INTRODUCTION: Approximately one-third of all ischemic strokes and the ensuing health and economic burden can be attributed to the presence of atrial fibrillation (AF). The global prevalence of AF continues to rise, thus making it by far the most common diagnosed cardiac arrhythmia. Percutaneous left atrial appendage (LAA) occlusion or obliteration has been developed to protect from the occurrence of stroke in patients with nonvalvular AF.
AREAS COVERED: We address the characteristics and techniques for implantation as well as some clinical registries and randomized trials of the various cathe…

Atrial fibrillation (AF) is the most common rhythm disturbance. Among the major thromboembolic complications associated with AF, strokes are … an alternative means of thromboembolic prevention is desirable. Percutaneous occlusion of the left atrial appendage (LAA) is an alternative, interventional, non-pharmacological treatment that has been used widely in Europe and for a few years in France, with encouraging results. However, it remains an invasive procedure with a low level of proof in comparison wit …

Extracorporeal membrane oxygenation (ECMO) has been utilized in the pediatric population for cardiogenic shock secondary to medically intractable arrhythmias. There is limited experience with cardiac radiofrequency ablation (RFA) on these patients while on ECMO. A 7-year-old girl presented with a tachycardia-mediated cardiomyopathy secondary to a left atrial appendage tachycardia. She suffered a cardiac arrest due to pulseless electrical activity and was placed on ECMO. Due to elevated left atrial pressures and the refractoriness of her arrhythmia to cardioversion and antiarrhythmic therapy, w…

BACKGROUND: Atrial fibrillation is associated with an increased risk of embolic events. The left atrial appendage (LAA) is believed to be an incubator for thrombus formation. LAA exclusion has been advocated to potentially reduce embolic events arising from the LAA.
OBJECTIVE: The aim of the study was to determine the feasibility of a closed-chest surgical suture ligation of the LAA in man.
METHODS: Thirteen patients undergoing either mitral valve surgery (n = 2) or electrophysiological study and radiofrequency catheter ablation for atrial fibrillation (n = 11) underwent ligation of the LAA …

A 9-month-old boy with left juxtaposition of the atrial appendages, tricuspid atresia, pulmonary atresia, and ventriculoarterial discordance underwent anastomosis between the atrial appendages after failure of balloon/blade atrial septostomy because of restrictive atrial septal defect. For surgical creation of atrial communication in patients with juxtaposed atrial appendages, anastomosis between the atrial appendages seemed to be safer, more effective, and less invasive than septectomy by Blalock-Hanlon technique or inflow occlusion technique.

Giant congenital intrapericardial aneurysms of the left atrial appendage and body are unusual cardiac anomalies. Only 48 cases have been previously reported in the literature, with the vast majority presenting in otherwise healthy young patients. These can present clinically with persistent arrhythmias and progress to stroke if not diagnosed and treated. Our index case is that of a 34-year-old woman discovered to have a giant left atrial aneurysm who developed intermittent palpitations with sinus tachycardia. We present a review of the literature pertaining to their diagnosis, potential morbid…

We report two autopsied cases of an unusual ventricular loop in hearts with right-sided juxtaposition of the atrial appendages. Case 1 showed usual atrial arrangement, a concordant atrioventricular connexion with a disharmonious ventricular loop showing left-hand topology and double outlet right ventricle with normally related arterial trunks. The atrioventricular connexions were crossing, with the inlet to the left-sided morphologically right ventricle being posterior to that of left ventricle. Mitral hypoplasia and coarctation of the aorta were the associated lesions. The second case showed …

A case is reported of herniation of the left atrial appendage through a partial pericardial defect, probably congenital. The diagnosis was suggested by the history of chest pain and bulging of the middle segment of the left heart border on the plain chest film, without other signs. Angiography revealed a dilated left atrial appendage. An artificial left pneumothorax confirmed the presence of a pleuropericardial defect. The surgical procedure included excision of the appendage and closure of the defect.

BACKGROUND: Right atrial appendage atrial tachycardia (AT) during pregnancy is rare and can progress to tachycardia-induced cardiomyopathy and shock.
CASE SUMMARY: A 22-year-old woman at 36 weeks and 5 days of gestation presented with fatigue, dyspnea, and narrow-complex tachycardia at 190-210 beats/min. Echocardiography showed an ejection fraction of 18%. After failed medical therapy, cardioversion, and esophageal pacing, she underwent emergency cesarean delivery and venoarterial extracorporeal membrane oxygenation. Mapping localized the AT to the base of the right atrial appendage; endocard…

IMAGE(S): Multimodality imaging of a tubular left atrial appendage outpouching mimicking coronary artery aneurysm on pediatric echocardiography.
CASE SUMMARY: A 3-year-old child with a recent history of febrile illness of unclear etiology underwent transthoracic echocardiography during evaluation of a heart murmur. A tubular outpouching adjacent to the proximal left coronary artery raised concern for possible coronary artery involvement, including coronary artery aneurysm. Doppler findings were nondiagnostic. Cardiac computed tomography demonstrated a narrow connection between the structure a…

Atrial fibrillation (AF) confers a substantial risk of stroke. Direct oral anticoagulants (DOACs) are highly effective for stroke prevention and form the cornerstone of AF management. Nevertheless, many eligible patients do not receive or do not maintain long-term anticoagulation because of bleeding risk, prior hemorrhage, or other contraindications. Percutaneous left atrial appendage closure (LAAC) has emerged as a non-pharmacological alternative. In 2026, two major randomized trials addressing the role of LAAC as an alternative to anticoagulation were published: CHAMPION-AF and CLOSURE-AF. …

Atrial fibrillation (AF) is a major cause of cardioembolic stroke, and oral anticoagulation (OAC) remains the standard of care for thromboembolic prophylaxis. Because the left atrial appendage (LAA) is the dominant source of thrombus in AF, percutaneous LAA occlusion (LAAO) has been developed as a mechanical alternative for patients in whom long-term anticoagulation is problematic. Randomized trials against warfarin established non-inferiority for the composite endpoint, though not consistently across coprimary endpoints, and more recent trials against direct oral anticoagulants (DOACs) have e…

OBJECTIVE: To describe an endocardial obliterative left atrial appendage closure (LAAC) technique using inverted appendage purse-string plication added to 2-layer orifice closure and to evaluate anatomic and clinical outcomes in minimally invasive and robotic mitral surgery.
METHODS: This retrospective study included consecutive patients who underwent robotic mitral valve surgery, or atrial septal defect repair with tricuspid intervention, with concomitant endocardial obliterative LAAC between April 2024 and June 2025. The technique inverts the distal half of the left atrial appendage (LAA), …

Surgical left atrial appendage occlusion (S-LAAO) is increasingly performed during cardiac surgery, but perioperative outcomes remain uncertain. Using a nationwide Japanese inpatient database, we analyzed 25,059 adults undergoing valve or coronary bypass surgery (2020 - 2022). After propensity score matching (n = 2,543 each), addition of S-LAAO was not associated with differences in in-hospital mortality, transfusion, reoperation, or 30-day readmission compared to non-S-LAAO group. However, prolonged inotropic support (≥ 2 days) was more frequent with S-LAAO (40.8% vs. 34.7%; odds ratio 1.29, …
