Hereditary angioedema with an estrogen trigger in a 12-year-old.

BACKGROUND: Hereditary angioedema (HAE) is rare autosomal dominant genetic disorder, commonly affecting girls around the menarche, which manifests clinically as recurrent episodes of angioedema. Laryngeal edema can lead to asphyxiation and death. Traditionally hormones have been avoided in the management due a reported association with flares in the literature. This case describes an alternative management with a progestin. CASE: A 12 year old HAE sufferer failed to receive relief from her symptoms of angioedema with standard treatment. A trial of depot medroxyprogesterone acetate has resulte…

Current update on cellular and molecular mechanisms of hereditary angioedema.

OBJECTIVE: To provide an update on the molecular mechanisms of hereditary angioedema (HAE). DATA SOURCES: MEDLINE and PubMed databases were searched to identify pertinent articles using the following key terms: hereditary angioedema, angioedema, C1 inhibitor, bradykinin, contact system, factor XII, mechanism, pathophysiology, severity, permeability, and estrogen. STUDY SELECTIONS: Articles were selected based on their relevance to the subject matter. RESULTS: Although the biochemical basis of "classic" HAE is known to result from C1 esterase inhibitor (C1INH) deficiency, a new form, HAE wit…

Diagnosis and management of hereditary angioedema: an emergency medicine perspective.

BACKGROUND: Hereditary angioedema (HAE) is a rare and often debilitating condition associated with substantial morbidity and mortality in the absence of appropriate intervention. An underlying deficiency in functional C1-inhibitor (C1-INH) protein induces a vulnerability to unchecked activation of the complement, contact, and coagulation/fibrinolytic systems. The clinical consequence is a pattern of recurring attacks of non-pitting, non-pruritic edema, the urgency of which varies by the affected site. Laryngeal edema can escalate rapidly to asphyxiation, and severe cases of abdominal swelling …

[Successful extubation in a patient with alacepril-induced tongue angioedema].

We describe a case of marked swelling of the tongue in a patient after a long prone position neurosurgery, who was treated for hypertension with alacepril. The tongue was not congestive and history of taking an angiotensin-converting enzyme inhibitor suggested an alacepril-induced angioedema. The day after surgery, the patient's tongue was still swollen; however, fibrescopic observation of the larynx and pharynx through the nasal cavity revealed that the edema was limited to anterior tongue, and the oral cavity and oropharynx were not involved. Although, tongue angioedema was observed, we intr…

Angioedema and urticaria associated with omeprazole confirmed by drug rechallenge.

Omeprazole is a substituted benzimidazole that has gained widespread use in the treatment of acidic and peptic ulcer disease. Adverse events with the drug are rare and involve mainly the gastrointestinal and central nervous systems. Skin inflammation, urticaria, pruritus, alopecia, and dry skin have been reported in 0.5-1.5% of patients. To date, no published report has linked angioedema with omeprazole. We report a case of a 34-year-old woman with cellulitis, ulcerative erosive esophagitis, and gastric and duodenal ulcers who developed several hypersensitivity reactions characterized by short…

Management of hereditary angioedema in the UK: a comparison with international guidelines.

Hereditary angioedema (HAE) is a rare genetic disorder characterised by recurrent episodes of swelling, which can potentially be life-threatening, resulting in substantial physical, psychological, educational and occupational burden. Treatment of HAE is divided into on-demand treatment for acute attacks and long-term prophylaxis for prevention of attacks, and there are now several effective therapies available. However, in the UK, use of on-demand therapy is intended for attacks defined as clinically significant, and access to long-term prophylaxis is determined primarily by frequency-based cr…

[Clinical aspects and diagnostics of mast cell-mediated angioedemas].

Angioedema is defined as transient swelling of the skin and/or mucous membranes. Based on the underlying mediator, angioedema is essentially classified as either mast cell-mediated or bradykinin-mediated. In general, mast cell-mediated angioedema is of shorter duration (1-3 days) and responds to antihistamines, corticosteroids and adrenaline, whereas swelling associated with bradykinin-mediated angioedema typically persists longer and does not respond to histamine-targeted drugs. Mast cell-mediated angioedema includes swelling occurring as part of allergic (IgE-mediated) or pseudoallergic (non…

Intramuscular Compound Betamethasone for the Emergent Management of Lingual Angioedema.

