Proceedings of a consensus conference: blood-borne HIV and hepatitis-optimizing the donor selection process.

Over the past several years, concern for the safety of blood transfusion has evolved from a primary focus on bacterial infections such as syphilis to viral infections including hepatitis B, C, and human immunodeficiency virus (HIV), and most recently, the theoretical risk of variant Creutzfeldt-Jakob Disease (vCJD). As a result of these changes, blood bankers have found themselves faced with the dilemma of balancing the rights of transfusion recipients to receive safe blood products, with the rights of individuals wishing to donate blood as an altruistic contribution to society.

Diagnosis and epidemiology of transfusion-associated human cytomegalovirus infection: recent developments.

OBJECTIVE: The prevention and diagnosis of transfusion-associated HCMV infection has to address different questions: Is the blood donor potentially infections? Is the transfused recipient undergoing an active primary or secondary infection? DATA SOURCES: International literature (Current Contents Life Sciences) and reports of our study-group. SELECTION CRITERIA: Original articles in English and German. RESULTS: Monocytes have been identified as the major site of latency of human cytomegalovirus (HCMV) in peripheral blood. High seropositivity rates are observed in polytransfused individuals,

Immature Ixodes scapularis (Acari: Ixodidae) Collected From Peromyscus leucopus (Rodentia: Cricetidae) and Peromyscus maniculatus (Rodentia: Cricetidae) Nests in Northern Wisconsin.

The blacklegged tick, Ixodes scapularis Say, is the primary Lyme disease vector in the eastern United States. Both immature stages of I. scapularis take blood meals from mice belonging to the genus Peromyscus. Mice are active during the night and spend the majority of diel periods in nests. Thus, immature I. scapularis have a greater opportunity to drop from Peromyscus hosts while in nests compared with the forest floor. Here, we collected 11 Peromyscus nests during a 3-mo period during which the immature I. scapularis are known to be active. We then examined nesting materials for the presence

Immune Evasion Strategies of Relapsing Fever Spirochetes.

Relapsing fever (RF) is claimed a neglected arthropod-borne disease caused by a number of diverse human pathogenic Borrelia (B.) species. These RF borreliae are separated into the groups of tick-transmitted species including B. duttonii, B. hermsii, B. parkeri, B. turicatae, B. hispanica, B. persica, B. caucasica, and B. myiamotoi, and the louse-borne Borrelia species B. recurrentis. As typical blood-borne pathogens achieving high cell concentrations in human blood, RF borreliae (RFB) must outwit innate immunity, in particular complement as the first line of defense. One prominent strategy dev…

Retrospective evaluation of laboratory data on canine vector-borne infections from the years 2004-2008.

The detection and therapy of canine vector-borne diseases in imported dogs are of major importance in small animal practice. Over the last years, the import of dogs from the Mediterranean region and Southeast Europe has increased, countries often endemic for a variety of vector-borne diseases such as babesiosis, hepatozoonosis, leishmaniosis, dirofilariosis or ehrlichiosis. This retrospective study presents the evaluation of data from our diagnostic laboratory on vector-borne infections in imported dogs from the years 2004-2008. Specific antibodies were detectable in 20.5% of all samples with

Identification of Theileria buffeli/orientalis and Babesia bigemina in Apulian cattle using molecular techniques and study of changes in blood parameters.

Recently several cases of theileriosis due to the haemoprotozoan Theileria buffeli/orientalis have been recorded in the Apulian region, Italy. In this area other tick-borne pathogens were usually identified such as Anaplasma marginale and Babesia bigemina. Outbreaks were recorded showing that these pathogens can be observed separately or in mixed infections. Sub-clinical cases and carrier animals were also previously identified. A lack of specific techniques could not rule out the presence of other haemoparasites such as T. annulata, B. divergens, B. bovis, Ehrlichia phagocytophila and E. bovi

Molecular detection of vector-borne pathogens in Greek cats.

Infectious diseases have been increasingly recognized in cats worldwide. The objective of this study was the molecular investigation of the prevalence of selected pathogens in healthy and sick cats from Greece, a country highly endemic for several canine vector-borne pathogens. Blood and/or bone marrow samples from 50 clinically healthy and 50 sick adult (>1 year-old) cats were retrospectively examined for the amplification of Bartonella spp., haemoplasmas, Ehrlichia spp., Anaplasma spp., Babesia spp., and Cytauxzoon spp. DNA. Overall, 14.9% of the cats were found to be infected or co-infected

Bartonellosis.

Bartonella spp are fastidious bacteria that occur in the blood of man and mammals; they are usually vector borne but can also be transmitted by animal scratches and bites. The bartonelloses of medical importance comprise Carrión's disease, trench fever, cat-scratch disease, bacillary angiomatosis, and peliosis hepatis. Carrión's disease, known as Oroya fever in the acute phase and verruga peruana (Peruvian wart) in its chronic form, has curious manifestations that, until recently, have been restricted in their geographic distribution to dwellers of the high, dry Andean valleys, but new sites o

Occupational hazards of traditional healers: repeated unprotected blood exposures risk infectious disease transmission.

