The diagnosis of arteriohepatic dysplasia may be difficult, particularly in very young patients with no family history, and the pathogenesis of the disorder remains obscure. Fifteen children under the age of 15 were identified to have congenital dilatation of the biliary tree. Thirteen of them (87%) had dilatation of both intrahepatic and extrahepatic bile ducts, while two had extrahepatic biliary dilatation only. Fifty-three patients were evaluated for carcinoma of the extrahepatic bile ducts at the University of Virginia from 1951 to April 1984. This population was retrospectively reviewed and 33 preoperative and postoperative variables were analyzed to evaluate the predictors of increased survival. The authors describe the scintigraphic appearance of a case of subcapsular biliary leak. Hepatobiliary scintigraphy using Tc-99m labeled radiopharmaceuticals is employed primarily for the diagnosis of acute cholecystitis and for the demonstration of biliary tract patentcy. This procedure can also provide functional and morphologic information for evaluation of biliary tract trauma.

Based on experience with six patients with high bile duct carcinoma, a rational therapeutic approach is proposed. When extent of tumor precludes resection, the authors recommend palliation with a biliary-enteric anastomsis to avoid the complications and inconveniences of long-term transhepatic stenting. This is best accomplished with construction of a left cholangiojejunostomy using the round ligament approach. Temporary bilobar decompression may be necessary in select cases.

Cholangiograms and clinical histories of 82 patients with biopsy-proved bile duct carcinoma were reviewed. The carcinomas were classified according to morphologic type and anatomic location, and these were correlated with surgical and pathologic findings and clinical outcome. Ulcerative colitis and antecedent inflammatory disease of the biliary tree, particularly primary sclerosing cholangitis, seem to predispose to the development of bile duct carcinoma. The authors describe results of using "one-agraffe" suturing devices and an original "many-agraffes" apparatus for suturing a wound of the common bile duct, papillosphincteroplasty and creation of bile-eliminating anastomoses in experiments in 47 dogs and clinical application in 104 patients. It was shown that these devices can considerably facilitate and accelerate the performing of operations, increase the hermetic properties of the suture and anastomoses, thus giving better remote results as compared with manual sutures with synthetic and catgut threads.

At operation for haematemesis in bilharzial hepatic fibrosis, the gallbladder was always found to be diseased but devoid of stones. This led to a study of gallbladder bile composition in a group of bilharzial patients and comparison with a group of cholelithiasis patients. When values for biliary lipid composition were plotted on triangular coordinates, both groups fell well outside the line of maximum cholesterol solubility.
