature of heparin-induced thrombocytopenia.

A 57-year-old man developed transient global amnesia within an hour of bolus unfractionated heparin administration on day 4 post-mitral valve replacement. Both immunoglobulin G-specific enzyme-linked immunosorbent assay and serotonin release assay were strongly positive for the antibodies that cause heparin-induced thrombocytopenia. The patient's cognitive functions returned to normal following discontinuation of unfractionated heparin and warfarin and commencement of lepirudin infusion.

Ischemic evidence of transient global amnesia: location of the lesion in the hippocampus.

BACKGROUND AND PURPOSE: Transient global amnesia (TGA) is a rare amnestic syndrome characterized by the sudden onset of a selective anterograde and retrograde amnesia with a time course of up to 24 hours. Recent studies have found a high frequency of small high-signal abnormalities in the hippocampus on diffusion-weighted imaging (DWI), and accordingly ischemia has been proposed as an etiology of TGA. We hypothesized that TGA lesions occur preferentially in the CA1 region of the hippocampus, which is known to be susceptible to ischemia. METHODS: Twenty consecutive patients with a clinical dia

Detection of intracranial venous reflux in patients of transient global amnesia.

BACKGROUND: The mechanism of transient global amnesia (TGA) is not clear. Attempting to support the hypothesis that retrograde venous hypertension causing cerebral venous ischemia plays a role in the pathogenesis of TGA, the authors used cranial three-dimensional time-of-flight (TOF) MR angiography (MRA) to detect a possible intracranial retrograde venous flow in TGA patients. METHODS: The frequency of abnormal venous signals on cranial three-dimensional TOF MRA was compared in 10 TGA patients with the signals in 50 age- and gender-matched normal individuals. In TGA patients with abnormal ven

The dynamic time course of memory recovery in transient global amnesia.

AIMS: To investigate the dynamic time course of transient global amnesia (TGA)--that is, the process of recovery and the interindividual variability--by testing four patients during the day of TGA itself (on three occasions) and at follow up (on two occasions). METHODS: A specially designed protocol focusing on semantic (both conceptual and autobiographical knowledge) and episodic (both anterograde and retrograde components) memory. RESULTS: Every patient showed marked impairment of both anterograde and retrograde episodic memory during the acute phase, with a relative preservation of person

Transient global amnesia in a young adult with cyanotic heart disease.

The etiology of transient global amnesia is poorly understood, particularly in children and young adults. Transient global amnesia may follow a wide range of precipitating events. Proposed causes have included vascular event, seizure, and migraine. A young man with cyanotic congenital heart disease experienced an episode of transient global amnesia in the setting of polycythemia. Differential diagnosis of acute confusional episodes in children should include transient global amnesia, as well as confusional migraine, and should include evaluation for underlying coagulation abnormalities and pol

Herpes simplex encephalitis with transient global amnesia as an early sign.

Cerebral ischemia has been proposed as the etiology of transient global amnesia. Recently, however, migranous and epileptic etiologies have attracted attention. A 56-year-old man had transient global amnesia and the next day began to display symptoms of meningoencephalitis. Herpes simplex encephalitis was diagnosed based on the titer of herpes simplex virus antibodies. The transient global amnesia appears to have occurred as an early sign of herpes simplex encephalitis and may have been provoked by an epileptic mechanism.

[The value of the EEG in patients with transient global amnesia (author's transl)].

The EEG findings from 30 patients with transient global amnesia (TGA) of vascular origin are described. A pathological result was seen in 18 cases, from which 17 had predominantly focal changes. 12 EEG's were normal. With increasing time interval between the TGA and the EEG investigation a normal record was slightly commoner. 20 patients had CT results, of which 8 cases were pathological. In 12 cases the EEG and CT results were correspondingly pathological or normal. The EEG shows functional disturbances even after the neurological deficits have resolved, which are related to transient metabol

Transient global amnesia: Signal alteration in 2D/3D T2-FLAIR sequences.

PURPOSE: Transient global amnesia (TGA) is one of the most enigmatic syndromes in clinical neurology. The detection rate of TGA lesions in 2D/3D FLAIR sequences has not been evaluated. METHODS: A total of 201 patients (44 men and 157 women; mean age, 60.34 ± 9.23 years; range, 22-91 years) diagnosed with TGA, who underwent diffusion-weighted imaging (DWI; b = 1000 s/mm2, b = 2000 s/mm2, 4 mm) and/or 2D/3D-FLAIR sequences (4 mm, 0.9 mm; respectively) using 3-T MRI within 28 days after onset of TGA symptoms, were included in this single-center, retrospective, cross-sectional study.
