Malformations involving the ear may be part of a developmental anomaly of the first and second branchial arch or of a malformation syndrome involving other regions of the skeleton. The anatomy and pathogenesis are discussed, a classification is suggested, and the general management is outlined. A surgical technic for the treatment of Group II (moderately severe) aural atresia is described. Of 21 patients, treated surgically during the past 12 years, 15 patients (71%) gained serviceable hearing. In six patients hearing was not improved.

Treacher Collins syndrome is a congenital craniofacial malformation with autosomal dominant inheritance as the main genetic pattern. In this condition, the biosynthesis of ribosomes in neural crest cells and neuroepithelial cells is blocked and the number of neural crest cells that migrate to the craniofacial region decreases, causing first and second branchial arch dysplasia. Definite causative genes include treacle ribosome biogenesis factor 1 (tcof1), RNA polymerase Ⅰ and Ⅲ subunit C (polr1c), and RNA polymerase Ⅰ and Ⅲ subunit D (polr1d). This paper provides a review of research of three m

Ultrasound has until recently found little application in oral and maxillofacial surgery despite wide use in other specialties. Ultrasound imaging uses the reflection of pulses of high frequency sound to display soft tissues and to differentiate between tissue types, particularly solid and cystic. A case of branchial (lymphoepithelial) cyst is presented to illustrate the use of ultrasound, and the aetiology and histogenesis of this controversial lesion is discussed. Ultrasound as a safe, painless, non-invasive technique is recommended as an additional investigation for neck swellings, particul

Hemifacial microsomia (HFM) is a congenital malformation in which there is hypoplasia of hard and soft tissues on one side of the face. It is considered the second most common congenital syndrome of the head-and-neck region. The hypoplasia manifests in all the structures derived from the first and second branchial arches and therefore, growth and development of these structures is affected in this condition. The present case is a variant of HFM in a 5-year-old Indian male child, which has been diagnosed based on clinical and radiological findings and managed successfully by surgical technique

Hemifacial microsomia is a congenital abnormality of the structures derived from the first and second branchial arches. The severity of this disorder is variable and affects the anatomical landmarks essential for establishing the proper pathway for needle insertion prior to deposition of the local anesthetic solution to attain regional mandibular anesthesia preparatory to dental procedures. This paper describes normal anatomical considerations for regional anesthesia in the mandibular arch, deviations in normal anatomy found in patients with hemifacial microsomia, the rationale for suggesting

Brachial cleft cyst is a congenital anomaly arising from persistent elements of branchial apparatus. It may assume the form of cyst, sinus or fistula and constitutes 30% of all congenital anomalies of the neck region. Brachial cleft cyst is usually recognized in patients before the 5th year of age, but it may appear in patients between 20 and 40, when it enlarges owing to infection. It reveals then as a smooth, resilient tumor in lateral region of the neck. Ultrasonography, computerized tomography and magnetic resonance may be used to evaluate and differentiate these lesions as well as to reve

The development of the head and the face requires an intimate interaction between two mesenchymal populations, a paraxial mesoderm and neural crest cells for the morphogenesis of the musculoskeletal components of the calvaria, the face and the branchial regions. The disruptions in these interactions can cause foetal fatalities or congenital craniofacial anomalies. We are describing a rarest case with a craniofacial malformation, who was born with complete absence of the facial skeleton and the neck structures, a set of well developed ears in their normal locations and eyelids at the junction b…

A 22-year-old woman presented with a lacrimal sac mass which on biopsy was an inverted papilloma. Two years later an ipsilateral cervical mass was removed, which contained inverted papilloma with a histologically typical pattern. Subsequently, the patient had a medial maxillectomy and remains well 5 years later. The cervical mass may represent inverted papilloma arising in a branchial cleft cyst or it may be a metastasis of benign papilloma to a regional lymph node. This is the second such case reported and the first with substantial followup.

