From Suspected Brain Tumor to Cerebral Abscess: A Case Report.

Cerebral abscesses can mimic brain tumors or metastases on imaging, posing a significant diagnostic challenge, especially in patients without obvious infectious sources. We report the case of a 46-year-old woman with a history of spinal cord lesion biopsy, who presented with a three-week history of headaches and vomiting, progressing to dysarthria, right-sided hemiplegia, and consciousness impairment over 24 hours. On admission, she was unconscious (Glasgow Coma Scale (GCS) 7/15), with left anisocoria, requiring intubation and mechanical ventilation. Brain CT revealed intra-axial lesions sugge

Cryptogenic Perirolandic Brain Abscess in an Otherwise Healthy Young Man.

A previously healthy 19-year-old male presented with 1 day of transient right-sided weakness, numbness, and gait disequilibrium after recent self-limited sinonasal symptoms and minor nasal trauma with epistaxis. He was afebrile but with focal deficits, leukocytosis, and elevated C-reactive protein. Noncontrast head CT showed a left frontal lesion with vasogenic edema; MRI demonstrated a 2.4 × 1.4 cm rim-enhancing lesion in the left paramedian precentral gyrus with homogeneous diffusion restriction, low apparent diffusion coefficient, and surrounding edema. Stereotactic aspiration was performed

Fusobacterium abscessus sp. nov., associated with brain abscess in humans.

Recent genome-based phylogenetic investigations of the Fusobacterium nucleatum group have revealed that genomes currently annotated as F. nucleatum (sensu stricto) bifurcate into two separate phylogenetic groups: a large cluster containing the F. nucleatum type strain and a smaller cluster possibly representing a novel species. To further investigate this, we searched our strain archive for Fusobacterium isolates potentially representing the smaller cluster. A strain Vestland17T, cultured from a polymicrobial brain abscess, had been found to share less than 99% similarity with the F. nucleatum…

Endoscopic endonasal drainage of a foreign body-associated brain abscess.

BACKGROUND: Although brain abscesses (BA) are uncommon, they represent a significant clinical challenge due to their potential for high morbidity and mortality. When combined with the presence of foreign bodies (FB), such as those resulting from penetrating craniofacial trauma, the management of BA becomes even more complex. Although craniotomy is the most frequent route described for resolution, endoscopic approaches for BA drainage or FB removal have not been widely documented. CASE DESCRIPTION: A 29-year-old female with the antecedent of an assault two months before her referral to our hos…

Comparison of surgical outcomes between primary and secondary brain abscess.

Brain abscess (BA) constitutes 1-8% of intra-cerebral tumors, and thus the present study aimed to compare the surgical outcomes of patients with primary and secondary BA. The present retrospective study examined 32 of cases BA who underwent surgery in a local institution between February, 2013 and December, 2023. All patients received intravenous antibiotic therapy according to the antibiogram for antimicrobial susceptibility. In total, 32 patients were separated into two groups as follows: Group A (16 patients, 50%) with primary abscess and group B (16 patients, 50%) with secondary abscess. O…

Cerebral syphilitic gumma misdiagnosed as brain abscess: A case report.

BACKGROUND: Cerebral syphilitic gumma is a relatively rare clinical disease. Its clinical manifestations are non-specific, and the imaging manifestations are similar to other intracranial occupying lesions, often misdiagnosed as tumors or abscesses. There are few reports on this disease in the relevant literature. To our knowledge, we have reported the first case of cerebral syphilitic gumma misdiagnosed as a brain abscess. We report this case and provide useful information for clinical doctors on neurosyphilis diseases. CASE SUMMARY: We report the case to explore the diagnostic essentials of …

Listeria brain abscess: a therapeutically challenging rare presentation of listeriosis.

We report a very rare case of Listeria multiple brain abscesses manifested as delirium, which represented diagnostic and therapeutic challenges overcome only by the close cooperation between Infectious Diseases and Neuroradiology, without which a satisfactory outcome would not be achieved. An elderly man presented with confusion and drowsiness with a background of type-II diabetes mellitus. Although computed tomography of the brain only showed frontal lobe oedema, contrast magnetic resonance (MR) imaging showed numerous irregular rim-enhancing lesions containing central diffusion restriction, s…

Pyogenic brain abscess associated with an incidental pulmonary arteriovenous malformation.

