Association of left dominant coronary arterial system with congenital bicuspid aortic valve.

Clinical angiographic studies have documented an association of left dominance of the coronary arteries with aortic stenosis and congenital bicuspid aortic valve. The postmortem arteriograms of 973 autopsy patients were reviewed for pattern of coronary dominance and the hearts examined for the nature of any aortic valve disease. There were 673 hearts (70 percent) with a right dominant pattern, 198 (20 percent) with equal dominance and 102 (10 percent) with left dominance. Of 34 hearts with congenital bicuspid aortic valve, 10 (29 percent) had left dominance, a difference significant at the 0.0…

Spontaneous rupture of bicuspid aortic valve. An unusual cause of aortic insufficiency.

This report documents the clinicopathologic correlation between pure aortic regurgitation and an exceptional form of congenitally bicuspid aortic valve. The patient was known for many years to have mild aortic insufficiency. His condition suddenly deteriorated, with signs of an aggravated aortic regurgitation. Infectious endocarditis was considered, but the diagnosis was never established. Surgery revealed an exceptional form of a bicuspid aortic valve in which the conjoined cusp had prolapsed, due to rupture of a fibrous strand which previously had anchored the free rim of the cusp to the inn…

Enterococcal Endocarditis in a Bicuspid Aortic Valve Associated With a Sinus of Valsalva Aneurysm.

This report describes a complex case of Enterococcus faecalis infective endocarditis (IE) in a 68-year-old man with an undiagnosed bicuspid aortic valve (BAV) and a concomitant sinus of Valsalva aneurysm (SoVA). A significant diagnostic challenge occurred as the patient's initial febrile illness was misattributed to Lyme disease; two subsequent 30-day courses of doxycycline partially treated the infection, masking the clinical severity and delaying diagnosis. The patient eventually presented with new-onset atrial fibrillation, heart failure, and a new bifascicular block. Transesophageal echoca…

The NavIIgister: Transcatheter Aortic Valve Implantation With the Navitor Platform in Bicuspid Anatomy.

BACKGROUND: Transcatheter aortic valve implantation (TAVI) for the treatment of bicuspid aortic valve (BAV) is still debated. Although several registries reported favorable outcomes with various transcatheter heart valves (THV) in BAV, there is no published data on the Navitor platform. AIMS: To provide preliminary data on the efficacy and the safety of the Navitor valve in BAV. METHODS: We retrospectively included patients who underwent TAVI using the Navitor valve (Abbott, Minneapolis, Minnesota, USA) for severe aortic stenosis in BAV anatomies, from October 2021 until August 2024. Patient…

Repair of a stenotic bicuspid aortic valve by extensive leaflet shaving and commissuroplasty.

The surgical repair of the bicuspid aortic valve in children has been shown to have good outcomes. The repair can be done by the delicate use of native valve tissue without using patch material. The patient is an 8-year-old boy diagnosed with a stenotic bicuspid aortic valve and supravalvular aortic stenosis. He had a balloon valvuloplasty when he was 6 years old. This video tutorial gives detailed information on how to repair the aortic valve by reshaping the aortic cusps and reconstructing the aortic commissures.

Different calcification patterns of tricuspid and bicuspid aortic valves and their clinical impact.

OBJECTIVES: Mechanical strain plays a major role in the development of aortic calcification. We hypothesized that (i) valvular calcifications are most pronounced at the localizations subjected to the highest mechanical strain and (ii) calcification patterns are different in patients with bicuspid and tricuspid aortic valves. METHODS: Multislice computed tomography scans of 101 patients with severe aortic stenosis were analysed using a 3-dimensional post-processing software to quantify calcification of tricuspid aortic valves (n = 51) and bicuspid aortic valves (n = 50) after matching. RESULT…

A heart in a heart: a case report of spontaneous flail of bicuspid aortic valve.

BACKGROUND: Aortic regurgitation is attributed to congenital and acquired abnormalities of the aortic valve or aortic valve supporting structures. The most common cause of aortic regurgitation is atherosclerotic degeneration of the valve, especially in the presence of a bicuspid aortic valve. A 25-year-old Persian man with no past medical history, developed dyspnea since 1 week before his first visit to the physician. He was an active person up to this time, and had no history of trauma or chest pain. Transthoracic echocardiography showed severe aortic regurgitation in the context of flail bic…

Acquired Gerbode Defect in a Patient With Infective Endocarditis of Bicuspid Aortic Valve.

