Parents' Perspectives on the Role and Value of School Nurses.

This commentary highlights three themes from the stories contained within this issue of Narrative Inquiry in Bioethics. First, we discuss differing perceptions of school nurses' roles and responsibilities. Administrators, parents, and nurses often have different understandings of nurses' roles. We explain why these differences may lead to confusion and conflict. Second, we describe the respective pressures that parents and school nurses often face. Doing so should help parents and nurses better understand one another's views. Third, we explore ways in which parents and nurses sometimes disagre

Candidacy Decisions for Long-term Ventilation.

Decisions to initiate long-term ventilation (LTV) in children with severe neurologic impairment have recently been subject to candidacy determinations by home ventilation teams that exclude patients based on their neurologic status alone. Determinations of whether decisions are inappropriate require careful analysis of specific clinical circumstances and attention to the family's values. In this Ethics Rounds, we present a case of a previously healthy child who sustained an acute severe anoxic brain injury and was assessed by the medical team to have a high likelihood of remaining minimally co

The Wrong of Eugenic Sterilization.

I defend a novel account of the wrong of subjecting people to non-consensual sterilization (NCS), particularly in the context of the state-sponsored eugenics programmes once prevalent in the United States. What makes the eugenicist practice of NCS distinctively wrong, I claim, is its dehumanizing core: the fact that it is tantamount to treating people as nonhuman animals, thereby expressing the degrading social meaning that they have the value of animals. The practice of NCS is prima facie seriously wrong partly, but crucially, on these grounds. I consider and reject accounts of the wrong of N

[Ethical questions in the clinical use and research of electroconvulsive therapy].

Electroconvulsive therapy (ECT) is one of psychiatry's most long-standing and criticized treatments. Some of the criticisms come from the ethical aspect of ECT. The authors review the ethical issues of clinical application and research of ECT. ECT in the treatment of psychiatric patients is considered ethical, if it is conducted according to the four main principles of bioethics (beneficence, non-maleficence, autonomy and justice). ECT research should also comply with other international guidelines and research ethics codes. In recent decades, the principle of informed consent has become wides

Frequent Preservation of Neurologic Function in Brain Death and Brainstem Death Entails False-Positive Misdiagnosis and Cerebral Perfusion.

Some patients who have been diagnosed as "dead by neurologic criteria" continue to exhibit certain brain functions, most commonly, neuroendocrine functions. This preservation of neurologic function after the diagnosis of "brain death" or "brainstem death" is an ongoing source of controversy and concern in the medical, bioethics, and legal literatures. Most obviously, if some brain function persists, then it is not the case that all functions of the entire brain have ceased and hence, declaring such a patient to be "dead" would be a false positive, in any nation with so-called "whole brain deat

Finding Consensus After Two Decades of Breast Implant-Associated Anaplastic Large Cell Lymphoma.

Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) is an emerging and indolent, but potentially fatal cancer of the immune system that can develop around textured-surface breast implants. The World Health Organization first recognized BIA-ALCL as a unique clinical entity in 2016. To date, over 600 confirmed cases have been reported worldwide. BIA-ALCL most commonly presents with disease confined to the capsule, as a seroma or a mass adjacent to the implant. While BIA-ALCL has a fairly indolent clinical course, with an excellent prognosis in early stage disease, disseminated ca

Organ Transplantation From Nonstandard Risk Donors: Midway Between Rigid and Flexible Rules.

In order to bridge the gap between available organs and patients needing transplants, donor selection criteria for donors are increasingly being extended; the possibility of using organs from nonstandard risk donors has been introduced in many countries. This clearly poses considerable ethical issues that should be analyzed and taken into consideration by the competent bodies and institutions. In this article, we illustrate the Italian situation regarding the possibility of using organs from anti-hepatitis C virus (HCV) and HCV RNA-positive donors (anti-HCV+ve) in negative recipients (healthy

[The status of the human embryo from a gradualistic perspective].

Bioethics debates topics regarding the beginning and ending of human life. The beginning of human life includes when a human embryo reaches full human status. There are multiple and interesting issues surrounding human embryo status. These include assisted reproduction, seeking and management of human gametes and embryos at specialized laboratories, embryonic and stem cell research, emergency contraception, voluntary interruption of pregnancy, etc. They are based on implicit or explicit philosophical concepts. Philosophical positions about human embryo status could be summarized in two posture

Malignant hyperthermia during pediatric kidney transplantation-A medical and ethical quandary.

