Although osteosynthesis using K-wires is a commonly performed procedure, the outcome is not always preferable. We aimed to identify the prognostic factors for postoperative complications in paediatric patients with both-bone forearm fractures. We reviewed 160 paediatric patients who underwent K-wire osteosynthesis for either distal or diaphyseal forearm fractures. We defined the occurrence rate of postoperative complications as the primary outcome and performed a multivariate logistic regression analysis. Variables such as age, sex, mechanism of injury, presence of an open fracture, ulnar or r

The case involves a 53-year-old patient admitted for trauma to the right hand following an accident with a grinder. The 4 cm wound on the first ray caused a tendon deficit without vascular or nerve damage. An open fracture of the first metacarpal with a 2 cm bone loss was diagnosed. Initial treatment included wound irrigation, debridement, suturing, and osteosynthesis using biological cement and a Bone fixation using Iselin pinning with two K-wires. A second procedure, six weeks later, involved a bone graft using the Masquelet technique, showing bone consolidation after five months. This techn

A 2-year-old female child with craniosynostosis was referred to our unit for definitive management. She presented with a turricephalic head, hypoplastic midface and obstructive sleep apnoea. Routine preoperative workup included radiographs, CT and polysomnography. She was provisionally planned for calvarial remodelling and midface advancement using transfacial pin fixated distractor (TPF). This involves insertion of a K-wire from one zygoma to the other, a few millimetres below the orbits, traversing across maxilla and nasal cavity. The ends of K-wire are then connected to the distractor ancho

Congenital coalitions are defined as abnormal osseous or fibrocartilaginous bridges between two bones due to failure of embryonic segmentation. In the foot, tarsal bone coalitions are commonly seen, but intermetatarsal coalition is extremely rare and very few cases have been reported in literature to date. We report a case of a unilateral, congenital metatarsal coalition between the fourth and fifth metatarsal bones in a kid in his middle childhood, who presented with unilateral chronic forefoot pain on activity with keratosis under the fifth metatarsal head.A thorough history with clinical ex

Lunotriquetral arthrodesis has been recommended for the treatment of disabling pain at the lunotriquetral joint after more conservative measures have failed. We retrospectively analyzed a series of 22 patients treated with a lunotriquetral arthrodesis for presence of fusion, method of fixation, duration of immobilization, carpal alignment, pain relief, and ability to return to work. Fixation with a Herbert screw supplemented with a Kirschner wire (K-wire) was superior to fixation with K-wires alone. Immobilization longer than 6 weeks was superior to immobilization less than 6 weeks. Combined u

When open reduction is indicated the lateral malleolus fracture should be reduced as accurately as the medial or posterior malleolar fragments. The cerclage for displaced oblique or spiral fractures of the lateral malleolus is a simple, effective way of internal fixation with a minimal amount of metal implanted. Under direct vision an anatomical reduction is achieved and maintained. The proper use of a 20- or 18 -gauge wire with a mechanical wire tightner-twister is mandatory. A below-the-knee plaster cast is necessary for support for an average of eight weeks. In this series, with correct tec

BACKGROUND: Chronic dislocation of the dorsal surface of the thumb metacarpophalangeal (MCP) joint is uncommon and often is accompanied by soft tissue interposition with degenerative changes, especially in delayed presentations, which are common in low resource settings.
CASE PRESENTATION: We present the case of a 21-year-old right-handed male that developed a dorsal MCP dislocation of the thumb 11 years ago with initial traditional bone setter management. He presented with pain, deformity and poor grip function. The chronic dislocation was confirmed on radiographs with secondary degenerative

The goal of this study was to describe an internal fixation technique for periprosthetic humeral fractures using a cortical onlay strut allograft stabilized with cerclage wires and to evaluate the preliminary results of this approach. An anterolateral approach was used to direct access the fracture and to protect the radial nerve. The fracture was reduced with two forceps, under visual control. The fracture was surrounded by two hemicylinder tibial allografts, placed around the humerus with no prior reaming to create a "sarcophagus" system. The allograft was as long as possible for optimal mec

The aim of the present study was to establish the typical ultrasound (US) patterns of metal implants used for internal fracture fixation, and consequently to use US for the identification and localisation of such implants. We investigated both the visualization of the implants in term of size and shape, material (titanium, steel, biodegradable screws) and surface structures, and possible changes in the echo pattern in relationship to surrounding structures (muscles, body fluid), proximity to bone, and changes in the angle of insonation. For this purpose ultrasonography was performed on artific

