Colloid cyst with hyphal-like structures: A rarity that mimics actinomycosis of athe third ventricle.

Colloid cysts are histologically well defined and consist of three main components, a capsule, with an underlying epithelial layer, and a mucinous heart. In our case, we present a 35-year-old female with acute deterioration of level of consciousness. An emergent CT scan showed a cystic lesion occluding the intraventricular foramen. The lesion was endoscopically excised through a transfrontal approach. Microscopic examination of the resected specimen revealed hyphal-like structures (HLS). This rare finding was first described by Dodds and Powers in 1977 and, in its microscopic nature, it mimics

Cat scratch esophagus.

The cat scratch esophagus is an uncommon entity. The first case described in the literature of this type of lesions was published in 2007 and was located in the colon. There are two cases described in the esophagus, this has been the first detected by endoscopic capsule. It is defined by the presence of linear, erythematous, shiny and superficial breaks of the mucosa without significant hemorrhage associated. The diagnosis is morphological. Even though the etiology is unknown, it has been postulated that the main pathogenic mechanism is barotrauma secondary to insufflation. It has also been de

Hemorrhage and intestinal obstruction secondary to a Meckel's diverticulum: a case report.

We present one case of a 17-year-old male with ten-year history of anemia complaining of recurrent bloody stools, abdominal pain and fatigue for 3 months. The gastroscopy, colonoscopy, the-first-time double-balloon enteroscopy (DBE) through the anus and capsule endoscopy were performed with negative results. 99mTc-pertechnetate scan showed a round-like high radioactive concentration at the ileocecum. Double-balloon enteroscopy through the anus was performed again by a more-experienced endoscopic physician, the diverticulum was found at a distance of 50 cm to the ileocecal valve. Then a laparoto

The Approach to Occult Gastrointestinal Bleed.

Occult gastrointestinal bleeding is not visible and may present with a positive fecal occult blood test or iron deficiency anemia. Obscure bleeding can be overt or occult, with no source identified despite an appropriate diagnostic workup. A stepwise approach to this evaluation after negative upper and lower endoscopy has been shown to be cost effective. This includes repeat endoscopies if warranted, followed by video capsule endoscopy (VCE) if no obstruction is present. If the VCE is positive then specific endoscopic intervention may be possible. If negative, patients may undergo either repea

Pericardiobiliary Fistulation: A Rare Complication of Therapeutic ERCP in a Patient With IgG4-Related Sclerosing Cholangitis.

A 70-year-old man presented with acute coronary syndrome 3 weeks after plastic stent insertion for hilar biliary stricturing secondary to IgG4-related sclerosing cholangitis (IgG4-SC). Imaging demonstrated haemopericardium due to proximal migration of the plastic biliary stent through the liver capsule and diaphragm into the pericardial sac. The stent was endoscopically removed and a pericardiocentesis was performed. The patient's clinical condition rapidly improved. We illustrate an unusual but potentially serious complication that may arise from migration of a biliary stent and discuss a man

Predictive factors of portal hypertensive enteropathy exacerbations based on long-term outcomes.

BACKGROUND: Portal hypertensive enteropathy (PHE) is a small-bowel lesion observed in patients with portal hypertension. The clinical significance of endoscopic findings in PHE remains unclear. We aimed to clarify the clinical significance and predictive factors of capsule endoscopic findings in patients with PHE based on long-term outcomes. METHODS: This retrospective study enrolled 55 patients with PHE (33 males and 22 females; median age, 64 years; range, 23-87) followed for > 3 years using capsule endoscopy (CE) between February 2009 and May 2023. We evaluated the clinical factors affecti

Pilot study of cytological testing for oesophageal squamous cell dysplasia in a high-risk area in Northern Iran.

BACKGROUND: Oesophageal squamous cell carcinoma (ESCC) is a fatal disease with 5-year survival rates of <5% in Northern Iran. Oesophageal squamous dysplasia (ESD) is the precursor histologic lesion of ESCC. This pilot study was conducted to assess the feasibility, safety, and acceptability of non-endoscopic cytological examination of the oesophagus and to provide initial data on the accuracy of cytological atypia for identifying patients with ESD in this very-high-risk area. METHODS: Randomly selected asymptomatic participants of the Golestan Cohort Study were recruited. A cytological specime

Factors associated with the presence of abnormal levels of fecal calprotectin in patients with negative panenteric studies.

BACKGROUND: fecal calprotectin is a selection tool prior to endoscopic studies in patients with gastrointestinal symptoms. However, some symptomatic patients with altered fecal calprotectin will not have any endoscopic lesions. The aim of the study was to determine the factors associated with the presence of altered fecal calprotectin in patients with negative endoscopic studies of the colon and small bowel. METHODS: this was an observational, prospective study of patients with digestive symptoms. The association of different clinical factors with elevated fecal calprotectin in the absence of

Long-Term Results After Endoscopic Resection of Colloid Cysts.

