The role of annexin A1 in Candida albicans and Candida auris infections in murine neutrophils.

Annexin A1 (AnxA1) is an anti-inflammatory protein expressed in various cell types, especially macrophages and neutrophils. Because neutrophils play important roles in infections and inflammatory processes and the relationship between AnxA1 and Candida spp. infections is not well-understood, our study examined whether AnxA1 can serve as a target protein for the regulation of the immune response during fungal infections. C57BL/6 wild-type (WT) and AnxA1 knockout (AnxA1-/-) peritoneal neutrophils were coinfected with Candida albicans or Candida auris for 4 h. AnxA1-/- neutrophils exhibited a mar

Tracing the Origin of Invasive Fungal Infections.

Invasive fungal infections are a major cause of mortality in immunocompromised patients. By using high-resolution sequencing, Zhai et al. provide insight into translocation of Candida strains from the gut mycobiota to the bloodstream of transplanted patients. Microbiota-driven diagnostic methods could rapidly emerge for preventing deadly fungal infections.

Closing the Gap: Advances and Barriers on Candida Vaccines Development in the Last Decade.

Invasive Candidiasis (IC) remains a critical threat to global health. Primarily caused by Candida albicans, it is increasingly driven by emerging Non-Candida albicans Candida Species (NCACs). IC typically arises from the overgrowth of commensal Candida species in immunocompromised hosts. IC accounts for an estimated 250,000-700,000 cases annually worldwide, with mortality rates between 40 and 55%, and contributes to more than 1.5 million annual deaths. Despite progress in healthcare systems, intensive care, and microbiological techniques, mortality rates remain largely unchanged. Triazole resi

Candidiasis of the Central Nervous System in Neonates and Children with Primary Immunodeficiencies.

PURPOSE OF REVIEW: Candida infections of the central nervous system (CNS) are a life-threatening complication of invasive infections that most often affect vulnerable groups of patients, including neonates and children with primary immunodeficiency disorders (PID). Here, we review the currently known risk factors for CNS candidiasis, focusing predominantly on the PID caused by biallelic mutations in CARD9. RECENT FINDINGS: How the CNS is protected itself against fungal invasion is poorly understood. CARD9 promotes neutrophil recruitment and function, and is the only molecule shown to be criti

The role of the gastrointestinal tract in hematogenous candidiasis: from the laboratory to the bedside.

The gastrointestinal (GI) tract is a frequent source of hematogenous candidiasis in humans. Animal models of GI and hematogenous candidiasis have provided insights into the nature of candidal infection of host mucosal tissue, mechanisms of fungal dissemination to body organs, and features of host response to candidal infections. Biological systems such as these that simulate human candidiasis can be used for testing novel antifungal drugs. We have focused on two murine models of candidiasis with similarities to this fungal disease in humans. The first model simulates a commensal association of

Review on the management of intra-abdominal candidiasis.

Intra-abdominal candidiasis, which includes Candida peritonitis and Candida produced intra-abdominal abscesses, accounts for 10-30% of all intra-abdominal infections diagnosed in the intensive care units. Intra-abdominal candidiasis is associated with longer hospital stay, and significantly higher morbidity and mortality. Although the management of invasive candidiasis has greatly improved in these past years, the optimal management of intra-abdominal candidiasis remains elusive. Questions concerning the microbiological diagnosis, optimal antifungal drugs doses, diffusion through peritoneal fl

Primary deep cutaneous candidiasis caused by Candida duobushaemulonii in a 68-year-old man: the first case report and literature review.

Superficial candida infections of the skin are common, but deep cutaneous candidiasis, including secondary dissemination to the skin from systemic candidiasis, candidaemia or primary invasion due to skin defects such as trauma, is rare. These patients are usually immunosuppressed, but immunocompetent hosts can be affected as well. Candida albicans is the most common pathogen. However, non-albicans Candida species can cause deep skin invasion in rare circumstances. We report a case of deep cutaneous candidiasis caused by Candida duobushaemulonii in a 68-year-old man. Deep tissue invasion was co

Invasive Fungal Infections after Liver Transplantation.

Invasive fungal infections represent a major challenge in patients who underwent organ transplantation. Overall, the most common fungal infections in these patients are candidiasis, followed by aspergillosis and cryptococcosis, except in lung transplant recipients, where aspergillosis is most common. Several risk factors have been identified, which increase the likelihood of an invasive fungal infection developing after transplantation. Liver transplant recipients constitute a high-risk category for invasive candidiasis and aspergillosis, and therefore targeted prophylaxis is favored in this p

Outcomes Following Antifungal Treatment for Candida Growth in Bile Cultures Collected During Endoscopic Retrograde Cholangiopancreatography.

Candida species are frequently detected in bile cultures during endoscopic retrograde cholangiopancreatography (ERCP), but their clinical significance and the value of antifungal treatment remain unclear. We performed a retrospective single-center cohort study of adults with growth of Candida species from bile cultures collected by ERCP performed between 2010 and 2023. We compared inpatients who received vs. those who did not receive antifungals within one week of ERCP and a subgroup with acute cholangitis. The primary outcome was a composite of death and invasive candidiasis within one year.

Fungal sepsis in the ICU: are we doing better? Trends in incidence, diagnosis, and outcome.

