Sonographic definition of the fetal situs.

The sonographic definition of the fetal situs is of diagnostic importance. However, the current methods that rely on the position of both heart and stomach are not always reliable. Presented are the right-hand rule for transabdominal scanning and the left-hand rule for transvaginal scanning which enable the definition of the fetal situs. The presented rules of thumb enable a definite diagnosis of the fetal situs in all cases and apply for all fetal positions and presentations.

[Premature detachment of an at the uterine septum implanted placenta. Case report].

In the context of a report on a case in which the placenta was implanted at the uterine septum, the diagnosis, course of pregnancy, and obstetric management are described and discussed. In the case reported here this extremely rare nidation site led to premature detachment of the placenta during the last expulsive pain in a delivery with a vaginal breech presentation (43-year-old patient with four previous unproblematic term births).

[Hydramnios as evidence of Steinert's congenital myotonic distrophy (author's transl)].

The authors describe a case of Steinert's disease. It was tracked down because of three anomalies of pregnancy: a lessening in active movements of the fetus, breech presentation and hydramnios, and it was diagnosed by neurological and electromyography of the mother. The existence of hydramnios should make it possible to avoid maternal trauma during the delivery, knowing that the child will be born with a very severe form of the illness, which is likely to be lethal. Those few cases that survive are severely mentally handicapped.

[Ectopic pregnancy carried to full term with a living infant].

A section delivery was performed for transverse lie on a fourth para of 38 years at the beginning of labour on term. Thereby a full term extrauterine gravidity was found. According to the placental insertion in the ampullar part of the tube, on the ovary and upon the visceral peritoneum it can be classified as tubo-abdominal pregnancy which probably had developed after rupture of a tubal pregnancy which initially was established in the ampullar part. Blood supply was accomplished mainly by the hypertrophied ovarian vessels of the right side. After removal of the amniotic sac and partial resect

Delivery outcomes in subsequent pregnancy following primary breech cesarean delivery: a retrospective cohort study.

Background: Women may be unaware of the potential impact on subsequent deliveries of the decision to undergo primary breech cesarean (PBC) delivery rather than attempt external cephalic version (ECV). We investigated cesarean delivery rates and adverse maternal outcomes in the subsequent delivery following management of breech presentation by ECV versus PBC.Methods: This retrospective study identified women who delivered subsequent to breech presentation managed by attempted ECV or PBC. We assessed cesarean delivery rates and adverse maternal outcomes including hemorrhage, placental complicati…

Do the risk factors for developmental dysplasia of the hip differ according to gender? A look from another perspective.

AIM: The aim of the present study was to examine if there were gender differences in risk factors in 4415 infants who were evaluated through … delivery type, swaddling, multiple pregnancies, duration of gestation period, gravida, birthweight, breech presentation, oligohydramnios and family history) were obtained from the families. Any accompanying musculoskeletal (torticollis, pes equinovarus, congenital knee dislocation) abnormality was recorded. RESULTS: Pathological hip was determined in 149 (3.37%) of …

The main indications for transfusion were placental disorders, pre-eclampsia and breech presentation. The packed cell volume, status of the anaesthetist and the ASA grading were other factors, which had statistical association with blood transfusion. The effect of the Term Breech Trial on medical intervention behaviour and neonatal outcome in The Netherlands: an analysis of 35,453 term breech infants.

OBJECTIVE: To examine the effects of the Term Breech Trial on the medical behaviour of Dutch obstetricians and on neonatal outcomes. DESIGN: Retrospective observational study. SETTING: The Netherlands. POPULATION: Infants born at term in breech presentation in the Netherlands between 1998 and 2002, with birthweights < or =4000 g (n= 33,024) and >4000 g (n= 2429), respectively. Multiple pregnancies, antenatal death and major congenital malformations were excluded. Perspectives on risk: Assessment of risk profiles and outcomes among women planning community birth in the United States.

BACKGROUND: There is little agreement on who is a good candidate for community (home or birth center) birth in the United States. METHODS: Data on n=47 394 midwife-attended, planned community births come from the Midwives Alliance of North America Statistics Project. Logistic regression quantified the independent contribution of 10 risk factors to maternal and neonatal outcomes. Risk factors included: primiparity, advanced maternal age, obesity, gestational diabetes, preeclampsia, postterm pregnancy, twins, breech presentation, history of cesarean and vaginal birth, and history of cesarean wi…

Pregnancy Outcomes in Three Distinct Uterine Anomalies: A Case Series.

This case series presents three patients with distinct Müllerian anomalies, highlighting variability in diagnosis, clinical management, and obstetric outcomes. Case 1 involved uterine didelphys with pregnancy in the right hemi-uterus, resulting in non-reassuring fetal heart rate tracings, prompting repeat cesarean delivery at 37 weeks' and six days' gestation. Case 2 involved a unicornuate uterus with a noncommunicating rudimentary horn and persistent breech presentation, requiring a scheduled cesarean delivery at 38 weeks' gestation. Case 3 involved a partial septate uterus associated with br…

The prognosis of breech delivered singleton low birth weight infants.

An estimate of the probability of long term neurological sequelae in breech delivered low birth weight infants is an important factor in deciding on the method of delivery in breech presentation. From the hospital records we have identified those infants who during the years 1973 and 1974 were live born of low birth weight (under 2500 g) and who were delivered by the breech. Of 51 infants, 36 survived the neonatal period. Two have died, one of multiple congenital abnormalities and one of accidental, two left the country during their second of life at which time they were known to be developing…

Patient satisfaction with childbirth after external cephalic version.

PURPOSE: To assess acceptance and impact of external cephalic version (ECV) for breech presentation at term on maternal satisfaction with childbirth. METHODS: Retrospective study on n = 131 women with breech presentation comparing maternal satisfaction after ECV and consecutive childbirth (n = 66; 50.4% of these successful attempts in n = 33; 50%) against the group without ECV and primary caesarean section (CS) (n = 65; 49.6%) instead using a questionnaire. Associated risk factors in children who had late presentation of developmental dysplasia of the hip.

PURPOSE: The purpose of this study was to assess the role of clinical examination, associated risk factors and plain radiograph of the pelvis in children who had late presentation of DDH. METHODS: We report on a 7-year prospective study, in children who had late presentation of developmental dysplasia of the hip (DDH). For this purpose, 740 hips in 370 referred children, age range 3-7 months (mean 3.44 months) were clinically and radiologically assessed, and the associated risk factors recorded. RESULTS: Female sex, first born, positive family history and breech presentation were confirmed a…

OBJECTIVE: The objective of this study was to report a multicentric study of nine cases of children presenting with a birth-related spinal injury. MATERIALS AND METHODS: The medical charts of nine patients identified by a questionnaire sent to the members of the French Society of Paediatric Orthopaedics (SOFOP) were reviewed. CONCLUSIONS: The pregnancy was uneventful in all cases. The fetal presentation was cephalic in three cases, a breech presentation in four cases, a face presentation in two cases. The lesion involved the cervical spine in six cases. Three patients presenting upper cervic…

Management of the hyperextended fetal head.

Hyperextension of the fetal head in utero is readily detectable by roentgenography and ultrasound scan. Management is determined by presentation of the fetus. There are three types of presentations: face presentation, star-gazing breech presentation, and the flying fetus presentation. Undiagnosed hyperextension of the fetal head is frequently associated with cervical cord injury.
