OBJECTIVE: To determine the method for locating the hemorrhagic site of biliary tract and assess the value of hepatectomy.
METHODS: Clinical data of 23 patients with biliary tract hemorrhage undergoing liver segmentectomy between January 1998 and October 2008 were analyzed. With the help of my colleagues, I have been conducting epidemiological studies on biliary tract cancer (BTC), including gallbladder cancer (GBC) and extrahepatic bile duct cancer (BDC), in Japan and Chile for about 19 years. Clustered areas with high mortality rates, especially for female GBC were found to correspond with places or prefectures in Japan that were famous for rice production. To clarify the optimal lymph node dissection for carcinoma of the biliary tract, we analyzed the mode of lymphatic spread in 86 resected cases with carcinoma of the gallbladder and 139 with carcinoma of the extrahepatic bile duct, and investigated long-term results after resection based on the degree of lymph node metastasis. Seven patients with advanced carcinoma of the extrahepatic biliary tract, including two with cancer of the gallbladder, underwent major hepatectomy with concomitant pancreatoduodenectomy. Spontaneous perforation of the biliary passages is a rare condition during the first months of life. The course of the condition may be acute or more chronic. The symptoms are jaundice, clay-coloured stools, dark urine, distended abdomen, regurgitation and failure to thrive. Five patients with fistulas of the biliary tract are reported; three had fistulas caused by operative injury and two, by accidental blunt abdominal trauma. All patients were treated successfully by endoscopic transpapillary insertion of biliary endoprostheses. The endoprostheses were removed after four to 17 weeks in all patients, and no evidence of biliary leakage was revealed by repeat cholangiograms and other clinical follow-up study. This effective, simple and safe method allows the fistula to heal and is a good alternative to surgical treatment.

A series of 53 patients who sustained extrahepatic biliary tract trauma were analysed to define the role of the various surgical options. Of the 45 patients with gallbladder injuries, 39 were due to stab wounds. Nine of the 45 injuries were repaired by primary suture without complication. This retrospective study was undertaken to evaluate the incidence of an abnormal pancreaticobiliary ductal union (long common duct) in biliary tract carcinoma. Estimation of alkaline phosphatase activity is a sensitive and specific test for biliary tract inflammation. The enzyme penetrates into bile from biliary tract epithelium. Activation of the enzyme was noted in cholelithiasis, while the maximal level of alkaline phosphatase activity was detected in patients with chronic cholecystitis. Inflammation of gall bladder may result in deterioration of its epithelium functions, which appears to be responsible for conversion of stable bile colloidal phase into precipitates.