Aortic valve abscess is a fatal complication of infective endocarditis. Transthoracic echocardiography is the initial imaging obtained in suspected infective endocarditis. However, its accuracy in detecting cardiac complications remains low, thus should be followed by transesophageal echocardiography if the clinical situation permits. Here, we present a case of a bicuspid aortic valve infective endocarditis caused by Streptococcus agalactiae and complicated with aortic valve abscess and acquired Gerbode defect, which appeared as a tricuspid valve vegetation on transthoracic echocardiography.

Differences in Aortopathy in Patients with a Bicuspid Aortic Valve with or without Aortic Coarctation.

OBJECTIVE: The combination of aortic coarctation (CoA) and bicuspid aortic valve (BAV) is assumed to be associated with a higher risk of ascending aortic dilatation and type A dissection, and current European Society of Cardiology (ESC) guidelines advise therefore to operate at a lower threshold in the presence of CoA. The aim of our study is to evaluate whether the coexistence of CoA in BAV patients is indeed associated with a higher risk of ascending aortic events (AAE). METHODS: In a retrospective study, all adult BAV patients visiting the outpatient clinic of our tertiary care center betw…

Is there a role for autophagy in ascending aortopathy associated with tricuspid or bicuspid aortic valve?

Autophagy is a conserved process by which cytoplasmatic elements are sequestered in vesicles and degraded after their fusion with lysosomes, … t controversial and epidemiologically important forms of TAA is that associated with the congenital bicuspid malformation of the aortic valve (BAV). Dysregulated autophagy in response, for example, to wall shear stress alterations, has been demonstrated to affect the phenotype of vascular cells relevant to aortopathy, with potential consequences on signali …

Aortic elasticity and the influence of valve morphology in children with bicuspid aortic valve.

UNLABELLED: AimWe investigated dimensions and elasticity of whole aorta in patients with bicuspid aortic valve and influence of valve phenotype. METHOD: The study group included 44 patients and 42 controls. Patients were divided into groups according to the type of valve opening as horizontal - fusion between right and left coronary cusps - and vertical - fusion between right-non-coronary cusps; according to age they were divided into younger (5-10 years) and older patients (11-16 years). Our study did not include valve phenotype with fusion between left and non-coronary cusps. Systolic-diast…

Prevalence and Complications of Bicuspid Aortic Valve in Chinese According to Echocardiographic Database.

As transcatheter aortic valve replacement has become an alternative treatment for patients at high risk for surgical aortic valve replacement, bicuspid aortic valve (BAV) draws our attention again. The reported frequency of BAV was 0.5% to 2% in western population. However, there was no such epidemiologic study in Chinese population. Our study sought to investigate the prevalence and complications of BAV in China by echocardiographic database. A total of 668 cases who were confirmed with BAV, identified from 195,708 echocardiographic records of 157,039 patients in the echocardiographic databas…

Elevated oxidative stress in the aortic media of patients with bicuspid aortic valve.

OBJECTIVE: Congenital bicuspid aortic valve (BAV) is distinctly associated with the development of ascending aortopathy in adulthood, portending risk of both ascending aortic aneurysm and dissection. Our previous work implicated deficiency in oxidative stress response as a mediator of the BAV-associated aortopathy. We hypothesize that reactive oxygen species generation invokes elevated local oxidative tissue damage in ascending aorta of patients with BAV. METHODS: Ascending aortic specimens were obtained from patients undergoing elective aortic replacement and/or aortic valve replacement and …

Abnormal Longitudinal Growth of the Aorta in Children with Familial Bicuspid Aortic Valve.

Bicuspid aortic valve (BAV) is the most common type of congenital heart defect (CHD) and is associated with clinically significant cardiovascular complications including valve calcification and ascending aortopathy (AscAo), predominantly occurring in adulthood. While a limited number of genetic etiologies for BAV have been defined, family members of affected individuals display BAV along with other left-sided CHD. This has led to guidelines from the American Heart Association and American College of Cardiology that recommend echocardiographic screening of first-degree relatives of affected adu…

Aortic shear stress in patients with bicuspid aortic valve with stenosis and insufficiency.

OBJECTIVES: Bicuspid aortic valve, characterized by valve malformation and risk for aortopathy, displays profound alteration in systolic aortic outflow and wall shear stress distribution. The present study performed 4-dimensional flow magnetic resonance imaging in patients with bicuspid aortic valve with right-left cusp fusion, focusing on the impact of valve function on hemodynamic status within the ascending aorta. METHODS: Four-dimensional flow magnetic resonance imaging was performed in 50 subjects with right-left bicuspid aortic valve and 15 age- and aortic size-matched controls with tri…
