[Biliary colonic ileus: an unusual cause of colonic obstruction].

We report a rare case of colonic gallstone obstruction in a patient with cholecystocolic fistula. Diagnosis of this condition is usually difficult and only achieved at surgery. We review the radiological findings, particularly the CT findings, helpful for diagnosis: ectopic gallstone, biliary gas and fistula. Early preoperative diagnosis could reduce morbidity and mortality. Treatment is surgical with enterolithotomy. There is some controversy over the need to repair the fistula.

Percutaneous transhepatic endobiliary drainage of hepatic hydatid cyst with rupture into the biliary system: an unusual route for drainage.

The most common and serious complication of hydatid cyst of the liver is rupture into the biliary tract causing obstructive jaundice, cholangitis and abscess. The traditional treatment of biliary-cystic fistula is surgery and recently endoscopic sphincterotomy. We report a case of complex heterogeneous cyst rupture into the biliary tract causing biliary obstruction in which the obstruction and cyst were treated successfully by percutaneous transhepatic endobiliary drainage. Our case is the second report of percutaneous transbiliary internal drainage of hydatid cyst with rupture into the biliar

Xanthogranulomatous cholecystitis complicated by a cholecysto-colonic fistula and liver abscesses.

A 57-year-old male presented to the emergency department with right upper quadrant pain and constitutional symptoms. Initial investigation revealed biliary sepsis with features of chronic cholecystitis, multiple liver abscesses and a fistulous connection between the gallbladder and colon. He was subsequently diagnosed with a cholecysto-colonic fistula, an unusual complication of biliary pathology, with an incidence of 0.06-0.14% at cholecystectomy. It is the second most common form of cholecystoenteric fistula, the first of which is cholecystoduodenal. A preoperative diagnosis was suggested us

Stone-Induced Purulent Choledocoduodenal Fistula Presenting with Ascending Cholangitis.

A biliary enteric fistula (BEF) is a rare, abnormal communication between any segment of the biliary tree with any portion of the small or large intestine. BEF is more frequently diagnosed with the increasingly widespread use of endoscopic retrograde cholangiopancreatography. Different theories have been postulated regarding the etiology of this fistula formation, with the most likely cause being gallstones. Treatment modalities, ranging from conservative management to surgical reconstruction, show varying levels of success. We present a case of BEF secondary to large common bile duct stones,

Haemobilia due to iatrogenic portobiliary fistula after cholecystectomy.

Haemobilia, defined as bleeding into the biliary tree is a rare condition. We describe a case report of a patient who presented it as a complication of iatrogenic portobiliary fistula, followed after an open cholecystectomy. The patient presented to the emergency department with late onset symptoms of haematemesis and melena a month after surgery. Findings were confirmed by Doppler ultrasound that showed the appearance of intragallbladder mass with high echogenicity representing a blood clot. Also, next to the portal vein and the biliary duct a lesion with mixed blood flow was detected confirm…

A Unique Management Strategy for Migrated Biliary Stent Causing Duodenal Perforation.

Distal stent migration leading to duodenal perforation is an uncommon complication of endoscopic biliary plastic stent placement. We present a case in which a patient with a migrated biliary plastic stent that perforated through the duodenum was managed expectantly until a duodenocolic fistula formed prior to endoscopic removal.

Biliary complications of laparoscopic cholecystectomy.

Laparoscopic cholecystectomy has rapidly become the preferred treatment for symptomatic gallstones. Although this procedure has certain adva … ngiogram leading to an unnecessary biliary-enteric bypass and subsequent dehiscence, resulting in a biliary fistula. Six patients were referred on an acute basis, whereas four patients underwent attempted biliary reconstruction at outside facilities and were ultimately referred with either a biliary stricture or a fistula. Review of cholangiograms suggested that …

Bronchobiliary fistula due to acute cholecystitis in a suprahepatic gall bladder.

We describe a patient presenting with painless jaundice, anorexia and pruritus. The gall bladder was found to be lying above and behind a hypoplastic right lobe of liver. There was no evidence of cholangitis or biliary obstruction. The patient subsequently developed a bronchobiliary fistula with severe wheeze, cough and bile-stained sputum. Emergency percutaneous drainage of the gall bladder led to immediate cessation of bronchospasm and biloptysis, rendering the patient fit for definitive surgery.

Gallstone Ileus Secondary to Cholecysto-Duodenal Fistula in a Middle-Aged Female: A Rare Surgical Emergency and Review of the Literature.

Gallstone ileus secondary to cholecysto-duodenal fistula is a serious and uncommon complication due to cholelithiasis, characterized by mechanical bowel obstruction due to gallstone impaction following a spontaneous biliary-enteric fistula. The case highlights the diagnostic challenge and the importance of clinical evaluation and thorough investigation of the patients.

Bouveret's syndrome treated with endoscopic electrohydraulic lithotripsy.

Bouveret's syndrome is a rare form of small bowel obstruction caused by a large biliary stone entering the lumen of the duodenum or the stomach through a bilioenteric fistula. Treatment options include various surgical techniques. However, recent advances in endoscopy also allow non-surgical endoscopic treatment options. We report a 68-year-old man, with a disseminated small intestinal neuroendocrine tumour, presenting with Bouveret's syndrome without any previously reported biliary disease. He experienced a number of symptoms 1 month prior to his admittance, which were difficult to differenti

Biliary Obstruction Secondary to Transjugular Intrahepatic Portosystemic Shunt Presenting as Alcohol-Associated Hepatitis.

Transjugular portosystemic intrahepatic shunt (TIPS) is a procedure used to alleviate portal hypertension. The two most common clinical indications for TIPS are secondary prevention of variceal hemorrhage and refractory ascites. Biliary complications, of which biliary-shunt fistulas are the most common, are very rare. There are very limited data on TIPS placement leading to biliary obstruction. We describe a patient who underwent TIPS placement leading to biliary obstruction, initially thought to have alcohol-associated hepatitis, who was successfully treated with interventional radiology-guid

Surgical management of hydatid disease of the liver.

The authors review 143 consecutive, surgically treated cases of hydatid disease of the liver seen over the past 10 years. Of the 208 cysts found, 82 (39.4%) were complicated; the remainder were simple. The complications included rupture into the biliary system (16.3%), suppuration (11.0%), partial calcification of the pericyst (5.8%), intraperitoneal rupture (4.8%), bronchobiliary fistula (0.9%) and cystocutaneous fistula (0.5%). External capitonnage was the most commonly used surgical technique (63.0%), followed by cystectomy (17.8%), omentoplasty (10.6%), tube drainage (7.7%) and cystojejuno…
