Fresh blood for transfusion in adults with beta thalassaemia.

BACKGROUND: Patients with beta-thalassaemia major require life-long blood transfusion with the aim of achieving normal growth and development whilst minimising iron overload. A pre-transfusion Hb between 9.5 and 10 g/dL is thought to achieve this balance. UK consensus is that fresh blood (less than 14 days) is better at maintaining this target pre-transfusion Hb but there is no firm stipulation in place and no robust evidence supporting this. METHODS: After introduction of a universal fresh blood policy for adult beta-thalassaemics in 2010, we reviewed locally transfused adult patients to det…

Blood transfusion and the anaesthetist: management of massive haemorrhage.

1. Hospitals must have a major haemorrhage protocol in place and this should include clinical, laboratory and logistic responses. 2. Immediate control of obvious bleeding is of paramount importance (pressure, tourniquet, haemostatic dressings). 3. The major haemorrhage protocol must be mobilised immediately when a massive haemorrhage situation is declared. 4. A fibrinogen < 1 g.l−1 or a prothrombin time (PT) and activated partial thromboplastin time (aPTT) of > 1.5 times normal represents established haemostatic failure and is predictive of microvascular bleeding. Early infusion of fresh froze

[The organizational issues of the blood transfusion service operation].

The data on the security maintenance of blood, its components and preparations is presented. The issues of blood plasma quarantinization period are discussed. The results of plasma culling as a consequence of second donor non-attendance and second examination of fresh-frozen blood plasma are discussed. It is mentioned that it would be appropriate to expose to inactivation and to virus elimination the blood plasma which has to be a subject to destruction for one or another reason. At the same time it is preferably to apply the technology of virus inactivation of the fresh-frozen blood plasma ut

Transmission of Babesia microti by blood transfusion in Texas.

In the USA, seasonal tickborne transmission of Babesia microti occurs in the Northeast and upper Midwest. A resident of Texas became infected through a red blood cell transfusion from an asymptomatic local donor who had summered in Massachusetts. The patient's infection was diagnosed by blood smear examination in January, 7 weeks post-transfusion. He died 1 week later from variceal haemorrhage complicated by haemolysis. Premortem patient specimens and archived blood from the donor unit tested positive for B. microti antibodies and DNA. Babesiosis should be included in the differential diagnosi

Blood transfusion and caesarean section in a developing country.

Our objective was to review blood transfusion practices during caesarean section in a developing country. An audit of 463 consecutive caesarean sections and blood transfusions over a 3-year period (2000 - 2002) was undertaken. The data were collected from the records department in a pre-designed proforma and analysed, using EPI - info Statistical Software version 6. A total of 117 out of 463 (25.2%) caesarean section cases were transfused. The rate of blood transfusion for the various indications were as follows: malpresentation (excluding breech), four out of six (66.7%); placenta praevia, 28…

Massive blood loss and transfusion in obstetrics and gynecology.

UNLABELLED: Massive perioperative or periparturitional bleeding occasionally occurs in obstetric and gynecologic patients. Placenta previa, uterine atony, and ectopic pregnancy are just a few examples of many conditions that could predispose patients to significant blood loss. Therefore, it is important for physicians specializing in obstetrics and gynecology to be proficient in managing episodes of massive hemorrhage and the practice of the most commonly used blood components. We review and update the management of massive hemorrhage for obstetrics and gynecologic patients. In addition, we ex

Allosensitization in patients receiving multiple blood transfusions.

Extended antigen (C, E, K) matching decreased the incidence of alloantibody (alloAB) and autoantibody (autoAB) formation, in addition to eliminating transfusion reactions in the multiply transfused sickle cell disease patients. AlloAB formation possibly transforms the immune system into a hyperactive state leading to further and earlier alloAB and autoAB formation. However, additional CEK matching results in marked overuse of Rh-negative packed red blood cell (pRBC) units, 30 minutes' extra time of a skilled technologist, and 153 dollars extra CEK reagent cost per unit to find CEK-matched pRBC

Immunologic consequences of blood transfusion and their clinical manifestations.

Aspects of clinical immunology, in the context of transfusion medicine, became highlighted in the last decade as a consequence of the accelerating expansion of basic immunology, immunogenetics and molecular biology. In addition sophisticated new technologies, which were capable of producing pure and safe blood products, attracted more attention to research and monitor the consequences of transfusion. These technologies also had obvious effects on supportive hematological therapy. The transfusion of blood components follows the rules of organ transplantation: when there is a mismatch between th

Blood transfusions in laryngeal cancer: effect on prognosis.

METHODS: To evaluate the influence of homologous perioperative transfusion on oncologic control and survival in patients with larynx and hypopharynx carcinomas, we conducted a retrospective study of 269 patients with larynx and hypopharynx carcinoma treated by major surgical procedures. A total of 20 variables were analyzed for each patient. RESULTS: Perioperative transfusion was required in 86 (32%) patients, with packed red blood cells being used in all cases. Recurrence at a local, regional, or distant level was 31% for nontransfused and 35% for transfused patients. The 5-year adjusted sur

Benefits of HIV screening of blood transfusions in Zambia.

Blood transfusion continues to be an important route of transmission of HIV in developing countries, especially for young children following the perinatal period. Testing for HIV is costly and reliable donor support for the purchase of test kits is often essential, yet difficult to secure. The costs of screening transfusions for HIV and the financial benefits in terms of savings on treatment costs averted were calculated for a district hospital in Zambia where seroprevalence among donors was 15.9%. Financial benefits exceed costs by a factor of 2.7-3.5. In 1991, 1073 transfusions were given an…

Transfusion of type-A and type-B blood to cats.

Although nearly all domestic shorthair and longhair cats have type-A blood (greater than 99%), the frequency of blood type B in various feline breeds ranges from 0 to 59%. All blood-type-B cats have strong natural alloantibodies, predominantly of the IgM class, whereas blood-type-A cats have low alloantibody titers of the IgG and IgM classes. We therefore studied the efficacy and safety of transfusing 20 ml of matched and mismatched 14C-potassium cyanate-labeled blood to cats. In autologous and allogeneic matched transfusions of blood-type-A and type-B cats, the half-life of labeled erythrocyt

Changes in the requirements for blood transfusion in brain surgery.

The amount of blood used in transfusions during certain neurosurgical operations was less in 1978-79 than in 1971-72 and in 1965-66. The operations investigated were for gliomas and meningiomas of the brain, pituitary adenomas, acoustic neurinomas, arteriovenous malformations, and arterial aneurysms. The major change in anaesthetic techniques between 1965-66 and 1971-72 was the introduction of hypocapnia by controlled artificial hyperventilation. We suggest that this was the main factor responsible for the reduction in the need for blood transfusions. The avoidance of halothane, the use of ind…

Post-transfusion cytomegaloviremia and persistence of cytomegalovirus in blood.

Cytomegaloviremia was documented in seven patients. Three patients were renal transplant recipients and two were liver transplant recipients. One was a postcardiac surgery patient, and one had acute myelogenous leukemia. The transplant patients had received only banked blood or fresh frozen plasma and developed evidence of infection about 1 month after surgery. Illness varied from no apparent signs to fever alone, fever and a morbilliform rash, hepatitis and an infectious mononucleosis syndrome. The virus was isolated from the blood erythrocyte layer, the leukocyte layer, and the plasma and se