BACKGROUND: Endoscopic resection of colloid cysts is a widely accepted treatment option instead of microsurgery. However, there is still a debate about a potentially higher rate of incomplete resections and recurrence. OBJECTIVE: The aim of this retrospective study was to evaluate long-term results after endoscopic removal of colloid cysts. METHODS: Twenty patients underwent endoscopic treatment in our department. Eighteen patients agreed to follow-up examinations. In 17 patients, magnetic resonance images were obtained. RESULTS: Total cyst resection was achieved in 16 procedures. In 1 pati

Spontaneous detachment of the spiral overtube from the endoscope in the upper esophagus: a case report and literature review of a rare complication during motorized spiral enteroscopy.

BACKGROUND: Motorized spiral enteroscopy (MSE) is a recent modality for diagnostic and therapeutic examination of the small bowel using a spiral-shaped overtube with an integrated electric motor attached to an endoscope. With growing evidence of its efficacy, MSE is increasingly being used by endoscopic facilities. We herein present an uncommon case of the detachment of the spiral overtube from the endoscope during MSE. CASE REPORT: We performed antegrade MSE in a 75-year-old female under general anesthesia for coagulation of multiple small-bowel arteriovenous malformations and resection of a

Translating artificial intelligence into clinical practice for gastrointestinal endoscopy: current applications and future perspectives.

Artificial intelligence (AI) has emerged as a transformative tool in gastrointestinal (GI) endoscopy, addressing challenges in detection, diagnosis, and decision-making. In upper GI endoscopy, AI supports blind spot monitoring, Helicobacter pylori diagnosis, and the identification of premalignant and malignant lesions, with high accuracy and reduced miss rates. In lower GI endoscopy, computer-aided detection improves adenoma detection, whereas computer-aided diagnosis supports "resect-and-discard" and "diagnose-and-leave" strategies. However, real-world benefits remain modest, with concerns re

Endoscope-Assisted Sutureless Intrascleral Haptic Fixation of a Posterior Chamber Intraocular Lens.

PURPOSE: In eyes with compromised capsule support, sutureless scleral fixation is a popular method of placing a posterior chamber intraocular lens (PCIOL). We describe an endoscope-assisted sutureless technique for intrascleral fixation of a 3-piece PCIOL. METHODS: Eyes of patients who had endoscope-assisted scleral-fixated intraocular lens (SFIOL) implantation were examined retrospectively. The technique comprised direct capture of the IOL haptic with a forceps through a pars plana sclerotomy with subsequent haptic fixation into scleral tunnels created with a 26-gauge needle. The endoscope w

Pancreas- and Pylorus-Preserving Duodenectomy for Advanced Familial Duodenal Polyposis.

Most patients with familial adenomatous polyposis (FAP) will develop duodenal polyps and 5% progress to cancer. Those with Spigelman stage IV have a 36% risk of cancer at 10 years. Endoscopic surveillance is necessary with local ablation for early disease. Unresectable duodenal disease and severe dysplasia are an indication for prophylactic radical surgery by pancreaticoduodenectomy or pancreas-sparing duodenectomy. Some preliminary results have shown better outcomes with duodenectomy. A 45-year-old female with FAP had restorative proctocolectomy at 24 years, desmoid of the mesentery with regr

Endoscopic intermuscular dissection of a rectal gastrointestinal stromal tumor.

BACKGROUND AND AIMS: Rectal gastrointestinal stromal tumors (GISTs) are rare, and optimal management of localized disease remains uncertain. Endoscopic intermuscular dissection (EID) is an emerging minimally invasive technique that allows en bloc removal of selected subepithelial lesions that do not extend beyond the circular layer of the muscularis propria while preserving anorectal anatomy. We present a case of a rectal GIST successfully treated with EID. A 53-year-old man undergoing high-risk colorectal cancer surveillance was found to have a subepithelial rectal lesion 1.5 cm proximal to t

Recent Advances in Artificial Intelligence for Endoscopic and Multimodal Assessment of Inflammatory Bowel Disease: A Review.

Inflammatory bowel disease (IBD) is a group of chronic inflammatory bowel disorders characterized by complex etiology and significant clinical heterogeneity. With the evolution of the "treat-to-target" (T2T) concept, progressive endpoints such as endoscopic mucosal remission, histological remission, and deep remission have become key outcomes in the management of IBD. Traditional endoscopic assessment of IBD suffers from issues such as high subjectivity, lack of consistency, limited quantitative capabilities, and reliance on specialist experience; artificial intelligence (AI) is driving the ev

Overview of Deep Learning in Gastrointestinal Endoscopy.

Artificial intelligence is likely to perform several roles currently performed by humans, and the adoption of artificial intelligence-based medicine in gastroenterology practice is expected in the near future. Medical image-based diagnoses, such as pathology, radiology, and endoscopy, are expected to be the first in the medical field to be affected by artificial intelligence. A convolutional neural network, a kind of deep-learning method with multilayer perceptrons designed to use minimal preprocessing, was recently reported as being highly beneficial in the field of endoscopy, including esoph
