Persistent sacral sensory deficit induced by intrathecal local anesthetic infusion in the rat.

BACKGROUND: Several cases of cauda equina syndrome after continuous spinal anesthesia have been recently reported. One possible etiology is toxic exposure of the sacral roots resulting from intrathecal maldistribution of a relatively large dose of local anesthetic. The current experiments sought to determine whether a local anesthetic solution, injected intrathecally to produce a restricted distribution of anesthesia, could result in a sacral deficit. In addition, we sought to test the hypothesis that, when equal volumes are administered intrathecally, significant differences exist in the pote

Local anesthetic drugs reduce endothelium-dependent relaxations of porcine ciliary arteries.

PURPOSE: During retrobulbar anesthesia ocular hemodynamics are altered. The purpose of this study is to investigate the effects of local anesthetics in isolated porcine ciliary arteries (diameter between 200 and 400 microns). METHODS: Isolated porcine ciliary arteries were suspended in myograph systems filled with modified Krebs-Ringer solution for isometric tension recording. RESULTS: In quiescent or serotonin-precontracted arteries, lidocaine, bupivacaine, or mepivacaine (10(-9)-10(-5)M) evoked no change in vascular tone. However, all local anesthetics (10(-5)M) and bupivacaine (10(-5)-10(

Pituitary thyrotropin-releasing hormone receptors: local anesthetic effects on binding and responses.

Pharmacological agents are widely used to probe the mechanism of action of TRH. A number of these drugs behave as local anesthetics at high concentrations. The effect of local anesthetics on the binding of [3H]Me-TRH to specific receptors was studied using the GH4C1 line of rat pituitary tumor cells. [3H]Me-TRH binding was inhibited by classical local anesthetics with the order of potency (IC50 values): dibucaine (0.37 mM) greater than tetracaine (1.2 mM) greater than lidocaine (3.3 mM) greater than procaine and benzocaine (greater than 10 mM). IC50 values for other drugs with local anesthetic

Comparative studies of local anesthetic effects on artificial membranes and isolated cardiac tissues.

Drugs with local anesthetic properties like propranolol, tetracaine, lidocaine, procaine, induce concentration dependent potentials in artificial membranes. The size of the drug induced potential correlates with cardiodepressant activity, measured as decrease in force of contraction of guinea-pig atria and papillary muscles. Drugs inducing high potentials like propranolol and tetracaine, show low EC50 values in cardiac tissues. Propranolol in comparison to lidocaine diffuses more rapidly across the lipid membrane as derived from the time dependent decline in membrane potential and from radioac

Local anesthetic effects of cocaine and several extracts of the coca leaf (E. coca).

Cocaine and a number of different fractions of a crude ethanol extract of the coca leaf (E. coca) were subjected to a local anesthetic screen using rat tail withdrawal from electric shock. Following an intradermal injection of 0.1 ml of a 2.0% (w.v) solution of cocaine HCl, an immediate response was observed. Two of the coca fractions also produced some local anesthesia. An alkaloidal fraction, containing an equivalent amount of cocaine, produced a maximum effect that was approximately 20% less than that observed with cocaine. The only other fraction producing any effect, a water soluble cocai

Effect of Intraperitoneal Instillation of Dexmedetomidine With Local Anesthetics in Laparoscopic Cholecystectomy: A Systematic Review and Meta-analysis of Randomized Trials.

OBJECTIVE: Dexmedetomidine (DEX) can strengthen the analgesic effects of local anesthetics (LAs) when used as an adjuvant through intrathecal, perineural, and intraperitoneal routes. Many studies have used intraperitoneal instillation of DEX with LAs in laparoscopic cholecystectomy (LC) to relieve postoperative pain. We performed a systematic review and meta-analysis to synthesize evidence of the efficacy and safety of intraperitoneal instillation of DEX as an adjuvant of LAs in patients undergoing LC. METHODS: A comprehensive literature search of the MEDLINE, PubMed, EMBASE, and Cochrane Lib

Influence of topical application of capsaicin, menthol and local anesthetics on intraoral somatosensory sensitivity in healthy subjects: temporal and spatial aspects.

The aim of this study was to investigate temporal and spatial aspects of somatosensory changes after topical application of capsaicin, menthol and local anesthetics (LA) on the gingiva with the use of intraoral palpometers and thermal devices. Sixteen healthy volunteers (eight male, eight female) participated. Four topical preparations (capsaicin, menthol, LA and Vaseline as a control) were randomly applied to the gingiva around the first premolar in the upper jaw via individual oral templates, which allowed spatial mapping of somatosensory changes at and adjacent to the site of application. Diagnostic lumbosacral segmental nerve blocks with local anesthetics: a prospective double-blind study on the variability and interpretation of segmental effects.

