Synchronous primary mucinous adenocarcinomas in the descending colon and perianal glands.

The descending colon is a relatively rare site for mucinous adenocarcinoma. Anal ductal malignancy and multiple primary coloperianal mucinous adenocarcinomas are also rare. We have described the unusual finding of multiple primary synchronous mucinous adenocarcinomas in the descending colon and perianal glands. There was no evidence of any metastasis from the Dukes B lesion in the descending colon.

HPV-driven cancer: from epidemiology to the HPV-driven tumor board proposal, everything you wanted to know but were afraid to ask.

BACKGROUND: Human papilloma virus (HPV)-related cancers are well-known clinical entities in different body sites. Cervix, anus, and oropharynx are the most common tumors related to the virus infection. The aim of this work was to highlight similarities and differences among HPV-related cancers of different subsites. METHODS: The narrative review was focused on the following topics: (1) epidemiology, (2) prognostic factors, (3) treatment approaches, (4) radiomic analysis, (5) radiosensitivity, (6) de-escalation strategies. Common aspects as well as differences have been highlighted for cervica

[Nine-valent HPV vaccine - new generation of HPV vaccine].

Huntington's disease (HD) is a neurological disorder characterized by motor disturbances. HD pathology is most prominent in the striatum, the central hub of the basal ganglia. The cerebral cortex is the main striatal afferent, and progressive cortico-striatal disconnection characterizes HD. We mapped striatal network dysfunction in HD mice to ultimately modulate the activity of a specific cortico-striatal circuit to ameliorate motor symptoms and recover synaptic plasticity. Multimodal MRI in vivo indicates cortico-striatal and thalamo-striatal functional network deficits and reduced glutamate/

Pelvic cancer recurrence: drop the arrow in the target.

BACKGROUND: Local recurrence of pelvic cancer is a therapeutic challenge. Medical history retrieve frequently high dose radiotherapy and multiple pelvic surgery. Thus, surgical resection is complex and leads to hazardous postoperative courses. We report a case of radiofrequency thermoablation (RFTA) of an isolated pelvic recurrence. CASE: A 53-year-old woman presented in 2006 with a small (20 mm) pelvic recurrence of an anal canal cancer initially treated by radiochemotherapy and amputation of the rectum. The recurrence was localized in the left vaginal wall, involving the obturator internus

Anorectal Mucosal Melanoma: A Case Report and Literature Review.

BACKGROUND Anorectal mucosal melanoma (AMM) is a rare and aggressive neoplasm, with a 5-year survival rate of 10%. Due to its rarity and nonspecific symptoms, the diagnosis is often made late. Surgical resection remains the criterion standard for treatment of anorectal melanoma. CASE REPORT We present the case of an 81-year-old woman presenting with hematochezia, anal secretion, tenesmus, difficulty in defecation, and perianal pain. On physical examination, there was a prolapse of a 5-cm melanocytic nodule in the anal canal, hard on palpation. Biopsy confirmed anorectal melanoma. Staging revea

Biological mesh used to repair perineal hernias following abdominoperineal resection for anorectal cancer.

PURPOSE: This study aimed to determine the outcome for patients who had undergone perineal hernia repair, via a perineal approach, using a biological mesh post-abdominoperineal excision (APE) for anorectal cancer. METHOD: All consecutive patients having undergone perineal hernia repair involving an extracellular matrix of porcine small intestinal submucosa at our hospital between 2015 and 2018 were included. Follow-up clinical examinations and computed tomography scans were performed. RESULTS: Six patients were treated surgically for symptomatic perineal hernia after a median of 31 months fr

Pathologic Features of Primary Colon, Rectal, and Anal Malignancies.

In the United States, colorectal cancer is the third most commonly diagnosed cancer in both men and women, as well as the third leading cause of cancer deaths (Colorectal cancer facts & figures 2014–2016, 2014 [2]). Worldwide, colorectal cancer is the fourth leading cause of death and causes almost 700,000 deaths each year (Cancer: fact sheet No. 297, 2015 [55]). This chapter discusses the clinical and pathologic features of the spectrum of epithelial, hematolymphoid, and mesenchymal malignant tumors of the colon, rectum, appendix, and anus.