Pulmonary arteriovenous malformations (PAVMs) are rare and often asymptomatic vascular anomalies that can be associated with serious neurological consequences due to right-to-left shunting. We report a case of a woman in her 80s without substantial medical history who presented with a headache, weakness and personality changes, and was found to have a pyogenic brain abscess requiring emergent neurosurgical evacuation. The abscess grew oral flora, suspected to have reached the brain via an incidentally discovered PAVM. With drainage and antibiotics, the patient achieved a full recovery and the …

Central nervous system manifestation of lupus erythematosus resembling brain abscess.

Manifestations of central nervous system involvement are one of the leading causes of morbidity and mortality in patients suffering from systemic lupus erythematosus. It frequently involves the central nervous system and sometimes need to be differentiated from lesions of infectious etiology, thus representing a major diagnostic dilemma. We present the case of a male adolescent with a known history of idiopathic thrombocytopenic purpura who presented with a seizure ictus and a space-occupying lesion, which posed significant diagnostic challenges to specify its characteristics.

Listeria Monocytogenes Brain Abscess in Crohn's Disease Treated with Adalimumab.

Listeria monocytogenes is a gram-positive bacterium that causes listeriosis. Brain abscess is a very uncommon manifestation of listeriosis and has not been reported to be associated with adalimumab (humira), one of the approved medications for treating Crohn's disease. A 45-year-old female with Crohn's disease presented with sudden onset of fever, headache, nausea, vomiting, and altered mental status for 1 day. She was on prednisone and 6-mercaptopurine. She had started taking adalimumab 17 days prior to admission. She had signs of toxicity, confusion, and nuchal rigidity, but showed neither c…

Novel partial anomalous pulmonary venous connection associated with brain abscess.

BACKGROUND: Right-to-left vascular shunts are associated with brain abscess. CASE REPORT: We present a 47-year-old female with a cryptogenic left thalamic abscess on which Streptococcus mitis grew upon aspiration. Computed tomography of the chest with contrast agent revealed an anomalous connection between the left superior pulmonary and brachiocephalic veins. A right-to-left shunt was confirmed in a transthoracic echocardiogram study in which bubbles were injected into the left arm; this shunt had not previously been noted upon right-arm injection. CONCLUSIONS: We recommend aggressive evalu…

Kocuria varians infection associated with brain abscess: a case report.

BACKGROUND: Kocuria, established by Stackebrandt et al., previously was classified into Micrococcus. Only two species, K. rosea and K. kristinae are reported to be associated as pathogenic and found with catheter-related bacteremia and acute cholecystitis. CASE PRESENTATION: We herein report the first case of brain abscess caused by Kocuria varians, a gram-positive microorganism, in a 52-year-old man. Hematogenous spread is the probable pathogenesis. CONCLUSIONS: This report presents a case of Kocuria varians brain abscess successfully treated with surgical excision combined with antimicrobi…

First successful treatment of Nocardia farcinica brain abscess with moxifloxacin.

Nocardia farcinica is an emerging pathogen in immunosuppressed patients causing both localized and disseminated infections. Many strains of this species are multidrug-resistant and require long-term antibiotic therapy, and consequently the choice of an oral treatment can be problematic. We report here the first case of successful treatment of N. farcinica brain abscess in a renal transplant recipient with an extended-spectrum fluoroquinolone, moxifloxacin.

Tuberculous brain abscess: clinical presentation, pathophysiology and treatment (in children).

BACKGROUND: Tubercular brain abscess (TBA) is a rare manifestation of CNS tuberculosis. It is characterised by an encapsulated collection of pus, containing viable tubercular bacilli without evidence of tubercular granuloma. PRESENTATION AND HISTORY: Patients may present with features of raised intracranial pressure and focal neurological deficit commensurate with the site of the abscess. A history of pulmonary tuberculosis may be present, as documented in one of our six cases; three of our six children developed TBA despite 3-weeks to 12-month courses of antitubercular chemotherapy prescribed…
