Leg amputation and dystrophic epidermolysis bullosa: A case report with 15 years of follow-up.

OBJECTIVE: Dystrophic epidermolysis bullosa is a rare disease characterized by widespread blistering of the skin and mucous membranes, which may ultimately prompt limb amputation. In this context, the outcome of fitting a prosthesis to a chronically wounded stump is not well known. Our patient's experience (with 15 years of follow-up) should contribute to better knowledge of this topic. CASE REPORT: A 37-year-old man presented with severe dystrophic epidermolysis bullosa. Recurrent skin carcinoma had led to an amputation below the knee. Despite incessant development of blisters on the stump a

[Does sugar play a role in the development of gastroenterologic diseases (Crohn disease, gallstones, cancer)?].

The consumption of cane- and beet-sugars per person has today reached a level of 100 g per day. In a discussion of the role of sugar in gastrointestinal diseases there must be a differentiation between a direct effect directly related to high sugar-consumption, and an indirect effect where sugar displaces other nutritive substances in the diet. Not only epidemiological, but also experimental data support that sugar has no direct, toxicological, carcinogenic or disease-inducing disposition for gastroenterological diseases. Investigating several diseases (e.g. M. Crohn) a final determination cou

Flexible intramedullary nails for ipsilateral femoral and tibial fractures.

Between June 1981 and August 1983, six patients were treated with flexible intramedullary nails for both femoral and tibial fractures in six floating knees. All six patients were males averaging 26 years of age and all six were involved in motor vehicle accidents. Concomitant associated injuries were common. Two femoral and five tibial fractures were open. All fractures were stabilized within 24 hours of injury by closed intramedullary nailing with Ender nails. At final followup, there was one femoral and one tibial nonunion in the same patient. For the remaining patients, femoral union averag

Gait training of poststroke patients assisted by the Walkaround (body postural support).

Improvement in gait abilities is one of the important goals of stroke rehabilitation. The Walkaround is a new postural assistance device for gait training, which allows an early start for gait training. This device provides body postural support (BPS) and trunk orientation by means of a lumbar belt that is connected to a powered rolling walker. We conducted a randomized, single-blinded, 4-week clinical trial of 22 subacute stroke patients with a follow-up period of 6 months. Patients were divided into two identically sized groups: the treatment group (BPS), which was assisted by the Walkaround

Design and Implementation of a Walking Stick Aid for Visually Challenged People.

Visually challenged people (VCPs) face many difficulties in their routine life. Usually, in many cases, they need to depend upon others, which makes them unconfident in an unfamiliar environment. Thus, in this paper, we present an aid that helps in detecting obstacles and water puddles in their way. This system comprises a walking stick and Android-based applications (APPs). The walking stick is embedded with Raspberry Pi and programmable interface controller (PIC) as a control kernel, sensors, a global position system (GPS) module, and alert-providing components. Sensors help to detect obstac

A triage strategy based on clinical risk factors for selecting elderly women for treatment or bone densitometry: the EPIDOS prospective study.

A triage strategy, based on a clinical hip fracture risk score, may be used to classify elderly women into three groups: one at high risk and requiring treatment, another needing further assessment by bone densitometry, and a third at low risk. We used prospective data from the EPIDOS study (7512 women older than 75 years and followed for an average of 3.9 years) to assess the potential value of such a strategy for identifying elderly women with a hip fracture risk twice the cohort average (i.e. > or =20 per 1000 woman-years). An individual fracture risk score was calculated with the final ris

A Rare Cause of Dysphagia by Extrinsic Compression.

With aging, compensatory mechanisms and physiological reserve may become insufficient to maintain balance and gait (BG), particularly when associated with stroke, Alzheimer's disease, diabetes, osteoarticular diease, vestibular disorders, orthostatic hypotension (OH), heart rhythm disorders, or drug side effects. This leads to poorer postural-motor function and increased risk of falling (RoF). This review aims to highlight recent scientific advancements relative to BG disorders (BGDs) for gerontology professionals. When assessing older adults (OAs) with BGDs, a thorough assessment of patient h

[Comparison between the short program and the long program of post-operative rehabilitation of hip fracture for making the critical path].

Patients receiving operative treatment for fracture are good candidate for critical path management. Recently, we have developed a Short Program (SP) for post-operative rehabilitation of hip fractures. The purpose of this study was to evaluate the effectiveness, the safety and the cost efficiency of this SP, by comparing it with the former Long Program (LP) in terms of choosing the better program for the critical path. The enrolled patients were over 65 years old, had been able to walk by themselves with or without canes, were free from neurological diseases like hemiplegia and Parkinson's dis

Direct biomechanical manipulation of human gait stability: A systematic review.

