[Persistent cerebellar ataxia following typhoid fever].

We report a patient who developed acute cerebellar ataxia, delirium, disseminated intravascular coagulation (DIC), and acute renal failure following typhoid fever. After treatment his general conditions improved except for cerebellar ataxia. Brain magnetic resonance imaging (MRI), before and after intravenous gadolinium administration, showed slight atrophy of the cerebellar vermis. 1-123 iodoamphetamine single photon emission computed tomography (IMP-SPECT) images showed markedly reduced radioisotope (RI) uptake in the cerebellum, and strongly implicated dysfunction of the cerebellum. Our rep…

Cerebellar ataxia and muscle spindle sensitivity.

1. The cerebellum has long been known to participate in movement control. One of the enduring theories of cerebellar function is that it "tu … direct information is available on animals or humans performing voluntary movements and exhibiting ataxia or other cerebellar symptoms. 4. We tested the hypothesis by recording from nine muscle spindle afferents in behaving cats before and during reversible inactivation of cerebellar interpositus and dentate nuclei. In normal cats fusimotor action varie …

Atypical clinical presentation of ataxia telangiectasia.

Ataxia telangiectasia (AT) is an autosomal recessive disorder characterized by progressive cerebellar ataxia, oculocutaneous telangiectasias, immunodeficiency with recurrent infections, IgA and IgE deficiency, and increased incidence of malignancies. The pathognomonic biological abnormalities consist of spontaneous chromosomal instability resulting in a high in vivo occurrence of cells with translocations, especially involving chromosomes 7 and 14, and a relative insensitivity of DNA replication in vitro to radiation exposure. We report on a patient with the biological hallmarks of AT but with…

[Cerebellar ataxia and acute cerebellar insult].

Ataxia, loss of muscle coordination during complex movement, is the most important sign of cerebellar lesion. Acute cerebellar attack of ischaemic and/or haemorrhagic nature is a focal disorder of cerebellar function which occurs rarely considering the prominent collateral net of blood vessels conveying blood to the cerebellum and the brain stem. We examined 12 patients with acute cerebellar attack (9 with ischaemic and 3 with haemorrhagic attack) of 300 patients with symptoms and signs of acute cerebrovascular attacks. We used all available diagnostic methods (EEG, neuro-ophthalmological exam…

Ataxia in epidural spinal cord compression.

Nine patients presented with ataxia as the primary manifestation of epidural spinal cord compression. Eight had known cancer, the ninth an epidural abscess. Lower-extremity dysmetria, gait ataxia, or both, were the only neurologic signs in five patients. An incorrect initial diagnosis led to delay in treatment and subsequent neurologic deterioration in six patients. Failure to recognize isolated, painless ataxia as the initial manifestation of spinal cord compression and appropriately treat the disorder can result in irreversible spinal cord deterioration.

[Vestibular ataxia caused by meningeal carcinomatosis].

A 74-year-old man developed progressive deafness and unsteady gait two years after gastrectomy for a gastric cancer. Neurological examination revealed an alert and intelligent Japanese male in no acute distress. The optic fundi were normal. The pupils and the extraocular muscles were normal, however, horizontal nystagmus was noted in right and left gaze. He showed marked bilateral deafness, and loss of caloric response bilaterally. No muscle atrophy nor weakness was noted. His gait was wide-based and ataxic. Tandem gait was impossible. Romberg sign was present. No cerebellar ataxia was noted i…

[Cardiac involvement in Friedreich's heredo-ataxia].

The frequency and characteristics of cardiac involvement have been evaluated in 22 patients with Friedreich's ataxia and in 10 patients with non Friedreich's ataxia (Strumpell-Lorraine 5 cases; Pierre Marie 5 cases), classified according to the severity and the lasting of neurological disease. In a high percentage (45%) of patients with Friedreich's ataxia, the results show left ventricular hypertrophy as proved echocardiographically by an increase of the interventricular septum thickness and of the posterior wall thickness. On the contrary, no patient with non Friedreich's ataxia had left ven…

Retinitis pigmentosa, ataxia, and peripheral neuropathy.

