INTRODUCTION: Arachnoid cysts are cavities with a content similar to cerebrospinal fluid, frequently communicating with the subarachnoid space. They make up 1% of the intracranial space occupying lesions, and although typically seen in children (13%), they may be undiagnosed until the patient has become adult.
PATIENTS AND METHODS: We review a series of 35 cases of congenital intracranial arachnoid cysts in children, which had been investigated in the Neuropaediatric Department during 1987 1999. The patients had had cranial CAT and MR, and transcranial echography studies.
RESULTS: In 85.7% t

BACKGROUND: It remains open to debate why totally removed benign meningiomas recur. Two recurrent cases forced us to reconsider something corresponding to their recurrence that we had overlooked during Simpson grade I surgery.
METHODS: This study is based on 24 recent and 9 earlier cases in which benign convexity meningiomas were totally removed by Simpson's grade I surgery. Tough or thick arachnoid membranes continuing to normal arachnoid membranes and contiguous to meningiomas but different from dura mater were encountered in 11 recent and at least 2 earlier cases. Such thick arachnoid memb

We present a case of suprasellar arachnoid cyst which was revealed by visual impairement and hypopituitarism. Neuroradiological imaging showed the peculiar association of the suprasellar cyst with cerebellar tonsillar herniation and a large asymptomatic cervical syringomyelic cavity. Surgical treatment of the suprasellar cyst allowed the reduction of both the cyst and the syrinx. A common pathophysiological mechanism of these lesions is discussed. We suggest the possibility that an initial obstruction of the basal cisterns caused the suprasellar cyst formation which led to medullar cavity form

BACKGROUND: Arachnoidal cysts are cystic lesions filled with a cerebrospinal-fluid-like content within the leptomeninges. Usually arachnoidal cysts represent a congenital malformation. Clinical signs (increased intracranial pressure and/or neurological defects) may become apparent in children or young adults.
PATIENT: A sixteen year old boy was referred to our outpatient clinic because of blurred vision in both eyes since five days. The patient complained of cephalgia and of an increased sleep requirement in the last two weeks. Morphological findings were within the normal range. The visual a

INTRODUCTION: It is very unusual to find cysts situated intraventricularly. Only 24 cases have been described to date in the literature.
CLINICAL CASES: In this paper we present two new cases considering that because of their exceptional size they deserve to be called 'giant'. The two patients had been diagnosed as having epilepsy during childhood or youth. No neuroradiological studies had ever been done. The cause of hospital admission in the first case was headache and vomiting and in the second case a convulsive crisis. We did cerebral CT scans in both cases and a MR control scan in the se

In two female individuals (37 and 47 years old) sacral intradural arachnoid cysts caused low back pain and sciatica for several years until the correct diagnosis was established by MR imaging. Both patients had micturition disturbances (pollakisuria). In both cases a sacral laminectomy was carried out and the dorsal border of the cyst wall was resected (marsupialisation) after evacuation of xanthochrome fluid. Neither cysts showed any communication with the subarachnoid space. Postoperatively the patients were free of complaints; pollakisuria disappeared within 2-4 weeks.

Human spinal dura and arachnoid, obtained during neurosurgical operations, were studied by transmission electron microscopy. The ultrastructure of spinal meninges largely conformed to the morphology of the cranial meninges, but some minor differences were detected. The dura was composed of an outermost loosely arranged fibroelastic layer, a middle basically fibrous portion and an innermost cellular layer (dural border cell layer). The dural border cell layer was characterised by multiple interdigitating cell processes, no extracellular collagen, significant extracellular spaces and few cell ju

Stereotactic cysto-ventricular shunting in three patients with congenital (subependymal cyst of the 3rd ventricle, subependymal cyst of foramen of Monroi, cyst of cavum septi pellucidi) is dealt with in this paper. The first operation was performed in May 1992 and the latest, being considered in this paper, in January 1993. All patients were admitted to our hospital suffering from increased intracranial pressure. CT-scans revealed an obstructive hydrocephalus subjected to the cystic lesions. After stereotactic puncture of these cysts, aspiration of their contents as well as biopsy of the wall

A sacral arachnoid cyst was found incidentally in a patient with ankylosing spondylitis. The clinical manifestations were perithecal bony erosions and thecal sac displacement while no associated neurological deficit was detected. This peculiar situation is of special interest as cauda equina syndrome invariably presented in such clinical complex. The pathogenesis of the arachnoid cyst remains unclear. And surgical intervention is only indicated in patients with evidence of nerve root compression.

Arachnoid cysts of the CNS are uncommon lesions of considerable interest and importance, the origins and exact nature of which remain uncertain (congenital, posttraumatic, infectious or other). They form a well defined pathological entity and account for less than 1% of all CNS space-occupying lesions. Their membranous walls resemble slightly thickened arachnoid with CSF-like fluid. They are loosely attached to the dura mater and cerebral cortex, and are often adherent along the base of the skull or falx or located next to midline of the posterior fossa. A variety of clinical problems may be p

A rare case of arachnoid cyst in the trigone of a lateral ventricle is reported. The patient was an 8-year-old boy who had had four episodes of convulsions prior to admission. Computed tomography (CT) and magnetic resonance imaging (MRI) demonstrated a cystic lesion containing fluid resembling cerebrospinal fluid. Although he received a cyst-peritoneal shunt, the lesion did not decrease in size. Direct removal of the cyst was then scheduled. The entire cyst was finally removed, although it was firmly attached to the choroid plexus. The enlarged trigone gradually decreased in postoperative CT.

A case of quadrigeminal cistern arachnoid cyst associated with hydrocephalus is reported. A 1-year-old girl was admitted to our service on July 31, 1984, because of mental retardation and an enlarged head. She was born of a full-term pregnancy and normal vaginal delivery without prenatal complications. Progressive increase in her head circumference was noticed at the age of 6 months by her family physician. On admission she was found to be a well-nourished infant with a head circumference of 56 cm, bulging anterior fontanelle and mental retardation. Marked dilatation of the lateral ventricles

Although associations of a variety of developmental abnormalities of the brain and the spinal cord with neurofibromatosis have been reported, association of a cerebral arachnoid cyst with neurofibromatosis is extremely rare. We report the second case of this association in an 8-year-old Japanese boy.

Clinical, radiological, and therapeutical aspects of 34 cases of childhood intracranial arachnoid cysts are reviewed. Two thirds (23) were located supratentorially, the remaining 11 had an infratentorial localization. Of the first group 15 were situated at the sylvian region, 6 were hemispherical and 2 suprasellar. Clinical manifestations were present before 6 months of age in 68% of all the cases. Macrocephaly and focal seizures were the most common clinical findings. Skull X-Ray examination was helpful only when cysts were located at sylvian region. CT Brain Scan was positive in all cases (3

A 4-year-old boy with suprasellar arachnoid cyst was reported. At the age of 30-month-old his aunt was aware of his squint. During the observation by ophthalmologists from the age of 1y. to 3y., enlargement of the head and impairment of the visual acuity were manifested. Cranial CT scan revealed the enlargement of the ventricular system and a round low density area located superior to the sella. Absorption coefficient of the lesion was similar to that of the cerebrospinal fluid. No abnormal contrast enhancement was seen. Examination revealed the head circumference of 53.3 cm larger than double
