Successful resuscitation of acute cardiogenic shock presenting as ventricular fibrillation via Bipella approach: a case report.

BACKGROUND: The use of mechanical circulatory support (MCS) in acute myocardial infarction and cardiogenic shock (AMICS) complicated by biventricular failure is poorly discussed in the literature. CASE SUMMARY: We present successful treatment of a 52-year-old old man presenting with AMICS following cardiac arrest and prolonged CPR via a Bipella approach for biventricular support and restoration of haemodynamic stability. DISCUSSION: This case demonstrates the importance of understanding the role of MCS in the management of cardiogenic shock; the value of the cardiac power output and pulmonar

Pathogenesis and pathophysiology of heart failure with reduced ejection fraction: translation to human studies.

Heart failure represents the end result of different pathophysiologic processes, which culminate in functional impairment. Regardless of its aetiology, the presentation of heart failure usually involves symptoms of pump failure and congestion, which forms the basis for clinical diagnosis. Pathophysiologic descriptions of heart failure with reduced ejection fraction (HFrEF) are being established. Most commonly, HFrEF is centred on a reactive model where a significant initial insult leads to reduced cardiac output, further triggering a cascade of maladaptive processes. Predisposing factors inclu

Manual vs. automatic capture management in implantable cardioverter defibrillators and cardiac resynchronization therapy defibrillators.

AIMS: The Secura ICD and Consulta CRT-D are the first defibrillators to have automatic right atrial (RA), right ventricular (RV), and left ventricular (LV) capture management (CM). Complete CM was evaluated in an implantable cardioverter defibrillator (ICD) population. METHODS AND RESULTS: Two prospective clinical studies were conducted in 28 centres in Europe and Israel. Automatic CM data were compared with manual threshold measurements, the CM applicability was determined, and adjustments to pacing outputs were analysed. In total, 160 patients [age 64.6 +/- 10.4 years, 77% male, 80 ICD and

Periodic breathing in healthy humans at exercise in hypoxia.

Periodic breathing is frequent in heart failure or ventilatory disorders during sleep, and common during sleep at high altitude, but has been rarely studied in wakefulness and during exercise. A retrospective analysis of ventilation from hypoxia exercise tests was realized in 82 healthy subjects separated into two groups with either high or low ventilatory response to hypoxia at exercise (HVRe). A fast Fourier transform spectral analysis of the breath-by-breath ventilation (V̇e) signal, O2 saturation, and end-tidal PCO2 evidenced a periodic pattern with a period of 11.1 to 12.0 s. The peak pow

Ventricular septal perforation following acute myocardial infarction in a patient with hypothyroidism--a case report.

A 61-year-old woman with profound hypothyroidism underwent surgery for a ventricular septal perforation 15 days following an acute myocardial infarction, with no preoperative thyroid replacement. Her immediate postoperative course was complicated by a low cardiac output, respiratory suppression and high output renal failure, requiring prolonged hemodynamic and respiratory support. Thyroid replacement (50 micrograms of thyronine and 100 micrograms of thyroxine daily) was given in two installments on the 2nd day after the operation. The effect of the inotropic agents increased gradually and the

Traumatic Aortopulmonary Fistula: An Acquired Type 1 Aortopulmonary Window.

We report a patient with an acquired traumatic aortopulmonary window. The patient presented with an aortopulmonary fistula between the proximal ascending aorta and pulmonary trunk, which was missed on the initial hospital admission. The 26-year-old patient presented with high-output cardiac failure and examination features of a diastolic runoff. Patch closure of the defect using a sandwich technique was undertaken, with resolution of symptoms.

Femoral artery complications after diagnostic procedures.

Of 36 cases of arterial complications after femoral artery puncture 11 were excessive haemorrhage and 25 femoral artery occlusions. Soft arteries are more likely to tear than atheromatous ones, and this had occurred in 9 of the 11 patients. A high puncture, soft arteries, and a low cardiac output are important factors predisposing to thrombosis. An aggressive policy is recommended for treatment of both conditions.

What is the significance of intra-operative oliguria with regard to post-operative acute kidney injury? A physiology-informed framework for intra-operative oliguria management.

BACKGROUND: Intra-operative oliguria is frequently interpreted as a surrogate for hypovolaemia or impaired renal perfusion, often prompting reflexive and potentially harmful fluid administration. However, urine output during anaesthesia is influenced by a complex interplay of neurohormonal activation, surgical stress, and positive pressure ventilation, which can decouple diuresis from actual renal blood flow. This perspective articles critically appraises the evidence linking intra-operative oliguria to acute kidney injury (AKI) and explores the physiological pitfalls of interpreting urine out

The effects of acute temperature change and digestive status on in situ cardiac function in mahi-mahi (Coryphaena hippurus).

In this study, we investigated the effect of acute increases in temperature on cardiovascular function of mahi-mahi (Coryphaena hippurus). We also describe, for the first time, an artery that supplies the gastrointestinal tract that originates from the fourth branchial artery. We used vascular casting to verify the anatomical location of this unique celiaco-mesenteric artery. We predicted that blood flow in this vessel would be correlated with the digestive state of the animal. Increasing water temperature from 25.0 to 30.5 °C resulted in a linear increase in heart rate (fH) from 165 ± 4 beats

The Variation of Hemodynamic Parameters Through PiCCO in the Early Stage After Severe Burns.

