Myoclonic spasm of an amputated extremity can be problematic for amputees and requires recognition and understanding by surgeons encountering the phenomenon. In the present brief report, we describe the condition in a female amputee after below-the-knee amputation. Our aim is to increase awareness of this condition among foot and ankle surgeons.

This chapter reviews focal dyskinesias that affect a restricted region of the body in isolation. Focal dyskinesias often affect body parts not commonly involved in isolation by movement disorders and are not readily classified into one of the major categories of movement disorders or peripheral nerve excitability syndromes. The use of ultrasound for diagnosing problems in the residual limb of an individual with an amputation has not been documented in the literature. We present a case where this modality was particularly useful in the diagnosis of an extensive abscess in the distal stump. Ultrasound imaging of compromised residual limbs is useful in the diagnosis of stump infections with infected muscle tissue and deep fluid collection. This technique can help shorten the course of intravenous antibiotics by clarifying the need for surgical intervention.

Shape sensing is useful in the computer aided prosthetic fitting process for two purposes. 1. To input characteristic prosthetic shapes that have been developed over the years through the experience of prosthetists. 2. To provide an accurate and rapid measurement of the anatomical shape of the stump. This paper describes two instruments which have been built to meet these objectives.

A computer-aided sculpting system for use in prosthetics is described. The prosthetist's sculpting tools now consist of a computer, a graphics terminal, a mouse and an on-screen moveable cursor. Accompanied by the system software, these tools allow systematic modification of a primitive socket using techniques analogous to those used by a prosthetist working with rasps and plaster.

In 59 above-knee amputations healing of the stumps was correlated with the local skin perfusion pressure (SPP) measured preoperatively as the external pressure required to stop isotope washout using 1318-- or 125I--antipyrine mixed with histamine. Out of the 11 cases with an SPP below 30 mmHg no less than nine (82 per cent) suffered severe wound complications. A young patient who had traumatic amputation of left arm developed extensive chylothorax on right side of chest. The pathophysiology appears to be left subclavian thrombosis secondary to gross sepsis of the amputation stump which resulted in occlusion of thoracic duct opening and chylothorax. Disarticulation of the knee results in an excellent end-bearing stump. Operative technique is easy, fast and gentle. The muscular balance in the femur is not disturbed. The post-amputation (pain) syndrome, including stump pain, phantom limb sensation, and phantom limb pain is common but difficult to treat.