BACKGROUND: Successful embryogenesis relies on the coordinated interaction between genes and tissues. The transcription factors Pax9 and Msx1 genetically interact during mouse craniofacial morphogenesis, and mice deficient for either gene display abnormal tooth and palate development. Pax9 is expressed specifically in the pharyngeal endoderm at mid-embryogenesis, and mice deficient for Pax9 on a C57Bl/6 genetic background also have cardiovascular defects affecting the outflow tract and aortic arch arteries giving double-outlet right ventricle, absent common carotid arteries and interruption of

Objective: To investigate the feasibility and efficacy of mini-incision with endoscope-assisted resection for bilateral congenital second branchial cleft fistula, and to report on a rare pedigree. Methods: The clinical data of 5 patients with bilateral congenital second branchial fistula admitted in Fujian Provincial Hospital from April 2007 to December 2018 were retrospectively reviewed, including 2 males and 3 females, aged from 3 to 31 years old. The surgical strateges and clinical experience of single mini-incision with endoscope-assisted fistulectomy were summarized, and a rare pedigree w

Fourth branchial cleft anomalies are rare lesions that present diagnostic dilemmas to otolaryngologists. The report presented is a case of a 17-year-old female with food matter and abscess within the thyroid gland that were associated with a 4th branchial cleft sinus. A thyroid ultrasound revealed a 3.0 x 1.5 x 2.5-cm lesion with abnormal echogenicity that was concerning for early abscess. The patient subsequently underwent direct laryngoscopy and right thyroid lobectomy. Her symptoms resolved after surgery. This case demonstrates an unusual presentation in which food passed through the tract

The great vessels of the heart originate from the pharyngeal arch arteries (PAAs). Anomalies of the PAAs often occur together with pharyngeal pouch malformations, but the reasons for this phenomenon are not fully understood. In the current study, we show that platelet-derived growth factor (PDGF) signaling derived from the pharyngeal pouches plays an important function in PAA vasculogenesis. During PAA development in zebrafish embryos, pdgfαa and pdgfαb are expressed in the developing pharyngeal pouches. Results from loss-of-function experiments revealed a critical role of these genes in PAA f

Branchial cleft anomalies are the second most common head and neck congenital lesions in children. It may sometimes be a part of branchio-oto-renal (BOR) syndrome, so in patients with branchial cleft anomalies associated with a complaint of auricular deformity or a similar history and findings in other family members, we should take an additional examination to find the possibility of BOR syndrome. Complete excision is essential for good prognosis. For the management of branchial cleft anomalies, various methods have been reported. Endoscopically assisted dissection technique and transoral rob

Patterning of the facial skeleton involves the precise deployment of thousands of genes in distinct regions of the pharyngeal arches. Despite the significance for craniofacial development, how genetic programs drive this regionalization remains incompletely understood. Here we use combinatorial labeling of zebrafish cranial neural crest-derived cells (CNCCs) to define global gene expression along the dorsoventral axis of the developing arches. Intersection of region-specific transcriptomes with expression changes in response to signaling perturbations demonstrates complex roles for Endothelin

Head and neck anatomy is topographically complex and the region is densely populated by vital nerves and vascular and lymphatic structures. Injury to many of these structures is associated with significant morbidity and may even be fatal. A thorough knowledge of regional anatomy is imperative and complications need to be managed in a thoughtful directed manner. The pediatric surgeon may be called upon to address both congenital and acquired conditions and should be prepared to encounter reoperative fields after failed initial surgery. This review summarizes the current literature on four frequ

OBJECTIVES: Hairy polyps are rare, congenital malformations of the oropharynx and nasopharynx. To date, approximately 145 cases have been reported. However, the histogenesis of these lesions remains unclear.
CASE REPORT: We report the case of a 2-day-old neonate presenting with a hairy polyp attached to the left palate, who re-presented aged 16 months with a discharging first branchial arch sinus.
CONCLUSION: We propose this case as supporting evidence for the theory that hairy polyps are a malformation of the first branchial arch system.

The range of pathology seen in the head and neck region is truly amazing and to a large extent probably mirrors the complex signaling pathways and careful orchestration of events that occurs between the primordial germ layers during the development of this region. As is true in general for the entire discipline of pediatric pathology, the head and neck pathology within this age group is as diverse and different as its adult counterpart. Cases that come across the pediatric head and neck surgical pathology bench are more heavily weighted toward developmental and congenital lesions such as branc