OBJECTIVE: Healers provide support for acute and chronic illnesses in rural Mozambique, such as socially acceptable traditional 'vaccinations' (subcutaneous cuts in the skin to rub herbs directly into the bloody lesion). We aimed to document the frequency of blood exposure by traditional practitioners in Mozambique. METHODS: We conducted surveys with a simple random sample of 236 traditional healers in Zambézia province. Chi-square and Wilcoxon rank-sum tests were used to compare 'injection' behaviours across districts. RESULTS: Healers treated a median of eight patients in the past month (I

A nurse-led intervention improved blood-borne virus testing and vaccination in Victorian prisons.

OBJECTIVES: Testing is the first step in treatment and care for blood-borne viruses (BBVs) and sexually transmitted infections (STIs). As new treatments for viral hepatitis emerge, it is important to document effective models for BBV/STI testing. A nurse-led intervention was implemented across three prisons in Victoria to improve BBV/STI testing. We evaluated the impact of the intervention on BBV/STI testing rates and hepatitis B (HBV) vaccination for reception prisoners. METHODS: BBV/STI testing and HBV vaccination data were collected from the medical files of 100 consecutive reception priso

[Sacrum B cell-non-Hodgkin's lymphoma complicating a chronic viral hepatitis C related to a blood exposure: a case report].

Frequency of the association between non-Hodgkin's lymphoma (NHL) and the hepatitis C virus (HCV) infection is variable according to previous studies. Besides, direct and/or indirect implication of the HCV infection in the development of NHL is probable but, its pathophysiological mechanisms remain unclear. In this report, we described the case of a 49-year-old patient with a B-cell NHL of the sacrum complicating a chronic HCV related to a blood exposure, and we report the recent data of this association.

Blood borne virus-innovative approach to evaluate, prevent cross-transmission in dialysis units.

Large public health interventions to control infectious disease outbreaks are common, but rigorous evaluation to improve the quality and effectiveness of these is rarely undertaken. This project aimed to prevent blood borne virus (BBV) cross-transmission in dialysis units. Following an incident concerning the diagnosis of acute Hepatitis B in a haemodialysis patient, possibly from healthcare associated acquisition, a multifaceted and multidisciplinary investigation was conducted involving consumers, health professionals and administrations. The results of this investigation were then used to p

A descriptive, retrospective study of 567 accidental blood exposures in healthcare workers in three West African countries.

We conducted a multi-centre study in West African hospital wards to document accidental blood exposure (ABE) risks in these settings, and assessed the incidence of ABE in participating healthcare workers (HCWs) retrospectively. In total, 1241 HCWs participated in the survey from 43 hospital wards. Among them, 567 (45.7%) had sustained at least one ABE with an estimated incidence of 0.33 percutaneous injuries (PCIs) and 0.04 mucocutaneous contacts (MCCs)/HCW/year in medical or intensive care personnel and 1.8 PCIs/HCW/year in surgeons. The ABE was a needlestick in 454 (80.1%) of 567 cases, a cu

Health professionals' knowledge of prevention strategies and protocol following percutaneous injury.

Prevention strategies and protocols for the management of percutaneous injuries are developed for the purpose of preventing transmission of HIV and other infections. However, implementation thereof requires health professionals to be conversant with the content of protocols and ways to prevent percutaneous injuries. The purpose of the study was to determine health professionals' knowledge of prevention strategies and protocols following percutaneous injury. The purpose was addressed within a quantitative survey design. Data were collected by means of a self-administered questionnaire. The stud

Prevalence of tattooing and body piercing in the Australian community.

Tattooing and body piercing are now worldwide fashion crazes. The health risks associated with these procedures are as yet unclear. This article examines the prevalence of body decoration and the associated health risks within the Australian population using a random sample survey of individuals aged 14 years and over, collected between June and September 1998 (n = 10,030). The results show that one in 10 people have had a tattoo at some point in their lives and 8 per cent some form of body piercing, excluding ear piercing. Men are more likely than women to report tattooing, while females are

Silicon phthalocyanine Pc 4 and red light causes apoptosis in HIV-infected cells.

The silicon phthalocyanine HOSiPcOSi(CH3)2(CH2)3 N(CH3)2 (Pc 4), is being studied as a photosensitizer for virus inactivation in red blood cell concentrates (RBCC). The RBCC spiked with cell-free human immunodeficiency virus (HIV) or with HIV actively replicating in the T-lymphocytic cell line CEM can be successfully inactivated (> or = 6 log10) when exposed to 2 microM Pc 4 and 90 J/cm2 red light (600-800 nm). Inactivation of > or = 6 log10 inducible HIV in the latently infected promonocytic cell line U1 occurred at 22.5 J/cm2 (H. Margolis-Nunno et al., Transfusion 36, 743-750, 1996). In orde

Parasites and microorganisms associated with the snakes collected for the "festa Dei serpari" in Cocullo, Italy.

While in much of the Western world snakes are feared, in the small, rural, mountainous town of Cocullo, in the middle of central Italy, snakes are annually collected and celebrated in a sacro-profane ritual. Every 1st of May, Serpari (snake catchers) capture and showcase dozens of non-venomous snakes to celebrate the ritual of San Domenico. In order to detect potential zoonotic pathogens within this unique epidemiological context, parasites and microorganisms of snakes harvested for the "festa dei serpari" ritual were investigated. Snakes (n = 112) were examined and ectoparasites collected, as