In a first experimental study we investigated the visualisation of metal implants in reference to dimension, shape, material (titanium, steel, biodegradable screws) and surface structures and an eventual change of the echo pattern in correlation to the surrounding structures, the vicinity to bone and the change of the sound angle. For this purpose ultrasound investigation was performed in artificial and cadaver bones and in cadaver limbs after implantation of screws, plates, K-wires and cerclage wires. We found that metal implants of a certain dimension can be localised by typical artefacts (r

Intraoperative trochanteric fractures during primary and revision total hip arthroplasty typically occur during femoral canal preparation and component placement. Several fixation strategies, including wires, cables, cable grips, and plating, are available for fracture fixation. Surgeons should consider patient activity level preoperatively, bone mineral density, and fracture morphology when deciding on fixation strategies. Patient activity must be modified postoperatively to prevent fracture displacement and additional complications. Patients must be counseled postoperatively about the possib

Isolated dislocation of the first cuneiform bone is an extremely rare lesion that can usually be diagnosed on plain radiography. Few cases of this injury have been previously described in the literature. The case presented here resulted from a torsion injury and was treated by means of closed reduction with the aid of pointed reduction forceps. After fixation with Kirschner wires, the foot was immobilized for 6 weeks. The patient recovered uneventfully and remains asymptomatic 3 years later.

Therapy and prognosis of the slipped capital femoral epiphysis are discussed in connection with the results obtained in the authors' own patients. Reduction of a dislocated femoral head is always desirable. If closed reduction fails, it will be necessary to reduce in full view. In smaller children, fixation is effected by means of several Kirschner wires, in older ones via screw osteosynthesis. Bone pegging or osteotomy are not suited for treatment of an acute or subacute slipped capital epiphysis.

Osteotomy may be indicated in digital phalanges to correct angular or rotational deformity resulting from malunited fracture, epiphyseal arrest, or congenital or developmental causes. The use of fine power tools facilitates precise correction. lateral angulation is corrected by closing wedge osteotomy, dorsal angulation by opening wedge and bone graft, rotational deformity by transverse osteotomy, and derotation through the involved phalanx. Smooth Kirschner wires stabilize the osteotomies to permit early joint motion. These procedures can safely be performed in children.

Fractures of the forearm are common among children and adolescents. Radial shaft fracture with dislocation of the distal radioulnar joint (DRUJ), called Galeazzi fracture, is unusual in pediatrics. The Galeazzi-equivalent fracture is a variant of the classic Galeazzi fracture that occurs in children and adolescents. It is a radius fracture associated with a distal ulnar displaced physeal injury without dislocation of the DRUJ. Our patient was a male, aged 15 years, who visited our emergency department after falling off a scooter onto his left hand. Left wrist X-rays showed a displaced Galeazzi

Osteochondral lesions (OCLs) that are frequently encountered in skeletally immature and adult patients are more common than once thought, and their incidence rate is rising. These lesions can appear in many synovial joints of the body, such as the shoulder, elbow, hip, and ankle, occurring most often in the knee. The term osteochondral lesion includes a vast spectrum of pathologies such as osteochondritis dissecans, osteochondral defects, osteochondral fractures, and osteonecrosis of the subchondral bone. When considering this, the term osteochondral fracture is preserved only for an osteochon

PURPOSE: The purpose of this study was to simulate and calculate the probability of iatrogenic perforation of the scaphoid cortical bone when internal fixation appeared to be safe on radiographs. The results will assist surgeons in determining proper screw placement.
METHODS: Thirty scaphoids were reconstructed using computed tomography data and image-processing software. Different central axes were determined by the software to simulate the surgical views. The safe zone (SZ) and risk zone (RZ) were identified on the axial projection radiographs by comparing the scaphoid bone stenosis measure

IntroductionFirst metatarsophalangeal (MTP) joint arthrodesis is the gold-standard treatment for end-stage arthritis and hallux deformities. Successful fusion is improved by achieving adequate compression across the joint. Traditional plating systems provide single-point compression, whereas the Arthrex MTP valgus plate is designed for double compression, potentially improving stability and union rates. This study aimed to confirm double compression and compare two anchoring methods: K-wire and olive tip guide wire (BB-tack).MethodsTwelve cadaveric feet were randomised to plate fixation using
