Variations of care with bronchiolitis associated apneas in infants: A retrospective study.

BACKGROUND: Bronchiolitis-associated apneas are a life-threatening complication that are observed more frequently in younger infants born premature. Although apneas associated with viral bronchiolitis are relatively common, at present there is no consensus on management. METHODS: This study was a retrospective chart review of viral bronchiolitis in children less than 2 years old, admitted to the pediatric intensive care unit (PICU) between July 1st 2020 and June 30th 2024, with at least one reported pre-hospital apnea. Electronic chart reviews were completed and documented demographics, pre-P

Mucosal IL-36 is a defining feature of severe paediatric bronchiolitis.

RATIONALE: Bronchiolitis is the commonest cause of hospital admission in children under the age of 1 year, most cases being due to respiratory syncytial virus (RSV) infection. The mechanisms causing infantile bronchiolitis are incompletely understood but include a deficient mucosal interferon response, neutrophilic inflammation and enhanced mucosal Type-2 responses. OBJECTIVES: We sought to determine the mucosal immune processes associated with severe paediatric bronchiolitis. METHODS: We performed transcriptomic analyses on mucosal samples from infants hospitalized with Moderate (n = 48) an

Foreign Body Aspiration Presenting as Recurrent Bronchiolitis: Case Report.

Foreign body aspiration (FBA) is a well-recognized pediatric emergency that is frequently misdiagnosed due to symptom overlap with common respiratory conditions such as bronchiolitis, asthma and pneumonia. This case report describes a toddler with recurrent wheezing and cough that persisted despite standard medical therapy. The child was evaluated at a pediatric urgent care center in which history, physical exam findings and imaging raised concern for airway obstruction. The child was transferred to a pediatric emergency department for suspected FBA prompting bronchoscopy evaluation, with conf

Effects of Nutritional Status on Treatment Outcomes and Prognosis of Infants With Bronchiolitis.

To explore how nutritional status impacts treatment efficacy and future asthma risk in infants with bronchiolitis. A retrospective analysis of 473 RSV-induced bronchiolitis cases from 2015 to 2018. No significant demographic differences. Obese infants showed increased respiratory distress, wheezing, and rapid breathing, with lower oxygen saturation. Elevated IgE and tumor necrosis factor-α (TNF-α) levels persisted in the obese group. A higher asthma incidence was observed after 5 years in this group. Nutritional status is crucial for bronchiolitis management; obesity prevention should be highl

Neutrophilic and eosinophilic inflammation in bronchiolitis and long-term respiratory sequelae.

INTRODUCTION: Risk factors for bronchiolitis and recurrent wheezing episodes remain unclear. Neutrophil or eosinophil counts at the time of hospitalization for bronchiolitis may play an important role. OBJECTIVE: To study the correlation between blood neutrophil and eosinophil levels in patients with bronchiolitis, recurrent wheezing episodes and lung function. MATERIALS: We followed 645 children hospitalized for bronchiolitis during nine consecutive seasons for six years. Clinical and laboratory information was obtained from medical records and a standardized questionnaire was used to colle

Bronchiolitis after Combination Immunotherapy With Ipilimumab and Nivolumab in a Melanoma Patient.

Therapy with immune checkpoint inhibitors (ICIs) has significantly improved the prognosis of metastatic melanoma but is also associated with various immune-related adverse events (AE), including pulmonary toxicity. Herein, we describe the case of a 60-year-old female with metastasized melanoma with BRAF mutation under combination immunotherapy with ipilimumab and nivolumab, who presented with a persistent, nonproductive cough for the last two months. Her CT-scan showed de novo bronchial inflammation and wall thickening in all lung fields. Initial treatment with antimicrobial treatment and inha

Asthmalitis? Diagnostic Variability of Asthma and Bronchiolitis in Children <24 Months.

BACKGROUND AND OBJECTIVES: Bronchiolitis and asthma have similar acute clinical presentations in young children yet have opposing treatment recommendations. We aimed to assess the role of age and other factors in the diagnosis of bronchiolitis and asthma in children <24 months of age. METHODS: We conducted a retrospective cross-sectional analysis of the Pediatric Health Information System database. We included children aged <2 years diagnosed with bronchiolitis, asthma, wheeze, or bronchospasm in emergency department or hospital encounters from 2017 to 2021. We described variation by age and

Acute viral bronchiolitis phenotype in response to glucocorticoid and bronchodilator treatment.

OBJECTIVE: To analyze whether infants admitted to hospital with Acute Viral Bronchiolitis (AVB), who received glucocorticoids and bronchodilators, and who had an atopic phenotype, spent less time in hospital and/or less time on oxygen therapy when compared to those who did not have the phenotype. METHOD: A cross-sectional, retrospective epidemiological study was developed with data from medical records of infants admitted to hospital due to AVB from 2012 to 2019 in a sentinel public hospital. It was verified that the frequency of prescription of glucocorticoids, bronchodilators and antibiotic

The implications of airway anomalies on children with congenital heart disease and bronchiolitis.

