BACKGROUND: Children with speech disorder are a heterogeneous group (e.g. in terms of severity, types of errors and underlying causal factors). Much research has ignored this heterogeneity, giving rise to contradictory intervention study findings. This situation provides clinical motivation to identify the deficits in the speech-processing chain that underlie different subgroups of developmental speech disorder. Intervention targeting different deficits should result in a differential response to intervention across these subgroups.
AIMS: To evaluate the effect of two different types of thera

Speech development, the occurrence of articulatory errors, speech therapy received and literacy were evaluated in children at preschool and school age. Data were obtained with questionnaires sent to the parents and teachers of 1,708 second-grade children in 119 school classes selected by multistage random sampling among Finnish-speaking schools throughout the country. Completed questionnaires were received from 1,531 parents (89.6%) and 1,601 teachers (93.7%). Early speech development was slower among the boys than among the girls. The proportion of children with articulatory errors decreased

Identification of children with specific language impairment (SLI, now known as Developmental Language Disorder) remains challenging. Morphosyntax difficulties have been proposed as potential linguistic 'markers' for SLI across a number of languages. This study investigates the existence of such a clinical marker in German-speaking children with SLI, looking in particular at German case marking, and makes comparisons with matched typically developing groups and a group with isolated phonological impairment (PI). A case-control study was completed with 66 pre-school children in four groups (1)

Auditory processing problems in persons with dyslexia are still subject to debate, and one central issue concerns the specific nature of the deficit. In particular, it is questioned whether the deficit is specific to speech and/or specific to temporal processing. To resolve this issue, a categorical perception identification task was administered in thirteen 11-year old dyslexic readers and 25 matched normal readers using 4 sound continua: (1) a speech contrast exploiting temporal cues (/bA/-/dA/), (2) a speech contrast defined by nontemporal spectral cues (/u/-/y/), (3) a nonspeech temporal c

Every symptom and complaint can be of psychogenic origin. In order to tell apart an organic disease from a psychogenic disturbance one has to look for objective findings, which correlate with the symptoms. Cases are presented, which illustrate psychogenic disorders, but also cases, which seem to be psychogenic but in fact are of organic origin.

The diagnosis and evaluation of velopharyngeal concerns is complex. It involves a variety of assessment techniques--both indirect, such as speech, and direct, using instrumentation to delineate how the velopharyngeal structures function. However, the most important tools we have for making decisions regarding velopharyngeal function and need for management are the speech characteristics. From thorough analysis of speech, there is much that can be learned about the status of velopharyngeal function and potential for change. The ultimate question that the speech diagnostician must attempt to ans

Phonetic transcriptions of 48 babbling samples from 11 normally hearing subjects, aged 4-18 months, and 39 samples from 14 hearing-impaired (HI) subjects, aged 4-39 months, were analyzed to determine the inventory of consonantal phones for each recording session. Group comparisons revealed that inventories of the hearing-impaired subjects were generally smaller than those of the hearing infants and that they evidenced little change in size prior to 20 months. Analysis of place and manner of articulation of supraglottal phones in the inventories showed that, compared with inventories of the nor

It has been suggested that a child's productive phonological knowledge may be one factor that potentially accounts for individual differences in generalization learning observed among phonologically disordered children (Dinnsen & Elbert, 1984; Elbert, Dinnsen, & Powell, 1984). This paper evaluates the hypothesis that productive phonological knowledge influences generalization. Three related studies involving 6 functionally misarticulating children were conducted. In the first study, a description of each child's phonological system was developed using procedures of standard generative analysis

This study examined the incidence of behavioral symptoms in youngsters with tic disorders. A total of 186 patients were studied using the Child Behavior Checklist and a Motor/Vocal Tic Severity Scale. The relationship between tic severity and the frequency of behavioral symptoms was found to be not simply linear, but influenced by the presence of patients without significant behavioral problems in spite of severe tics (resilient patients) and patients with very negative adjustment although their tic disorder was mild (vulnerable patients). Behavioral problems were not related to age. Patients

PURPOSE: Functional speech disorders (FSDs), a subtype of functional neurological disorders, are distinguishable from neurogenic motor speech disorders based on their clinical features, clinical course, and response to treatment. However, their differential diagnosis and management can be challenging. FSDs are not well understood, but growing evidence suggests a biopsychosocial basis distinct from structural lesions that cause neurogenic motor speech disorders.
METHOD AND RESULTS: Following an overview of FSDs, four patients are described to illustrate the range of clinical manifestations, bi

Autism spectrum disorder (ASD) is a neurodevelopmental disorder that causes deficits in social interactions, communication skills, intellectual limitations, and self-injurious behaviors. Common systemic comorbidities include gastrointestinal issues, obesity, and cardiovascular disease. Recent study has found a link between gut microbiota alterations and neurobehavioral symptoms in children with ASD. Physical activity and exercise therapy have been demonstrated to promote communication and social interaction while also positively influencing microbiota composition. This research intends to high

Neuropathic arthropathy is a chronic, progressive degenerative disorder affecting one or more peripheral or vertebral articulations, which develops as the result of a disturbance in the normal sensory (pain or proprioceptive) innervation of joints. Diabetes, syphilis, and syringomelia are the most commonly associated clinical entities. When neuropathic arthropathy is suspected, careful clinical evaluation should be performed to identify an underlying neurologic disorder. Patient education, joint protection, and early recognition of fractures are the most important general management principles

OBJECTIVE: The objective of the study was to determine the most common speech impairments among mouth breathing (MB) children and to assess … arious speech abnormalities and malocclusion.
CONCLUSION: The findings of the study suggested that articulation disorders were frequently associated with MB children. It emphasizes the necessity of monitoring MB children through a multidisciplinary approach to prevent the adverse effects of MB and improve the overall development of individuals.

INTRODUCTION: Disorders affecting the lower motor neurons in childhood, with a congenital or acquired aetiology, give rise to difficulties in neuromotor response and, therefore, motor disorders affecting speech in a period that is especially critical for the development of language. The low incidence of this pathology, its comorbidity with other brain conditions and its uncertain prognosis make it a particularly interesting area of study.
AIMS: The purpose of this work is to review the motor disorders affecting speech in flaccid dysarthria, together with its functional evaluation and speech t

In the Netherlands, autonomypromoting policy (APP) is increasingly applied to patients with chronic suicidality and self-harming behaviours for whom specialized psychotherapy for personality disorders is not yet feasible. A national clinical framework and a critical evaluation by MIND have recently highlighted the urgent need to articulate the psychotherapeutic principles underpinning APP. In this article we explore the practical implementation of APP, grounded in interventions from various evidence-based psychotherapeutic modalities for personality disorders that emphasize autonomy. The musculoskeletal structures of the thoracic wall and the neck are a relatively common source of chest pain. Pain arising from these structures is often mistaken for angina pectoris, pleurisy or other serious disorders. In this article the clinical features, pathogenesis and management of the various musculoskeletal chest wall disorders are discussed. The more common causes are costochondritis, traumatic muscle pain, trauma to the chest wall, "fibrositis" syndrome, referred pain, psychogenic regional pain syndrome, and arthritis involving articulations of the sternum, ribs and thoracic spine

The wrist is one of the most complex articulations in the human body. The distal radioulnar joint (DRUJ) is particularly controversial, enduring as the area least understood with regard to anatomy and pathomechanics. The popular systems of classifying DRUJ disorders are based on etiology and treatment, but this approach has inspired schemes that are cumbersome, redundant, and incomplete. The need for an improved system prompted us to devise a simplified system that is based on the pathomechanics of dysfunction and the mechanical requirements for optimal joint function.
