Insufficient vascularisation of substitutes in the operative reconstruction of the oesophagus is one of the main causes of the occurrence of anastomotic leaks. BACKGROUND: Animal tissues and tissue-derived biomaterials are widely used in the field of xenotransplantation and regenerative medicine. A potential immunogenic risk that affects the safety and effectiveness of xenografts is the presence of remnant α-Gal antigen (synthesized by GGTA1 or/and iGb3S). GGTA1 knockout mice have been developed as a suitable model for the analysis of anti-Gal antibody-mediated immunogenicity. BACKGROUND: We previously developed substitutes for red blood cells (RBCs) and platelets (PLTs) for transfusion. These substitutes included hemoglobin vesicles (HbVs) and fibrinogen γ-chain (dodecapeptide HHLGGAKQAGDV, H12)-coated, adenosine diphosphate (ADP)-encapsulated liposomes [H12-(ADP)-liposomes]. Hemodilution can be used to save blood transfusion during total hip replacement. We have carried out a randomized study to compare the hemorheological effects of two plasma substitutes: a hydroxyethylstarch 200,000/0.62 versus a dextran 60,000. Twenty-two patients were hemodiluted with 20 mk/kg of either substitute, just after the spinal anesthesia. Blood-transfusions are affected with considerable risks and undesirable reactions, for that reason the application of whole blood is indicated strictly. The comparison of transfusions in patients having undergone a lung resection showed a decrease in transfusions about 50 per cent in a period of 10 years. The reduction of transfusions is the result of an increased use of blood substitutes. Advantages and possibilities of hemodilution are discussed. The application of whole blood depends on preoperative hematocrit and calculated blood volume.