BACKGROUND: Bronchiolitis is a common airway infection in young children. Hemodynamically significant congenital heart disease (CHD) predicts a more complicated course. However, the role of airway anomalies remains unknown. METHODS: We retrospectively reviewed the records of patients under 2 years old, diagnosed with CHD, and admitted between January 2011 and December 2013, before the palivizumab era. Records of bronchiolitis admissions were also extracted. Patients were grouped according to CHD condition and airway anomalies. RESULTS: A total of 230 patients with CHD were enrolled. A total

Severe bronchiolitis profiling as the first step towards prevention of asthma.

Bronchiolitis is the most common respiratory infection leading to hospitalization and constitutes a significant healthcare burden. The two main viral agents causing bronchiolitis, respiratory syncytial virus (RSV) and rhinovirus (RV), have distinct cytopathic, immune response, and clinical characteristics. Different approaches have been suggested for subtyping bronchiolitis based on viral etiology, atopic status, transcriptome profiles in blood, airway metabolome, lipidomic data, and airway microbiota. The highest risk of asthma at school age has been in a subgroup of bronchiolitis characteriz

Application of Asthma Prediction Tools in a Cohort of Infants with Severe Bronchiolitis.

Background: Severe bronchiolitis is a strong childhood asthma risk factor. Early and accurate asthma prediction is key. We applied the Asthma Predictive Index (API), the modified Asthma Predictive Index (mAPI), and the Pediatric Asthma Risk Score (PARS) in a cohort of high-risk infants to predict asthma at age 6 years. Methods: We conducted a 17-center cohort of infants (age <1 year) hospitalized with severe bronchiolitis during 2011-2014. We used only infancy data to predict asthma at age 6 years. Results: The prevalence of parent-reported asthma at age 6 years was 328/880 (37%). The prevalen

Reducing Overutilization of High-flow Nasal Cannula in Children with Bronchiolitis.

BACKGROUND: Bronchiolitis is a leading cause of pediatric hospitalizations. A high-flow nasal cannula (HFNC) does not significantly improve clinical outcomes and is associated with increased costs and intensive care unit (ICU) utilization. Despite this, hospitals continue to overuse HFNC in children with bronchiolitis. We aimed to reduce HFNC initiation in children hospitalized with bronchiolitis by 20 percentage points within 6 months. METHODS: This study included patients aged 1 month to 2 years diagnosed with bronchiolitis, excluding patients with prematurity less than 32 weeks or preexist

Monocyte and neutrophil to lymphocyte ratios in hospitalized children with RSV bronchiolitis.

OBJECTIVE: Acute bronchiolitis is the most common cause of hospitalization in young children. Data on monocyte-to-lymphocyte-ratio (MLR) and neutrophil-to-lymphocyte-ratio (NLR) as biomarkers are limited. We aim to evaluate these ratios in children hospitalized with respiratory syncytial virus (RSV) bronchiolitis and their value as biomarkers for severe clinical outcomes. STUDY DESIGN: A single-center retrospective cohort study of children aged <2 years hospitalized due to RSV bronchiolitis, between January 2018 and March 2022, with a complete blood count upon admission. We divided the cohort

Epigenetic regulation of inflammation by microRNAs in post-infectious bronchiolitis obliterans.

OBJECTIVES: Post-infectious bronchiolitis obliterans (PiBO) is a rare, chronic disease initiated by severe infection and followed by perpetuating inflammation and obliteration of the small airways. MicroRNAs (miRNAs) have been proposed to play a central role as epigenetic regulators, which control resolution and prevent the uncontrolled progress of inflammation. The aim of this study was to define biomarkers on the level of post-transcriptional gene regulation in order to characterise PiBO. METHODS: A total of 39 patients with well-defined PiBO and 31 controls from two centres, Barcelona, Spa

TGFβ1 and POSTN as biomarkers of postinfectious bronchiolitis obliterans and asthma in children.

RATIONALE: Asthma and postinfectious bronchiolitis obliterans (PIBO) are common chronic lung diseases in company with wheezing in children. However, it is not clear what is common and unique mechanisms between the two diseases. Thus, we used proteomic analysis to compare differences in biomarkers between children with asthma and PIBO. METHODS: Overall, 30 healthy children without respiratory underlying diseases, 18 children with asthma and 15 with PIBO were included for this study. Sequential window acquisition of all theoretical mass spectra (SWATH)-mass spectrometry (MS) was used to measure