To investigate early hemodynamics of severely burned patients via PiCCO and to discuss clinical significance of hemodynamic monitoring for burn shock resuscitation, 55 extensive burn patients were enrolled in this retrospective study. The fluid resuscitation was guided according to Chinese General Formula and adjusted with urinary output of 0.5-1.0 ml/h/kg as a resuscitation goal. All patients were diagnosed within a relatively stable condition during burn shock stage, and they received PiCCO monitoring within 6 hours after burn. The preload parameter intrathoracic blood volume index was low a

The upper limit of alveolar capillary recruitment in a young man with lung growth impairment.

In order to obtain further insight into the adaptive mechanisms relating to gas exchange in anatomically small lungs, tests of mechanical lung function and gas exchange were made in an active young man, whose lung growth had been severely impaired due to pectus excavatum developed in childhood. We found our patient to have small (total lung capacity, 59% of predicted) but mechanically normal lungs. He had a normal cardiac output, a normal single-breath diffusing capacity (100% pred), and a high diffusion coefficient (148% pred) associated with a high pulmonary capillary blood volume (131% pred

Intraoperative cardiac function assessment by transesophageal echocardiography versus FloTrac/Vigileo™ system during pectus excavatum surgical repair.

BACKGROUND: Pectus excavatum (PE), a congenital deformity of the chest wall, can lead to cardiac compression and related symptoms. PE surgical repair can improve cardiac function. Intraoperative transesophageal echocardiography (TEE) has been successfully employed to assess intraoperative hemodynamic variations in patients undergoing PE repair. FloTrac/Vigileo™ system (Edwards Life-sciences Irvine, CA) (FT/V) is a minimally invasive cardiac output monitoring system. This retrospective study aimed to assess hemodynamic changes in surgical repair of PE using FT/V and concordance with parameters

Current concepts in diagnosis and perioperative management of carcinoid heart disease.

Carcinoid tumors are neuroendocrine tumors with a very unpredictable clinical behavior. In the setting of hepatic metastases, the tumor's release of bioactive substances into the systemic circulation results in carcinoid syndrome: a constellation of symptoms among which cutaneous flushing, gastrointestinal hypermotility, and cardiac involvement are the most prominent. Cardiac manifestations, also known as carcinoid heart disease, are secondary to a severe fibrotic reaction which frequently involves the right-sided valves and may extend towards the subvalvular apparatus leading to valve thicken

Ultrasound dilution cardiac output and echocardiography findings in anesthetized mature alpacas (Vicugna pacos) during normotension, hypotension and hypertension.

Alpacas (Vicugna pacos) have physiologic adaptations to live at high altitude. These adaptations may result in unexpected responses to changes in cardiac performance and blood pressure during general anesthesia. There are few studies evaluating cardiovascular variables in anesthetized alpacas. The purpose of this study was to report cardiovascular performance in anesthetized mature alpacas during normotension, hypotension, and hypertension using ultrasound dilution and echocardiography. Six adult alpacas, 3 females and 3 castrated males, weighing 62.6 to 88.7 kg were anesthetized and maintaine

Postoperative heart failure after stage 1 palliative surgery for single ventricle cardiac disease.

Outcomes for patients with single ventricle congenital heart disease (SV-CHD) continue to improve over time. However, the prognosis for patients who develop heart failure immediately after surgery is poorly understood. We conducted a single-center, retrospective cohort study of patients with SV-CHD, who suffered postoperative heart failure. Of 1038 cardiac surgeries performed on 621 SV-CHD patients between 2004 and 2010, 125 patients met inclusion criteria, including non-septatable anatomy, stage 1 surgery, and verified low cardiac output or heart failure state per STS definition. Overall surv

[Anesthetic aspects of heart tamponade due to amebic liver abscess. A case report].

The problem of tamponade due to rupture of an amoebic abscess, although rare, is extremely complex, and in the planning of the anaesthetic technique the following factors should receive attention: (i) the low cardiac output in which bradycardia, vasodilation, high inflation pressures and excessive fluid load must be avoided: (ii) pulmonary complications in which a double-lumen endotracheal tube should be considered; and (iii) sepsis.

Reduced ventricular excitability causes atrioventricular block and depression of heart rate in fish at critically high temperatures.

At critically high temperature, cardiac output in fish collapses as a result of depression of heart rate (bradycardia). However, the cause of bradycardia remains unresolved. To investigate this, rainbow trout (Oncorhynchus mykiss; acclimated at 12°C) were exposed to acute warming while electrocardiograms were recorded. From 12°C to 25.3°C, electrical excitation between different parts of the heart was coordinated, but above 25.3°C, atrial and ventricular beating rates became partly dissociated because of 2:1 atrioventricular (AV) block. With further warming, atrial rate increased to a peak val

Exogenous ketosis does not alter oxygenation, oxygen uptake kinetics, or whole-body efficiency during submaximal exercise in early high-altitude acclimatization.

Exogenous ketosis, induced via ketone monoester (KE) ingestion, has been shown to attenuate hypoxia-induced blood, muscle, and brain deoxygenation and augment oxygen uptake (V̇o2) under acute normobaric hypoxia. However, its effects on exercise responses during early acclimatization at terrestrial high-altitude remain unexplored. Thirty-four healthy, active adults completed four exercise sessions: one near sea level and then once per day during a 3 day altitude sojourn (3,375 m), with regular KE or placebo ingestion. Pulmonary gas exchange, minute ventilation, cardiac output, pulse oxygen satu