The clinical features of four patients with retinitis pigmentosa, ataxia and peripheral neuropathy but with no increase in serum phytanic acid are reported. Three patients also had sensorineural deafness and radiological evidence of cerebellar atrophy. Nerve conduction studies revealed abnormalities of sensory conduction and normal or only mild slowing of motor conduction velocity. Sural nerve biopsy demonstrated a reduction in the density of myelinated fibres. There were no onion bulb formations. These cases clinically resemble Refsum's disease, but differ in having no detectable biochemical …

Acute cerebellar ataxia and infectious mononucleosis.

A 28-year-old man, who presented with acute cerebellar ataxia, was found to have haematological features of infectious mononucleosis. There was serological evidence of recent infection with Epstein-Barr virus. It is speculated that cerebellar dysfunction results from virus-induced inflammatory changes within the central nervous system.

Visually evoked potentials in Friedreich's ataxia.

Visual evoked potentials to pattern-reversal stimulation have been recorded in 45 patients with Friedreich's ataxia. In six patients, no response could be obtained because of optic atrophy; altogether, pathological results were found in 25 patients (55%). Normal latencies were recorded in 20 patients (45%). No statistically significant connection could be drawn between the result of the VEP testing and visual acuity, age, or onset and duration of sickness, but there was a strikingly familiar accumulation of results. There is a good conformity between our results and the incidence of optic atro…

Heredo-ataxia and cochleo-vestibular disorders.

Cochleo-vestibular dysfunction of patients suffering from a heredo-ataxia, have been frequently described. A family and a single case are presented, which show the association of a heredo-ataxia, more specifically the Roussy-Lévy syndrome, progressive sensorineural loss, nystagmus and vestibular a- or hyporeflexia. A review of the literature is included and the standpoints concerning the nosological entities are discussed.

[Bite anomalies in familial Friedreich's ataxia].

Besides the spinal bone and foot deformations which are well known in Friedreich's disease, attention is drawn to deformations of the maxillodental system, the state of which was previously not studied. There was a pathology of bite in 16 of the 24 patients with Friedreich's familial ataxia. As a rule, these anomalies were combined with other bone deformations. In 2 families where several patients were examined there was a similar pathology of bite. A study of 24 patients and an examinations of a control group consisting of 151 individuals allows a conclusion that in Friedreich's disease patho…

Ataxia-telangiectasia. (Clinical and immunological aspects).

This syndrome was defined by the authors in 1947. Earlier publications of similar disease descriptions were by Syllaba and Henner (1926), Louis-Bar (1941). The authors at present have a stock of 253 cases. The cardinal symptoms of this phakomatosis are: Cerebellar ataxia which begin in infancy and take a slowly progressive course. In the late stages free walking and standing are no longer possible. Progressive atactic speech disorders, cerebellar atrophy in the pneumoencephalogram. Slowly progressing symmetrical skin and mucosal telangiectasia in the face and especially on the conjunctivae at …

Differential effects of PSEN1 mutations (p.P117L with ataxia and p.P264L without ataxia) in early-onset Alzheimer's disease.

Presenilin 1 (PSEN1) plays a pivotal role in early-onset Alzheimer's disease (EOAD). The clinical phenotype of EOAD is typically marked by cognitive decline, with ataxia rarely reported. We identified mutations at different positions of PSEN1 in two Chinese patients with EOAD. Interestingly, one patient carrying the PSEN1 p.P117L mutation manifested symptoms of ataxia, while another patient harboring the PSEN1 p.P264L mutation did not exhibit any signs of this disorder. Computational analyses using PolyPhen-2, SIFT, Provean, and Mutation Taster predicted both mutations as pathogenic, while str…
