In 210 patients divided in 7 groups depending on surgical disease or pregnancy the authors estimated pH of the gastric content during the preoperative period. It was found that pH was below 2.5, i.e. the acid level of the gastric content was high enough to present a hazard for appearance of Mendelson's syndrome, should eventual regurgitation or aspiration occur during or after general anesthesia. The effectiveness of cimetidine for symptomatic relief in patients with chronic gastroesophageal reflux was studied in a multicenter, double blind clinical trial. Patients were entered into the study for a total of 8 weeks, receiving either cimetidine, 300 mg four times daily, or identical placebo tablets. A case of metastatic myocardial calcification is reported in a patient with chronic renal failure. The characteristic features are failure to take phosphate-binding antacids on a regular basis, intractable congestive heart failure, atrioventricular block, a calcium phosphate product consistently greater than 60, and sudden irreversible cardia arrest. Dyspepsia is a common problem in ambulatory care. While many authorities recommend ordering upper gastrointestinal x-ray series (UGI) before therapy, there is evidence that clinicians frequently treat dyspepsia without diagnostic procedures. This case report describes a 69-year-old woman on long-term pantoprazole therapy, initially prescribed for nonspecific upper gastrointestinal symptoms. Despite resistance to discontinuation due to rebound symptoms, a gradual dose reduction combined with patient education and use of antacids led to successful cessation. The case highlights the importance of regular medication review, patient involvement, and awareness of rebound acid hypersecretion in facilitating rational PPI use and deprescribing efforts.

In Part II, we consider the anti-inflammatory salicylates, nonspecific antidiarrhoeal agents, laxatives and cathartics.

Gastro-oesophageal reflux disease results from impaired function of the LOS and acid clearance of the distal oesophagus. Most patients do not require investigation and respond either to lifestyle changes, antacid/alginates, H2A, PPI or a combination of these treatments. Surgery is only rarely indicated.

Both genetic and environmental factors seem to predispose to the development of food allergy. A most notable factor is diet, particularly during infancy. Possible other factors include maternal diet during pregnancy and lactation, birth by cesarean section, exposure to tobacco smoke, multivitamin supplementation, and intake of antacids. It is important to identify and control such risk factors to reduce the development of food allergy.

Duodenal ulcer healing was followed by endoscopy in 186 ethoxzolamide-treated patients. Ethoxzolamide was given in doses of 5-10 mg/kg body weight/day in association with Na and K salts to avoid electrolytic losses. A control group of 161 duodenal ulcer patients received the usual doses of antacids and anticholinergics. The clinical course of pain, inhibition of gastric acid secretion and endoscopic healing of ulcers after 15 and 21 days of treatment were followed, together with the incidence of relapses over a 2-year period. In 156 children suffering from chronic gastroduodenitis, secretory gastric function was found to be impaired in the stage of exacerbation. Comparative assessment of different methods of that impairment correction revealed antacid therapy to be of the highest efficacy. Meanwhile reflexotherapy turned out the most effective approach during antirelapse treatment.
