BACKGROUND: Impairments in psychosocial functioning have been demonstrated in 30-60% of adults with bipolar disorder (BD). However, the majority of studies investigating the effect of comorbid mental health disorders and age at onset outcomes in BD have focused on traditional outcome measures such as mood symptoms, mortality and treatment response. Therefore, this project aimed to investigate the impact of comorbid mental health disorders and age at onset on longitudinal psychosocial outcome in participants with BD.
METHOD: Mixed effects modelling was conducted using data from the Stanley Fou…

OBJECTIVE: Obsessive-compulsive disorder (OCD) frequently co-occurs with bipolar disorder (BD) or schizophrenia (SCZ), and, importantly, can often precede their onset. However, the genetic architecture and directionality underlying these relationships remain unclear. We leveraged large-scale genome-wide association study (GWAS) data to examine shared genetic architecture and directional relationships among OCD, BD and SCZ, and used major depressive disorder (MDD) as a comparator.
METHODS: Using linkage disequilibrium score regression (LDSC), MiXeR, and Generalized Summary-data-based Mendelian…

OBJECTIVE: Autism spectrum disorder (ASD) and bipolar disorder (BD) pose significant diagnostic challenges due to their clinical complexity. This review aims to examine the interface and overlapping features of these conditions, with a particular focus on the challenges associated with their comorbidity.
METHODS: We conducted a narrative review to examine clinical overlap, common psychiatric comorbidities, and a shared neurobiological basis between ASD and BD.
RESULTS: There is a notable convergence of symptoms in ASD and BD, including mood instability and emotional dysregulation; irritabili…

BACKGROUND: Conduct Disorder (CD) is highly comorbid with Bipolar Disorder (BP) and this comorbidity is associated with high morbidity and dysfunction. We sought to better understand the clinical characteristics and familiality of comorbid BP + CD by examining children with BP with and without co-morbid CD.
METHODS: 357 subjects with BP were derived from two independent datasets of youth with and without BP. All subjects were evaluated with structured diagnostic interviews, the Child Behavior Checklist (CBCL), and neuropsychological testing. We stratified the sample of subjects with BP by the…

IMPORTANCE: Reward circuitry dysfunction is a candidate mechanism of co-occurring bipolar disorder and alcohol use disorder (BD + AUD) that remains understudied. This functional magnetic resonance imaging (fMRI) research represents the first evaluation of alcohol cue reward processing in BD + AUD.
OBJECTIVE: To determine how alcohol cue processing in individuals with BD + AUD may be distinct from that of individuals with AUD or BD alone.
DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional case-control study (April 2013-June 2018) followed a 2 × 2 factorial design and included individuals…

BACKGROUND: Major depressive disorder and bipolar disorder are prevalent and debilitating psychiatric disorders that are difficult to distinguish, as their diagnosis is based on behavioural observations and subjective symptoms. Quantitative protein profile analysis might help to objectively distinguish between these disorders and increase our understanding of their pathophysiology. Thus, this study was conducted to compare the peripheral protein profiles between the two disorders.
METHODS: Serum samples were collected from 18 subjects with major depressive disorder and 15 subjects with bipola…

BACKGROUND: Patients with obsessive-compulsive disorder (OCD) often present with comorbidities, mainly anxiety and affective disorders, which may influence OCD course, help-seeking and treatment response. Some authors have studied bipolar disorder (BD) comorbidity in patients with OCD, but usually in small samples. The objective was to estimate the lifetime prevalence of BD in a large clinical sample of OCD patients, and to compare demographic and clinical features of patients with and without BD comorbidity.
METHOD: This cross-sectional study with 955 adult OCD patients from the Brazilian Re…

BACKGROUND: Local functional connectivity (FC) indicates local or short-distance functional interactions and may serve as a neuroimaging marker to investigate the human brain connectome. Local FC alterations suggest a disrupted balance in the local functionality of the whole brain network and are increasingly implicated in schizophrenia (SZ), bipolar disorder (BD), and major depressive disorder (MDD).
METHODS: We aim to examine the similarities and differences in the local FC across SZ, BD, and MDD. In total, 537 participants (SZ, 126; BD, 97; MDD, 126; and healthy controls, 188) completed re…

Obsessive compulsive features occurring in mania have been well documented. Though there have been some studies on obsessive compulsive disorder (OCD) comorbid with mania in the western countries, there are very few case reports and studies in India. Our aim is to report one such case here, who presented with OC features which are not typical of the symptom cluster of the OCD commonly seen with mania in earlier reports. Also, the comorbidities in OCD should be recognized as this can have important diagnostic and therapeutic implications.

Recent theories suggest that children with pediatric bipolar disorder (PBD) may exhibit more difficult temperaments premorbidly, including traits such as behavioral disinhibition and difficulty with emotion regulation. We investigated temperament characteristics retrospectively during infancy and toddlerhood in subjects with PBD (n=25), attention-deficit/hyperactivity disorder (ADHD; n=25), and healthy controls (n=25). Children with PBD were reported to experience increased difficult temperament in both infancy and toddlerhood compared to children with ADHD. Several characteristics of difficul…

PURPOSE OF REVIEW: To examine the changes in prevalence estimates and concepts of core disorder in two child mental disorders that were once considered rare, and to place these changes in a cultural context.
RECENT FINDINGS: Up to one-quarter of people with bipolar disorder may have experienced onset of symptoms prior to puberty, but the precision of the diagnosis in children is uncertain. An ongoing challenge is differentiating bipolar disorder from other child mental disorders. Reliable markers of persistent bipolarity have yet to be identified. Despite a popular perception, pervasive devel…

BACKGROUND: Manic-depression or bipolar disorder (BD) is a multi-faceted illness with an inevitably complex treatment.
METHODS: This article summarizes the current status of our knowledge and practice of its treatment.
RESULTS: It is widely accepted that lithium is moderately useful during all phases of bipolar illness and it might possess a specific effectiveness on suicidal prevention. Both first and second generation antipsychotics are widely used and the FDA has approved olanzapine, risperidone, quetiapine, ziprasidone and aripiprazole for the treatment of acute mania. These could also b…

BACKGROUND: Because of their overlapping phenomenology and mutually chronic, persistent nature, distinctions between bipolar disorder and cluster B personality disorders remain a source of unresolved clinical controversy. The extent to which comorbid personality disorders impact course and outcome for bipolar patients also has received little systematic study.
METHOD: One hundred DSM-IV bipolar I (N = 73) or II (N = 27) patients consecutively underwent diagnostic evaluations with structured clinical interviews for DSM-IV Axis I and cluster B Axis II disorders, along with assessments of histor…

The aim of this study was to assess the comorbidity of lifetime and current prevalences of anxiety disorders among 70 patients with bipolar I disorder in remission using structured diagnostic interviews and to examine the association between comorbidity and several demographic and clinical variables. Forty-three (61.4%) bipolar I patients also met DSM-IV criteria for at least one lifetime comorbid anxiety disorder. Obsessive-compulsive disorder (39%) was the most common comorbid lifetime anxiety disorder, followed by simple phobia (26%) and social phobia (20%). First episode and male sex were

Ultra-rapid cycling disorder, in which mood episodes shift over days rather than months, sits at one of the most difficult ends of the bipolar spectrum. It resists standard treatment, tends to worsen with antidepressants, and has very little long-term outcome data to guide clinical decisions. We report the case of a 51-year-old woman whose ultra-rapid cycling bipolar disorder was tracked for 130 weeks. She underwent trials of multiple mood stabilizers, five second-generation antipsychotics, and two courses of electroconvulsive therapy (ECT), none of which produced sustained remission. Clozapin…