Despite the availability of broad-spectrum antifungal agents, fungal sepsis remains an issue in Intensive Care Unit (ICU) patients. In terms of occurrence rates, the most important fungal infections are invasive candidiasis and invasive pulmonary aspergillosis. Early diagnosis is essential to optimize the chances of survival. As clinical rules based on risk factor assessment lack specificity, early initiation of antifungal therapy depends on fast and reliable diagnostics. Blood cultures or histopathologic evidence is the gold standard but blood cultures lack sensitivity and biopsy sampling may

Rapid screening of invasive fungal infections in the hospital setting using the (1,3)-β-D-glucan assay.

Disseminated candidiasis and other invasive fungal infections are a substantial cause of morbidity and mortality in immunocompromised and critically ill patients. Diagnosis of disseminated candidiasis via blood culture and skin biopsy can be unreliable and may delay treatment. (1,3)-β-D-glucan (BDG) assay is a rapid, cost-effective, noninvasive diagnostic screening tool for the dermatology hospitalist to consider.

CARD9 Mutations in Patients with Invasive Fungal Infections.

BACKGROUND: Homozygous mutations in the Caspase-associated recruitment domain 9 (CARD9) gene increase susceptibility to invasive fungal infections, particularly those caused by Candida species. This study aims to assess the spectrum of CARD9 gene mutations that predispose individuals in the Turkish population, especially children, to invasive fungal infections. METHODS: Our study included 30 patients who were admitted to Ege University due to invasive fungal infection or chronic mucocutaneous candidiasis between 2020 and 2023. Demographic and clinical data of the patients were recorded, and a

Postmortem candidaemia: marker of disseminated disease.

AIM: The significance of finding Candida species in heart blood cultures obtained at postmortem examination has never been studied. This article describes the findings of autopsy patients with postmortem candidaemia and it compares them with findings in autopsy patients with antemortem candidaemia. METHOD: 23 patients with Candida species isolated from heart blood at autopsy were identified over a 10-year period. These patients were compared with 10 autopsy patients found during the same time period with antemortem blood cultures isolating Candida species, but not positive postmortem heart bl

Experimental antifungal chemotherapy in granulocytopenic animal models of disseminated candidiasis: approaches to understanding investigational antifungal compounds for patients with neoplastic diseases.

Disseminated candidiasis is the most common life-threatening invasive fungal infection in granulocytopenic patients. A review of recent approaches to pre-clinical laboratory investigation of promising antifungal compounds, which may have potential utility in granulocytopenic patients is presented. A particularly useful strategy is the study of persistently granulocytopenic rabbit models of acute, subacute, and chronic forms of disseminated candidiasis. When the antifungal triazoles (fluconazole, itraconazole, and SCH 39304 [SCH 42427]) were each evaluated for use as preventive, early treatment

Epidemiology and Mortality-Associated Factors of Invasive Fungal Disease in Elderly Patients: A 20-Year Retrospective Study from Southern China.

INTRODUCTION: Invasive fungal disease (IFD) is a life-threatening infection. The epidemiology and clinical features of IFD in the elderly population are less discussed. The aim of this study was to explore the epidemiology and mortality-associated factors for IFD in the elderly inpatients. METHODS: A retrospective study enrolling 512 elderly inpatients from The First Affiliated Hospital of Sun Yat-sen University during the last two decades was performed. RESULTS: The annual prevalence of IFD was 0.1-0.5%. Candidiasis was the most common (236/521, 45.3%). An increasing trend was observed in a

Esophageal candidiasis. Managing an increasingly prevalent infection.

Esophageal candidiasis is an opportunistic infection that is being recognized increasingly often in certain patients, including those who have a neoplastic disease, are undergoing protracted antibiotic therapy, or hae acquired immunodeficiency syndrome (AIDS). Impaired cell-mediated immunity may predispose the patient to esophageal mucosal colonization, whereas chemotherapy-induced granulocytopenia may predispose to disseminated candidiasis. Esophageal candidiasis should be suspected in susceptible patients with complaints of substernal odynophagia or dysphagia. The diagnosis is confirmed by e

Immunization protocols for use in animal models of candidiasis.

Immunoprotection during most forms of candidiasis (oropharyngeal, invasive) is lacking since most candidiasis patients are immunosuppressed either as a result of their allogeneic transplant, cancer chemotherapy, or HIV infection. Consequently, immunization might be considered as an unlikely way to protect patients from such infection. Nonetheless, there are a number of investigations that indicate active immunization or the passive treatment with hyperimmune, specific antibodies can result in protection in models of experimental candidiasis. The former subject, active immunization, is the subj

Hepatic Candidiasis in a Non-neutropenic Patient.

Hepatic candidiasis is an uncommon but serious manifestation of invasive candidiasis (IC), most often associated with prolonged neutropenia in patients with hematological malignancies or in those who have undergone abdominal surgery. Diagnosis is challenging because of the low sensitivity of cultures, the limitations of serologic assays, and the frequent impracticality of tissue biopsy. Imaging, particularly CT and [18F]fluoro-2-deoxy-D-glucose positron emission tomography (18FDG-PET), plays a central role in identifying characteristic hepatosplenic lesions and in monitoring response to therap