Comparison of the time and latitude trends of melanoma incidence in anorectal region and perianal skin with those of cutaneous malignant melanoma in Norway.

BACKGROUND: Melanoma incidence is increasing in many parts of the world. The main environmental risk factor is exposure to solar radiation. However, melanomas may arise also on non-sun-exposed areas (uveal and mucosal melanomas) and little is known about a possible relationship between sun exposure and melanoma on such locations. OBJECTIVES: We have compared the time and latitude trends of melanoma incidence in the anorectal region and perianal skin (non-sun-exposed sites) with those of cutaneous malignant melanoma (CMM) (sun-exposed skin) to gain more information about the relationship betwe

[Results of cryosurgery in the treatment of inoperable tumor stenoses of the anus and rectum].

Inoperable tumor stenosis of anus and rectum may be treated surgically only by implantation of artificial anus. Cryosurgery proves to be a feasible alternative in order to maintain free passage of the gut. The cryoprobe is introduced through a rectoscopy under direct view. Visible tumor is touched with the tip of the cryoprobe and subsequently temperature is lowered to approximately -19 degrees C. Usually anesthesia is not required. From 1976 to 1989, 213 patients had been treated by cryosurgery in our department. In 23 cases the use of an artificial anus became inevitable afterwards.

Treatment of invasive squamous cell carcinoma complicating anal Buschke-Löwenstein tumor: a case history.

We present here the case of a patient with Buschke-Löwenstein tumor of the anus complicated by invasive squamous cell carcinoma metastatic to the inguinal and periaortic pelvic lymph nodes. Successful local palliation was achieved with 5-fluorouracil and mitomycin-C-based chemotherapy combined with radiation therapy, but systemic progression was unaffected.

Outpatient anorectal surgery.

Over a period of 16 months, three-fourths of the proctologic surgery performed by our clinic was done on an outpatient basis. By doing so, 1,200 patient visits and approximately 300 histories, physicals, and discharge summaries are eliminated, while obviously benefiting patients and reducing health care costs. Thus, we can conclude that hospitalization is not necessary for the majority of proctologic surgery patients.

Anal human papillomavirus infection and its relationship with abnormal anal cytology among MSM with or without HIV infection in Japan.

Anal high-risk human papillomavirus (hr-HPV) infection is widely considered a cause of anal cancer. However, epidemiological data are quite limited in Japan. This study investigated anal HPV infections and cytological abnormalities among MSM with or without HIV infection. Anal swabs were obtained, and cytological results were examined. Hybrid capture-based methodology was used for hr-HPV genotyping. The exclusion criterion was a history of vaccination against HPV. 644 subjects participated, and the overall prevalence of hr-HPV was 59.7% (95% CI 54.7-62.3), HIV-infected had higher prevalence th

MRI evaluation of anal and perianal diseases.

Anal and perianal region is a commonly affected area in the course of several inflammatory, infectious, and neoplastic diseases. Several imaging modalities may be used in imaging evaluation of this area and magnetic resonance imaging (MRI) emerges as the imaging modality of choice due to its superb soft tissue resolution. MRI is not only useful for initial detection of anal/perianal pathologies but also in the follow-up of these disorders. In this article, we aimed to illustrate MRI findings of several diseases affecting this area including perianal fistula as well as anal fissure, hypertrophi

Prevalence of anal human papillomavirus infection and anal HPV-related disorders in women: a systematic review.

The aim of this study was to systematically review the findings of publications addressing the epidemiology of anal human papillomavirus (HPV) infection, anal intraepithelial neoplasia, and anal cancer in women. We conducted a systematic review among publications published from Jan. 1, 1997, to Sept. 30, 2013, to limit to publications from the combined antiretroviral therapy era. Three searches were performed of the National Library of Medicine PubMed database using the following search terms: women and anal HPV, women anal intraepithelial neoplasia, and women and anal cancer. Publications wer