BACKGROUND AND OBJECTIVES: Selective spinal nerve infiltration blocks are used diagnostically in patients with chronic low back pain radiating into the leg. Generally, a segmental nerve block is considered successful if the pain is reduced substantially. Hypesthesia and elicited paresthesias coinciding with the presumed segmental level are used as controls. The interpretation depends on a standard dermatomal map. However, it is not clear if this interpretation is reliable enough, because standard dermatomal maps do not show the overlap of neighboring dermatomes. The goal of the present study i

Local Administration of (-)-Epigallocatechin-3-Gallate as a Local Anesthetic Agent Inhibits the Excitability of Rat Nociceptive Primary Sensory Neurons.

While the impact of (-)-epigallocatechin-3-gallate (EGCG) on modulating nociceptive secondary neuron activity has been documented, it is still unknown how EGCG affects the excitability of nociceptive primary neurons in vivo. The objective of the current study was to investigate whether administering EGCG locally in rats reduces the excitability of nociceptive primary trigeminal ganglion (TG) neurons in response to mechanical stimulation in vivo. In anesthetized rats, TG neuronal extracellular single unit recordings were made in response to both non-noxious and noxious mechanical stimuli. Follo

[Reaction of local myocardial blood flow in non-anesthetized dogs and anesthetized cats to the oral and parenteral administration of a Crateagus fraction (oligomere procyanidines)].

Local blood flow in the myocardium of the left ventricle in unanesthetized dogs was measured by chronically implanted heat-conduction probes. Oral administration of a fraction of Crataegus (oligomere procyanidines) led to a significant rise in blood flow for several hours depending on the dose, the highest increase reaching an average value of about + 70% of the resting flow. In dogs regularly fed with this substance for a longer period of time, a rising tendency of the matutinal resting values of myocardial blood flow towards a maximum was seen. Intravenous application of oligomere procyanidi

Local anesthetic effects in the presence of chronic osteomyelitis (necrosis) of the mandible: implications for localizing the etiologic sites of referred trigeminal pain.

The aims of this study were: (1) to demonstrate how reproducible variations in incomplete anesthesia of the inferior alveolar nerve can be used as a guide to locate the etiologic sites of referred trigeminal pain emanating from the mandible; (2) to describe the salient histopathologic features of a lowgrade, nonsuppurative osteomyelitis seen in this patient population. Forty-six patients with idiopathic facial pain were subjected to a specific protocol of local anesthetic injections to sequentially block branches of the mandibular nerve to determine the effects on his/her pain. If this signifi

Nonallergic hypersensitivity to nonsteroidal antiinflammatory drugs, angiotensin-converting enzyme inhibitors, radiocontrast media, local anesthetics, volume substitutes and medications used in general anesthesia.

Urticaria and angioedema are common allergic manifestations and medications are one of common triggering factors. The most severe immediate drug reaction is anaphylaxis. Apart from the well established IgE-mediated immediate type hypersensitivity reactions, the pathogenesis of drug-induced urticaria, angioedema and anaphylaxis often remains obscure. In this article, emphasis is put on nonallergic reactions to the most commonly used drug groups of nonsteroidal antiinflammatory drugs, angiotensin-converting enzyme inhibitors, radiocontrast media, volume expanders and drugs used in general anesth

Effect of infiltrating nasal packing with local anesthetics in postoperative pain and anxiety following sinonasal surgeries: a systemic review and meta-analysis.

INTRODUCTION: Packing of the nasal cavity has traditionally been used for postoperative bleeding control and decreasing synechia formation in patients undergoing nasal surgeries. Although absorbable nasal packing has been gaining popularity in the recent years, nonabsorbable nasal packing is still often used in nasal surgeries in various parts of the world. It is known to be associated with pain and discomfort especially upon and during removal, and previous reviews have only evaluated the effects of local anesthetic infiltration of nasal packing in septal surgeries. OBJECTIVE: To evaluate th

Correlation between the increased release of catecholamines evoked by local anesthetics and their analgesic and adverse effects: Role of K(+) channel inhibition.

Because local anesthetics are known to inhibit both sodium and potassium channels, and anesthetic properties have been attributed to the former effect, we compared their effects with those of tetrodotoxin (TTX), a selective Na(+) channel inhibitor with anesthetic activity, and 4-aminopyridine (4-AP), a selective potassium channel blocker with convulsive activity, on transmitter release during rest and in response to field (axonal) stimulation using the microvolume perfusion method and isolated prefrontal cortex and spinal cord slice preparations loaded with the radioactive transmitters [(3)H]d