A 38-year-old male developed acute tongue swelling with slurred speech 2 hours after taking amoxicillin. On the basis of the patient's history, physical and oral examination, and laboratory findings, the diagnosis was confirmed as histamine-mediated lingual angioedema. Treatment included immediate discontinuation of amoxicillin, intramuscular injection of compound betamethasone, and oral levocetirizine. Tongue swelling resolved completely within 1 hour, with no recurrence at 3-day and 1-week follow-ups. The occurrence of histamine-mediated angioedema solely in the tongue is highly uncommon. Su…

Isolated angioedema in the emergency department: rarely an allergic reaction.

Angioedema is a common, potentially life-threatening emergency characterized by swelling of subcutaneous or submucosal tissues. While allergic reactions are common, a substantial proportion of ED angioedema presentations, particularly those without urticaria, involve non-allergic, bradykinin-mediated mechanisms. Differentiating between these pathways is critical, as standard anaphylaxis treatments like epinephrine are often ineffective for non-allergic variants. A key clinical indicator is the presence or absence of urticaria; isolated angioedema is rarely an allergy and suggests a bradykinin-…

Pain management in patients with hereditary angioedema: A case report.

RATIONALE: Hereditary angioedema (HAE) is a rare genetic disorder caused by C1 esterase inhibitor (C1-INH) deficiency or dysfunction. Presentation with isolated abdominal pain is uncommon, often leading to diagnostic delays and inadequate pain management. Effective pain control is essential to improve patient comfort and prevent complications. PATIENT CONCERNS: A 21-year-old male with type II HAE presented exclusively with recurrent severe abdominal pain and experienced a transient loss of consciousness attributed to a pain-induced vagal reflex. DIAGNOSES: Diagnosis of type II HAE was confir…

Physiological evaluation of the emotional regulation of patients with hereditary angioedema.

OBJECTIVE: Hereditary angioedema (HAE) is a rare disease that presents with recurrent attacks that can be triggered by stress or other emotions. The emotional impact of HAE is well-documented, although mainly via self-reported questionnaires. In this study, we used physiological measures to investigate stress regulation in patients with HAE and compared them to patients with immune thrombocytopenia (ITP), a disease that also progresses with attacks but without emotional triggers. METHODS: Twenty-six adult patients with HAE or ITP (13 each) were included in the study. Patients were asked to per…

Exposure-Response Analysis of Donidalorsen for the Treatment of Hereditary Angioedema.

Hereditary angioedema (HAE) is characterized by recurrent attacks of severe tissue swelling. In the OASIS-HAE phase 3 study (NCT05139810), donidalorsen, an RNA-targeted antisense oligonucleotide that reduces prekallikrein production in the liver, significantly reduced HAE attack rates. To characterize the relationship between prekallikrein concentrations and HAE attack rates following donidalorsen and predict the efficacy of potential dosing regimens, an exposure-response model was developed using data from OASIS-HAE. Simulations were conducted to evaluate the following regimens: 80 mg once ev…

Neutrophilic Urticarial Dermatosis With Angioedema: A Diagnostic and Therapeutic Challenge.

Neutrophilic urticarial dermatosis (NUD) is a rare subtype of neutrophilic dermatoses (NDs), presenting with recurrent erythematous papules and plaques often resistant to conventional therapies and associated with systemic symptoms such as fever and polyarthritis. Here, we report the case of a 32-year-old female with chronic pruritus, hives, and angioedema, unresponsive to antihistamines and progressing to diffuse urticarial lesions. Initial differential diagnoses included chronic urticaria, systemic lupus erythematosus, and Schnitzler syndrome. Laboratory testing showed elevated C-reactive pr…

Genital Attacks in Hereditary Angioedema and Their Effects on Sexual Life.

Background and Objectives: Hereditary angioedema (HAE) is characterized by unpredictable skin and mucosal angioedema attacks. We aimed to find the frequency of sexual-activity-triggered attacks (STAs) and understand how the sexual life of HAE with C1-inhibitor deficiency (HAE-C1INH) patients is affected. Materials and Methods: Adult HAE-C1INH patients were included in this cross-sectional study, which started in March 2020. Demographic information, marriage properties, gender-specific sexual life characteristics, and the HAE-specific histories of the patients were collected. The Hospital Anxie…