People fall more often when their gait stability is reduced. Gait stability can be directly manipulated by exerting forces or moments onto a person, ranging from simple walking sticks to complex wearable robotics. A systematic review of the literature was performed to determine: What is the level of evidence for different types of mechanical manipulations on improving gait stability? The study was registered at PROSPERO (CRD42020180631). Databases Embase, Medline All, Web of Science Core Collection, Cochrane Central Register of Controlled Trials, and Google Scholar were searched. Effect of Passenger Encumbrance and Mobility Aid Use on Dwell Time Variability in Low-floor Transit Vehicles.

Public transit serves users with a broad range of physical capabilities and design needs. However information about the operational effects of diverse users interacting with the transit system is scarce. This paper examined the occurrence and effects of boarding and alighting passengers with mobility aids (wheelchairs, scooters, walkers and canes), or with large items (carts, strollers, bicycles, or carrying an infant) on bus stop dwell time in a fixed-route bus service. On-board video data from low-floor public transit buses serving Ann Arbor, Michigan were used from 199 bus stops with at lea

Visual profile of students in integrated schools in Malawi.

BACKGROUND: Blindness and visual impairment are very common in African countries and are often loosely linked to inadequate resources. We designed this study to assess clinical visual and ocular characteristics of children in three integrated schools in Malawi, so that students needing low vision services or those with correctable refractive error will be identified. METHODS: We included 95 students, who underwent a detailed optometric examination. The assessment included distance visual acuity measurement in logMAR notation, near visual acuity, oculo-motor assessment, pupillary assessment an

Medical treatment of osteonecrosis of the knee associated with thrombophilia-hypofibrinolysis.

In 6 patients with stage II knee osteonecrosis, all 6 with thrombophilia and 4 with concurrent hypofibrinolysis, the authors prospectively determined whether anticoagulation with enoxaparin could prevent collapse and progression to osteoarthritis, ameliorate pain, and restore function. The 6 patients were treated with enoxaparin (40 to 60 mg/d for 3 or more months) as mandated by a US Food and Drug Administration-approved protocol. In post-enoxaparin prospective follow-up, patients were reassessed clinically every 4 to 6 months, and radiographs were obtained every year. The 6 patients followed

Ultrasonographic findings of multicentric malignant lymphoma involving the urinary bladder in a dog: Diagnosis and monitoring during chemotherapy.

A 5.5-year-old spayed female cane corso dog was presented for recurrent hematuria and dysuria. Abdominal ultrasound demonstrated severe hypoechoic urinary bladder wall thickening with irregular mucosal surface and polypoid-like lesions protruding into the lumen, abdominal polyadenomegaly, and splenic honeycombing. High-grade lymphoma was diagnosed on urinary bladder wall cytology. Clinical signs and ultrasonographic findings initially improved following a modified CHOP-based chemotherapy protocol, but the multicentric lymphoma then progressed with thoracic spread, leading to the dog's death at

Evolutionary simulations of Z-linked suppression gene drives.

Synthetic gene drives may soon be used to suppress or eliminate populations of disease vectors, pathogens, invasive species, and agricultural pests. Recent proposals have focused on using Z-linked gene drives to control species with ZW sex determination, which include Lepidopteran pests, parasitic trematodes, and cane toads. These proposals include Z-linked 'W-shredders', which would suppress populations by cleaving the W chromosome and causing females to produce only sons, as well as Z-linked female-sterilizing gene drives. Here, I use eco-evolutionary simulations to evaluate the potential of

Unprecedented Synergistic Effects of Nanoscale Nutrients on Growth, Productivity of Sweet Sorghum [Sorghum bicolor (L.) Moench], and Nutrient Biofortification.

Evidence-based synergistic effects of nanoscale materials (size of <100 nm in at least one dimension) were scantly documented in agriculture at field scale. Herein, we report for the first time on effects of nanoscale zinc oxide (n-ZnO), calcium oxide (n-CaO), and magnesium oxide (n-MgO) on growth and productivity of sweet sorghum [Sorghum bicolor (L.) Moench]. A modified sol-gel method was used to prepare nanoscale materials under study. Characterization was performed using transmission and scanning electron microscopies, X-ray diffraction, and dynamic light scattering. Average sizes (25, 53.