Malignant hyperthermia (MH) is a rare life-threatening anesthetic complication with high mortality rates. MH during adult kidney transplant has been reported previously. However, the occurrence of MH after multiple previous uneventful anesthetic exposures in a pediatric kidney transplant recipient is rare. To our knowledge, this is the first reported case of MH in a child undergoing a live donor kidney transplant. The approaches for addressing perioperative challenges and ethical dilemmas to ensure successful outcomes are described. The recipient, a 5-year-old male child, weighing 20 kg, with

Constructing the death elephant: a synthetic paradigm shift for the definition, criteria, and tests for death.

In debates about criteria for human death, several camps have emerged, the main two focusing on either loss of the "organism as a whole" (the mainstream view) or loss of consciousness or "personhood." Controversies also rage over the proper definition of "irreversible" in criteria for death. The situation is reminiscent of the proverbial blind men palpating an elephant; each describes the creature according to the part he can touch. Similarly, each camp grasps some aspect of the complex reality of death. The personhood camp, in contrast to the mainstream "organism" camp, recognizes that a huma

Ethical considerations of genetic presymptomatic testing for Huntington's disease.

The aim of this literature review was to determine if there is adequate ethical justification for presymptomatic genetic testing on potential Huntington's disease patients. Huntington's disease is a neurological genetic disorder characterized by midlife onset which consists of cognitive, physical, and emotional deterioration. Although genetic testing has traditionally been guided by the principle of autonomy, severe psychological consequences such as depression, anxiety, survival guilt, and suicide have complicated the ethical issue of providing a presymptomatic yet definitive diagnosis for an

How Bioethicists Can Contribute to Democratic Deliberation.

It is sometimes suggested that policymaking on bioethical questions should be more democratic, either through direct popular participation (as in a referendum) or through improved forms of representation (such as random mini-publics or elected officials). Such proposals seem to leave no special place for input from bioethicists. This paper argues that it is not contrary to democracy to give bioethicists additional influence over relevant policy areas. First, while referenda, mini-publics, or electoral representation may be more democratic in some respects, these processes also have limitations

Maternalism in Palliative Care: A Case for Relational Autonomy and Informed Non-Dissent.

BACKGROUND: Clinicians often face situations in which patients or families desire aggressive care that medical teams believe offers no meaningful benefit. Traditional shared decision-making frameworks may inadequately address these tensions. CASE: A 53-year-old woman with metastatic lung cancer desired full resuscitation despite progressive multisystem organ failure. She identified as a "fighter" but faced a clinical dilemma: adequate pain control required accepting respiratory compromise. DISCUSSION: We apply the bioethical construct of maternalism, integrating relational autonomy and infor

[Early detection of risk for antisocial behaviour: towards moral bio enhancement?].

BACKGROUND: New technological possibilities for early identification of (risk) of later antisocial behavior. AIM: To raise the question whether, or under what conditions, early identification is in the best interest of the (young) child. METHOD: Ethical analysis in which fetal alcohol spectrum disorder (FASD) is central. RESULTS: The bioethical debate on 'moral bio enhancement' investigates the ethical desirability of practical applications of neurobiological and behavioral genetic knowledge of moral and immoral human behavior. One of those applications is the early detection of (risk) of b

Brief psychosis induced by methylphenidate in a child with attention deficit disorder: a case report and literature review.

Methylphenidate-induced psychosis has been scarcely studied due to bioethical and neurobiological issues regarding its research. Although some authors have hypothesized that there might be a vulnerability for the development of a major psychiatric disorder in the long term, there is no agreement about the predictive value of this type of psychosis in children and adolescents, and its origin is also uncertain. It has been suggested that higher dopamine levels in some cerebral regions and a family history of mental disorders might increase the risk of psychosis secondary to psychostimulants. We

Brains, genes, and primates.

One of the great strengths of the mouse model is the wide array of genetic tools that have been developed. Striking examples include methods for directed modification of the genome, and for regulated expression or inactivation of genes. Within neuroscience, it is now routine to express reporter genes, neuronal activity indicators, and opsins in specific neuronal types in the mouse. However, there are considerable anatomical, physiological, cognitive, and behavioral differences between the mouse and the human that, in some areas of inquiry, limit the degree to which insights derived from the mo

All in the Blood: A Review of Aboriginal Australians' Cultural Beliefs About Blood and Implications for Biospecimen Research.

Public participation in medical research and biobanking is considered key to advances in scientific discovery and translation to improved health care. Cultural concerns relating to blood have been found to affect the participation of indigenous peoples and minorities in research, but such concerns are rarely specified in the literature. This article presents a review of the role of blood in Australian Aboriginal cultures. We discuss the range of meanings and uses of blood in traditional culture, including their use in ceremonies, healing, and sorcery. We draw on more recent literature on Abori
