Craniovertebral junction (CVJ) disorders in the Occident are usually associated with systemic disorders. In contrast, in the Orient, a greater incidence of isolated CVJ anomalies is seen. Although these are developmental anomalies, they manifest late in life, with trauma and/or infection playing a promotive role. The most significant and common of these anomalies are basilar invagination and atlantodental dislocation, which also occur together. Accurate diagnosis of these anomalies is feasible using computed tomography and magnetic resonance imaging. Greater awareness of this subset of patient

We reported a 12-year-old boy with neck pain and dysphagia 6 days after his neck being tied by another boy's arms from the back. The boy was found to have a tear-drop fracture of the axis in both radiograph and MRI, which was a rare case in children. He was placed in a cervical traction with Glisson belt and received transfusion for 2 weeks until his neck pain and dysphagia were released. Thereafter, an operation of anterior fusion and fixation was chosen to ensure further stability of the cervical spine and replacement of the fracture fragment. An 8-year follow-up examination showed a satisfa

We describe a rare case of marked segmental stenosis of the axis secondary to developmental hypertrophy of the posterior neural arch causing severe neck pain and headache in the occipital region. The patient made a remarkable recovery following decompressive laminectomy and foraminal decompression.

We report on three cases of a rare fracture of the posterior body of C2. This is a variant of the classic traumatic spondylolisthesis and appears to respond to conservative management.

BACKGROUND: Aneurysmal bone cysts (ABCs) comprise 1%-2% of all bone tumors. ABCs involving the upper cervical spine in pediatric patients complicate intervention because of an immature skeleton and nearby neurovascular elements. Treatment often consists of surgical intervention with preoperative embolization to mitigate blood loss. During selective arterial embolization, it is important to be aware of extracranial-intracranial anastomoses from tumor feeding vessels. We describe a C2 vertebrae ABC that had multiple tumor arteries anastomosing with bilateral vertebral arteries.

INTRODUCTION: Os odontoideum (OO) is a rare anatomic variant of the axis characterized by the separation of a part of the axis. It may cause cervical instability, atlantoaxial dislocation, myelopathy, or permanent paralysis. We present an extremely rare case of an OO with posterior atlantoaxial dislocation in a child.
CASE REPORT: A nine-year-old female with OO, cervical instability, and posterior atlantoaxial dislocation had intermittent neck pain for several years. The patient was surgically treated by performing a C2-C3 laminectomy with C1-C5 posterior fixation for stabilization.

The acute-stage intraoperative findings of the spinal dural laceration in patients with traumatic cerebrospinal fluid (CSF) leakage have not been clearly revealed so far. Herein, we report an interesting case of traumatic CSF leakage in a patient who underwent acute-stage direct closure for a dural laceration similar to a stab at the C1/2 level. Because a 41-year-old man strongly twisted his neck, he had a typical orthostatic headache. Whole-spine T2 fat-saturated magnetic resonance imaging and computed tomographic myelography were examined, and fluid collection between the intrathecal site an

BACKGROUND: A retro-odontoid pseudotumour compressing the spinal cord and causing myelopathy is often associated with an inflammatory condition such as rheumatoid arthritis. A degenerative non-inflammatory retro-odontoid pseudotumour responsible for clinically relevant spinal cord compression is a rare condition described in small clinical series and is likely associated with craniovertebral junction hypermobility or instability-like conditions. For several years, direct removal of the lesion through an anterior or lateral approach has been advocated as the best surgical option. However, in th

Osteoid osteoma affects the spine in only 10% of cases. More than 50% of the spinal cases involve the lumbar and cervical vertebrae. Involvement of C-1 and C-2 vertebrae has previously been reported only very rarely in the published literature. The authors report 4 cases of upper cervical osteoid osteoma, 1 involving C-1 and the other 3 C-2, and they discuss different aspects of management in similar cases. The patients were 14, 17, 35, and 46 years old, and all presented with neck pain and various degrees of painful limitation of head rotation not ameliorated by ordinary analgesics. Radionucl

STUDY DESIGN: Anatomic bony measurements were manually performed on 50 dry atlantoaxial vertebral complexes with an electronic digital caliper, and a reliable landmark for insertion of a pedicle screw in C1 vertebra was described and evaluated.
OBJECTIVES: To investigate the feasibility of placing a screw in the C1 pedicle and evaluate the reliability of the C2 lateral mass as a landmark for determining the optimal site of screw entry.
SUMMARY OF BACKGROUND DATA: The use of C1-C2 pedicle screws with the additional rods or plates has been an alternative method of stabilization for patients wh

A 43-year-old man developed cerebellar infarction due to atlantoaxial dislocation associated with os odontoideum. Cervical X-ray films confirmed os odontoideum, and conventional angiography revealed irregular narrowing of the right vertebral artery at the axis level. A correlation between the vertebral artery lesion and atlantoaxial instability was shown with 3-dimensional (3-D) computed tomographic (CT) angiography. This is the first case of atlantoaxial dislocation with vertebral artery stenosis demonstrated 3-D CT angiography. 3-D CT angiography can support the conventional angiography with

OBJECTIVE: To describe the surgical treatment outcome of cervical fractures in 3 horses.
STUDY DESIGN: Case report.
ANIMALS: Three client-owned horses with cervical vertebral fractures.
METHODS: Three horses were refered for neck stiffness, pain, and ataxia after a cervical trauma because of a fall. Radiographic examination showed an oblique displaced fracture of the caudal aspect of the body of the second cervical vertebra (C2) in horse 1, an oblique displaced fracture of the caudal aspect of C4 involving the disc between C4 and C5 in horse 2, and a displaced transverse fracture of the bod

PURPOSE: An understanding of the normal evolution of the spine is of great relevance in the prenatal detection of spinal abnormalities. This study was carried out to estimate the length, width, cross-sectional area and volume of the neural ossification centers of vertebrae C1-S5 in the human fetus.
MATERIALS AND METHODS: Using the methods of CT (Biograph mCT), digital-image analysis (Osirix 3.9) and statistics (the one-way ANOVA test for paired data, the Kolmogorov-Smirnov test, Levene's test, Student's t test, the one-way ANOVA test for unpaired data with post hoc RIR Tukey comparisons) the

A 58-year-old man presented with a 6-month history of progressive cervical myelopathy. Magnetic resonance imaging and computed tomography of the cervical spine revealed a bone tumor arising from the posterior arch of the atlas and osteophytes at a pseudoarthrosis between the tumor and the lamina of the axis, resulting in marked spinal cord compression. The patient's symptoms resolved after en bloc resection of the tumor and removal of the osteophytes. The histological diagnosis was osteochondroma. The primary cause of myelopathy in the present case was osteochondroma arising from the posterior

The craniocervical junction is a highly specialized unit simultaneously supporting head during movements in all planes and protecting the spinal cord. Anatomically, it includes an atlantoaxial complex, part of which embryonically arises from the occipital region of the skull. This review deals with the gross anatomy, kinematics, and growth reactions associated with functional alteration in this complex. Particular attention is paid to the atlas, the connecting element between the head and the vertebral column proper. From several studies it is concluded that the horizontal growth of the atlas
